PEME (Pre-Employment Medical Examination) services are undoubtedly useful so that, before facing an international assignment, expatriation, stay abroad for any other reason, long (or even short) trip, enlistment in a shipping company (passenger or cargo) or airline or even in some kind of off-shore platform, this is done having taken the greatest precautions beforehand and thus taking possible mitigating measures so that the mission is carried out with the greatest success avoiding medical complications that could arise due to a possible pre-existing medical condition.

I was recently honored with the "Credentialed Pharmacist of the Year" award, partly for my rural health advocacy but primarily for my early adoption of and advocacy for the normalisation of the "GP Pharmacist" role in Australia. These titles, along with my award, prompts reflection. Just last year, this recognition was known as the "Consultant Pharmacist of the Year." Does "#consultantpharmacist" suggest a higher level of expertise or education, or merely a different scope of tasks to other #pharmacists? And what about "#credentialedpharmacist"? What does that mean to other healthcare professionals and to patients? Spoiler: probably nothing. The nuances of healthcare titles are not just semantics though- they guide expectations and understandings of the care delivered. It's crucial for us to discuss and clarify these roles and get these titles right to enhance transparency and trust within our healthcare system. So, does my title as a "#GPPharmacist" imply a partnership or co-location with General Practitioners, or does it suggest I am both a GP and a pharmacist? It is the former, but looks like it can be misconstrued as the latter, given the attached recent local newspaper article referred to me as a “local doctor,” repeatedly using the prefix “Dr.” Now, spoiler #2, I am neither a medically trained doctor nor do I hold a PhD. This mislabeling goes beyond a simple mistake—it highlights a broader issue: the crucial distinction between the scope of practice of a profession and the individual professional's scope of practice.. and what does that mean to the patient and to other HC professionals? It’s pretty easy for me to clarify my role and skill set with the patient in front of me, but not so easy when it comes to legislation and healthcare structures. Ultimately, governance should facilitate skilled professionals working to their full scope, not restrict them, because for most patients, particularly those in the bush, the answer is straightforward: they likely do not care about the specific titles their healthcare providers hold as long as they receive person-centered, high-quality, safe, timely access to care. We need a revolution in primary health care delivery- one that unites rather than fragments, and strategically deploys the best talent for the task. A flexible and dynamic model that honours each practitioner's scope, streamlines care, and boosts every team member's impact, all while keeping the patient at the heart of every decision. I’m not married to the GP Pharmacist title, I’ll answer to anything, as long as I can use my skills to serve my community. A goal shared by us all. Bring on the Scope of Practice review, the normalisation of pharmacists using their skills wherever medicines are and the removal of barriers to all healthcare professionals working to their full potential. I don’t think I know any other profession that is so modest or understated about their professional impact…. Yep, Pharmacists, I’m talking to you. Especially #GPPharmacists who have truly had to forge their own way. But it’s changing and we here at Soar Beyond Ltd have been instrumental in facilitating this change since the creation of the i2i Network 7 years ago (okay, I’m not a pharmacist, and I’m not modest!). I completed my first set of Pharmacy First consultations last week. I worked in a busy late night pharmacy and got to see my fair share of patients. It’s brilliant that this service has opened up access to these conditions for certain patient cohorts. As a clinical safety officer and independent prescriber who has been working in acute care for a few years, I find the pharmoutcomes technology provided extremely inefficient and borderline unsafe. The pharmacy first training provided by CPPE has been brilliant, however the lowest form of risk control for patient safety is training. The highest is through changes in the technology. When the system is running (temperamental) Community pharmacists across England with a range of expertise and abilities are required to triage patients by following an awful flow chart (see photo), whilst consulting with patients and taking observations. I have worked with multiple health technology companies and risk is always higher when the technology is not fit for purpose. It would be ideal for EMIS and the NHS commissioning body to revaluate the Pharmoutcomes technology. Given the network of community pharmacies in most countries around the world, their accessibility and convenience and their reach into communities who traditionally have poor access to high-quality medical care, there is clearly a huge opportunity to build a proactive medication review service, led by pharmacists, which has the potential to improve medicines optimisation. The working patterns of community pharmacists are changing because of automation and centralisation of supply. Their role in the clinical management of prescribing is also underused. 📢 Pharmacists: Time for Unified Leadership in Our Profession If you haven’t seen it yet, last year’s review into pharmacy professional leadership highlighted a critical issue: pharmacy is a vital player in healthcare, but our leadership bodies are fragmented and uncoordinated. 🔍 Key Findings: The report emphasised the lack of collective leadership with the credibility and resources needed to shape the future of pharmacy and wider healthcare. This disjointed voice is holding us back from making the best contributions to UK healthcare. 💡 Why Does This Matter? Without a unified voice, we risk being overlooked or taken advantage of. Consider how the British Medical Association (BMA) has successfully unified doctors’ voices to advocate for better working conditions. Pharmacy needs a similar authoritative body. 🛑 Current Challenges: - Low funding for pharmacy services like Pharmacy First. - Upcoming demands on pharmacists graduating as independent prescribers in 2027. - Inadequate undergraduate funding compared to other healthcare professions. These issues are compounded by the lack of a powerful voice to advocate for us, leading to potential negative consequences for the profession. 🤔 Where’s Our BMA? Many look to the Royal Pharmaceutical Society (RPS), but it hasn’t provided the value or power we need. Membership is low, likely because it doesn’t offer enough value for money or meaningful influence. 🚀 What Needs to Change: - Stronger leadership to unify and represent all pharmacy professionals. - Active engagement with pharmacists across all sectors. - Concrete actions to show influence and build trust. This is where the role of PHARM D people also comes into place. These set of new post graduate's learn Pharmacology for 6 years and our Dr's whatsoever their degree is, learn for only 6 months. The issue is the 'EGOTISM' of the degree. Dr's will not allow these set of new people to change their prescription whereas they have more knowledge in Pharmacology. Of course Clinical Doctor are good in Anatomy and Physiology. IMA did not allow them to keep the recipe DR initially, but later Supreme Court intervened and now they can keep. I have trained the first batch of them in Apollo hospitals. They have technical knowledge but they will learn the practical knowledge in due course. India can boast about the QUALITY and KNOWLEDGE of SCIENCE in our people whether they have studied DPharm, BPharm or MPharm without an doubt. As you have emphasized, Aster through out the world have taken the best TEAM, but my request would be to have a constant traing and reputation of the Changes in the Pharma market and the Drug Administration. Unfortunately the Pharmacists I have come across are just dispensers or drug delverers. I feel like Aster should taka the lead in Training and Development of the Pharmacists in the Hospital Or stand alone pharmacies. I feel Aster under your leadership can take it to a different level. Because as they say " CHANGE THE CHANGE, OTHERWISE CHANGE WILL CHANGE YOU. ".
