At Providence, we provide care to over 1.2 million unique patients per year via our virtual care and digital health services. Lack of continued support for these critical access points in our care delivery system would create complete chaos and negatively impact patient access, clinical quality and #clinician burnout! The integrated neighbourhood team model imagines a system where patients access a wide range of care services through clear, easy-to-navigate care pathways. By streamlining patient access, we reduce the pressure on services and create more capacity for planned care tailored around local and community needs. Our hub-model of triage helps PCNs to provide patients with a single point of access to care across a whole network, allowing triage decisions to be made centrally while also providing the data needed to inform long-term resource and budget decisions.
