Case (44): Pneumatosis intestinalis ultrasound. Elderly pt presented with severe abdominal pain and distention US showed dilated large bowel loop with thickened hypo echoic wall and absent of vascularity on Color Doppler. contains multiple small hyperechoic foci with dirty shadow suggestive of air locales. This findings are suggestive of bowel ischemia complicated by pneumatosis intestinalis.

Kush Desai presented during VIO 2023 on managing occluded stents, why and how to fix it. Moderated by Stephen Black, joined by Nicos Labropoulos and Marie Josee van Rijn. The occlusion occurred because the inflow lesion was not properly addressed. The inflow to the stent was compromised, which led to persistent symptoms in the patient. The hypogastric vein remained open and was perfusing part of the stent through collateral veins like the obturator and pudendal veins. However, the caudal portion of the stent in the external iliac vein was occluded. The right common femoral vein was post-thrombotic and hypodense, which was confirmed by ultrasound, indicating a restricted flow contributing to the stent occlusion. The stent should have been extended further down to the profunda femoris vein to ensure better coverage and avoid occlusion, which was not done initially. Despite concerns about the profunda femoris vein, efforts were made to salvage as much flow as possible by bridging the diseased segment and hoping the profunda vein could compensate for the occlusion. #viocongress #venous #vascular

Modified percutaneous radiological gastrostomy without nasogastric access. 84y/M, advanced laryngeal malignancy. Failure to pass nasogastric tube and small french angiographic catheter (with coaxial guidewire) into the stomach under fluoroscopic and endoscopic guidance despite repeated attempts. Collapsed gastric lumen accessed by direct puncture under ultrasound guidance by targeting the intraluminal air and guidewire passed into the lumen to allow passage of catheter. Standard technique followed subsequently to place the gastrostomy tube. With Dr Akhil Baby and Dr Shubham Suryavanshi.
