What a rich repository of wisdom on #patientexperience! So much resonated for me! Of the 87 leaders, this one from Garrett A. South, MPH, CPXP hit home the most: "As the executive with oversight of patient experience at my hospital, this question is at the forefront of my mind every single day. This year, I am focusing on what I believe to be the future of the #humanexperience in healthcare: fostering human connections. Healthcare is fundamentally about human beings caring for other human beings, and the key to enhancing patient experience lies in deepening these connections and fostering a sense of belonging. "Emerging from the pandemic, we are acutely aware of the epidemic of social isolation that has afflicted our society and its detrimental effects. Since everyone interacts with the healthcare system at some point in their lives, we see it as our responsibility to combat this isolation by creating meaningful connections and a sense of belonging. As a safety net hospital, there is an even greater sense of urgency to nurture these concepts for the diverse community we serve. Many of our patients and their families have been marginalized or disenfranchised due to systemic racism and historical biases, leading them to believe they don’t belong and can’t connect. The goal is to counteract this narrative and foster a community where everyone feels they belong, incorporating health equity into all our experience efforts. "We are also broadening our perspective from patient experience to consumer experience. This involves adding a consumer lens, a health equity lens, and a patient advocate lens to all our decision-making processes. Great to be speaking alongside my friend Lawrence Hamilton, Vice President of Kaiser Permanete, at a packed CHN (Cambridge Health Network) event. The theme focussed on successful models of integrated care around the world and the debate, with such a full house, was lively. Of course, given the new 10 Year Plan and recent edicts on ICBs in the NHS, comments were vocal! However, we kept coming back to three central themes: data and digital; workforce agility and medical staff; and, innovation and experimentation at scale. Put simply, population health cannot be meaningfully sustained without intelligent data, segmentation and stratification. Traditionally, we seem to have spent more time on structures than the best integrators: people & pathways. Second, isn’t it about time consultant medical staff & general practitioners get together in groups to streamline chronic disease management and much more? Kaiser, over 75 years, has refined the model where generalists & hospitalists, collectively, give their best to patients. Third, how can the government advance on policy and practice, simultaneously? Given the anticipated shift to prevention, public health and primary care, some large scale experimentation is crucial. We monitor ER visits, documentation rates, hospitalizations, falls, medication errors—you name it. This data-driven approach allows us to identify trends and make informed decisions to enhance resident care. In just one month of implementing AllieHealth across our portfolio, we saw a remarkable 14.1% increase in Solera’s documentation rate. At several of our communities, we've also achieved a reduction in hospitalizations, thanks to partnership with CarePredict, Inc. By flagging team members about residents at risk for falls and UTIs, we've been able to intervene early and prevent serious health issues.
