Questions: • Thoughts on the best means for patient extraction in this situation? • I also am kind of dubious on the removal of boots and Kevlar in this situation. I can make a case for a more complete visual patient evaluation, but I also feel like you’re still in a really unsecure environment, no obvious exanguination to the patient’s foot, so in this instance, I’m kind of into leaving the boot on — it’s holding the foot together, possibly, and adding a bit of protection if we end up dragging and moving the patient. I also am making an assumption that the blast partially cauterized the foot or at least some vascular/arterial retraction that’s tamponading. Something to keep on your radar during movement of it does start exanguinating. Also, the Kevlar, in the absence of other sympomolgy, shortness of breath, thoracic bleeding, etc, I might leave it on until we can get to a better point of cover/concealment…just a better spot to evaluate. Caveats, maybe there is nowhere to go, so just get it done. Also, don’t know what preceded this, what followed the event, so TONS of context missing. This is exactly what NOT to do if you enter a minefield. These Russians find themselves in a mine field and took a casualty. They call for a litter team who enter the field from another direction and the litter team hits another mine.
