SOLAS Safe Mooring. During mooring incident, often lines are parted due to tensile overload even though the load being exerted is far less than specified minimum breaking load. Two most important findings are due to unfavorable mooring arrangement and undetected rope degradation condition. It is true, there are redundant steering systems, that puff of black smoke was the emergency generator kicking on and power is shifted simultaneously through an ABT. Steering is controlled by electrohydraulic pumps. Ships crews frequently conduct exercises to train on shifting their steering control from primary to secondary just for these casualty situations. What is odd to me is the sudden hard starboard shift of the rudder to starboard, makes zero sense. Another oddity is that the ship was not flanked by tugs (who are in radio communication with the embarked pilot), this is standard protocol when approaching structures such as bridges. I have navigated on ships throughout harbors, channels and rivers around the world. There are standard practices that are international. No tugs, the unexplained rudder shift (especially when the ship is under restricted maneuvering procedures which is, once again standard when you are in restricted navigation waters such as harbors and rivers. The bridge at a minimum would have the Pilot, Master, Navigator, Helmsman/LeeHelmsman, Lookouts and likely the 1st and 2nd Mates on the bridge. The chief engineer would be in or near the engine room with emergency backup equipment in ready standby (part of the restricted maneuvering manning plan). Sure accidents happen, was this a perfect storm moment, perhaps, but I don’t think so. The extreme rudder angle shift to starboard under restricted maneuvering i at the precise worst and unrecoverable moment, lack of tugs… It stinks.
