Being discharged from a hospital, in its most basic form, is the transition or movement of a patient from one level of care to another. Sounds easy enough, right? Unfortunately, it’s often a confusing and overwhelming process as the patient and/or family members scramble to make arrangements. Adding to this confusion is the fact that the discharge team is often made up of many people: the doctor, the nursing staff, discharge managers, case managers, care managers, social workers, therapists, care givers, family, and of course the patient. Here are a few tips to help reduce the overwhelm of a discharge.... Think about to where you’re being discharged. There are a number of options: • Acute Rehab Facility (inpatient) • Skilled Nursing Facility (inpatient) • Assisted Living Facility • Personal home or other residence (with or without nursing care) Stop and ask yourself, is this the BEST option? Is it covered by insurance ( and for how long)? Question anything you feel is not aligning with your needs. Speak to the discharge planner or your point of contact. Ask. Questions. Always! In the process of planning a discharge, be sure to include the following questions, and take notes!!! 1. What type of care will be required once I leave this hospital? 2. Will there be a need for help with activities of daily living? (i.e. toileting, dressing, etc.) 3. How will I handle mobility issues? 4. Are there any dietary considerations? 5. Can you show me how to care for a wound? 6. Can you provide me with a list of medications and a schedule? 7. What are ALL of my discharge options? 8. Can you provide me with a list of health providers for follow-ups? 9. Can you provide a list of resources for the specific services that will be needed? 10. Is there a list of support groups to whom I can turn? 11. Is there an immediate and long-term plan in place that addresses services, supplies, and lifestyle adjustments? 12. Who do I reach out to if there’s a problem with the plan after I’ve been discharged? Although I’ve only scratched the surface, having at least some knowledge of discharge will go a long way toward smoothing the transition. Dame Cicely Saunders (1918–2005) is considered a pioneering figure in modern palliative care. Her work redefined how society approaches end-of-life care, focusing on dignity, compassion, and relief from pain. Trained as a nurse, social worker, and eventually a physician, Saunders introduced the idea that terminally ill patients needed not only physical symptom management but also emotional and spiritual support. Saunders coined the term “total pain,” which encompasses physical, psychological, social, and spiritual suffering. This concept shifted the focus of care for terminally ill patients to a more holistic approach. In 1967, she founded St. Christopher’s Hospice in London, the world’s first modern hospice, which became a model for palliative care worldwide. Through her research and teachings, she promoted palliative care as an interdisciplinary practice involving medical professionals, counselors, and family members to address all aspects of a patient’s well-being. Her influence continues to be felt globally, as palliative care has grown into a recognized specialty in healthcare systems around the world.
