🔴Diaphragmatic Hernia🔴 🔴SOFY intact rescued tom cat, with unclear history suggested to have a blunt trauma. 🔴C.S: Anorexia, mild cyanosis, reluctant to move, weight loss, abdominal pain, dyspnea, labored breathing, tachycardia. 🔴Physical ex: •Palpation revealed empty flank region (viscera at the chest). • Elevated HR & RR with muffled heart sounds and weak pulses. • Auscultation reveal borborygmal sound at chest. 🔴Laboratory Findings: Elevated ALT was the most marked finding. 🔴Diagnostic Imaging: X-ray: revealed empty abdomen with multiple radiographic densities within the chest (herniated viscera), loss of diaphragm outline. 🔴Final Diagnosis: Diaphragmatic hernia with communication between peritoneal and pericardial cavities and allowing herniation of cranial abdominal organs and omentum into the pericardial sac. 🔴Treatment Overview: Surgical correction 🔴Surgical approach: The animal is placed in dorsal recumbency. •Make a ventral midline abdominal incision. •Enlarge the diaphragmatic defect if necessary. •Replace the abdominal organs in the abdominal cavity. •If adhesions are present, gently dissect the tissues from the thoracic structures, resecting or debriding necrotic tissue as necessary. •Debride the edges of the defect and close in a simple continuous suture pattern. •Remove air from the pericardial sac or pleural cavity or both after closing the defect to retain the normal negative pressure within the chest. If pneumothorax or effusion is likely, insert a chest tube. •Close the abdominal muscles and in cross mattress interrupted pattern, s/c in simple continuous pattern, and finally skin in cross mattress interrupted pattern. It helps keep the heart in place and prevents friction as the heart beats. Pericarditis is the inflammation of the pericardium. It can cause sharp chest pain due to the layers rubbing against each other. Constrictive pericarditis is a chronic condition where the pericardium becomes thickened, scarred, and less flexible, leading to restricted heart movement. This limits the heart's ability to expand properly during filling (diastole), reducing its efficiency in pumping blood. Causes: Recurrent or chronic pericarditis. Radiation therapy to the chest. Heart surgery or trauma. Infections, such as tuberculosis. Symptoms: Fatigue and weakness. Swelling in the legs or abdomen (due to fluid buildup). Shortness of breath, especially during physical activity. Difficulty breathing while lying flat (orthopnea). Treatment: Diuretics to manage fluid buildup. Anti-inflammatory medications. Sometimes it’s called SCIWORA, sometimes “post-traumatic central cord syndrome on the background of congenital stenosis” and sometimes it’s just spinal cord trauma usually arised after a devastating motor vehicle collision injury or fall (not necessarily from height) with mild to moderate pre-existent neck kyphotic deformity. No matter what’s the correct term and diagnosis, whenever the case is about incomplete spinal cord injury, that always warrants early and aggressive intervention to improve functional outcome. Deformity warrants 360 stabilization. Just decompression leads to dorsal cord shifting and postop. worsening. This is case of a middle-aged patient post. bicycle accident that according to the patient’s sayings and descriptions I assume hyperflexion plus axial loading has been the mechanism of injury that took place. Patient presented (transferred via ambulance) at ER with incomplete quadriplegia. I recall our attending nurses carrying the patient to get an urgent MRI & CT scan. Patient was operated all night long and recovered remarkably well and eventually became independent but still with diffuse and patchy dysesthesias. 1st stage: ACDF C3/4 & C5 corpectomy - fusion with expandable Ti cage plus anterior plating. Aggresive Uncus depression carried out 2nd stage: C3-6 lateral mass & C7-T1 pedicle screw fixation. Multilevel wide laminectomy (all done in one session) Steroids administered even though debatable literature. CARDIAC TAMPONADE Cardiac tamponade describes a heart that has so much fluid around it that it can’t pump enough blood. The force of this fluid makes it hard for your heart to do its job. This is a medical emergency. A healthcare provider has to remove the extra fluid with a needle or surgery. What happens in cardiac tamponade:Inside your chest, a sac called the pericardium surrounds your heart. The pericardium has two layers and pericardial fluid that holds your heart in place. How cardiac tamponade relates to pericardial effusion: is the technical term for when the pericardium is filling up with fluid. Effusions can consist of blood, discharge and other blood component products. A pericardial effusion doesn’t always cause cardiac tamponade, especially when the fluid builds up slowly over time. This is because it has time to stretch and make more room for fluid, and the process may not cause symptoms. Symptoms and Causes:1.sharp pain in chest 2.trouble breathing 3.faiting dizziness or lightheadedness 4.Changes in skin color, especially pale, gray or blue-tinted skin. Prevention:Because either diseases or injuries can cause it, cardiac tamponade is extremely difficult to predict and prevent.
