So many questions... What went wrong? Was it solely the cladding issue, the evacuation strategy, the preparedness of fire and rescue services, or perhaps the policies and enforcement? Were regulations implemented correctly? Should we always have a Plan B, or even a Plan C? Was it an issue with the design, or the control during implementation? What about the maintenance of systems and regular controls? Should we merely point fingers and assign blame, or should we also look into the underlying systemic failures and interactions that contributed to the incident? There are so many aspects to consider in our increasingly complex sociotechnical system, which extends beyond regulatory requirements. Should we reassess the way we are doing things? What if this occurred at night while residents are sleeping?
