The ED is indeed in crisis and not just for older Americans. I just spent 14 hours in the ED with my 87-year-old mother before she was admitted--and it took 9 of those 14 hours for her to be seen by a physician. This is not acceptable. We especially owe older patients a better ED experience. Solutions?

Falls are both serious and expensive, with significant impacts on older adults. Around 10% of falls result in injuries that force individuals to limit their activities for a day or more or require medical attention. Annually, approximately 3 million emergency department visits are due to falls among older adults, and around 1 million fall-related hospitalizations occur each year. In 2019, falls were responsible for 83% of hip fracture-related deaths and 88% of emergency visits and hospitalizations for hip fractures. Additionally, nearly 319,000 older adults are hospitalized each year for hip fractures. Falls also rank as the leading cause of traumatic brain injuries (TBI). Have you ever been forced to do chest compressions on an elderly patient because the family wanted all heroic measures done? I have and it was an extremely traumatic and frustrating experience. Especially when goals of care conversations have been held on an ongoing basis educating the family members on the reality of CPR. Deciding between full code vs DNR in long term care can be an extremely difficult decision to make. Family members are often unaware that the likelihood of an elderly resident with multiple comorbidities fully recovering after CPR is less than 1%. For the less than 1% that do "survive" their quality of life is compromised dramatically. Opting for DNR/DNI does not correlate to giving up. It avoids the need to pound on the chest of an individual that most likely will not survive the event secondary to an already compromised state of health.

By 2030, almost 1 in every 5 Americans will be 65 years or older, increasing the demand for high-quality elder care facilities. These facilities need to critically address the challenges of fall risk and mobility, to ensure the well-being and quality of life of these seniors. The CDC lists falls as the leading cause of both fatal and non-fatal injuries among older adults (Older Adult Falls Data I Fall Prevention / Injury Center I CDC, 2023) . Each year, about 1 in 4 older adults falls. 30 to 40% of these falls results in a visit to the ER, and possibly up to 21 days of hospital stay.
