In this operative video, Dr. Cohen demonstrates anastomosis of the STA and MCA for low flow revascularization. The distal end of the donor STA is prepared by removing the surrounding soft tissue. Once the donor and recipient vessels are approximated, a ‘fish mouth’ technique can be used to maximize the surface area available for anastomosis. In this instance, an interrupted suture technique was used, given its ability to prevent stenosis at the level of the anastomosis.
