We have this thing at Helixiora BV where we do a Reflection Log every Friday. It's automated using Geekbot, which we use for daily stand-ups and such. It comes every Friday afternoon and asks you about a mistake you made, something you learned, and someone you want to give kudos to (that bit was added in version 2 as requested by people). I was doubtful it would pick up because talking about a mistake publicly just isn't comfortable. But I was surprised to see it get adopted and it helped maintain a culture of transparency and openness. Have you ever done something to deliberately strengthen an element of company culture?
