How does your company pass on work experience to junior individuals? Here's the scenario... You are flying SFO to Singapore. The dispatcher informs you that there is no large weather system along your flight planned route. The TAF for SIN arrival indicated light to moderate thunderstorms for 30 - 60 minutes. The dispatcher lists 3 alternates: Paya Lebar Air Base (5 minutes), Kuala Lumpar (60 minutes) and Senai Airport (10 minutes). A fourth airport, Batam was discussed where the weather was VFR. REMF fuel was 17,000lbs (75 minutes). See image. 1. Do you accept the fuel load? If no, how much? 2. Which alternate is your primary? You take off and fuel burn was normal. Prior to descent, you brief ILS 20R. Radar paints heavy precip over SIN and FMC indicates a landing fuel of 16,400 lbs. Passing through 20k arrival says runway 20R has tailwinds greater than 10 knots and you are given holding. You assess the 3 alternates and found they too are having significant weather. Batam ATIS indicates weather is clear. You tell arrival you can only hold for one turn. 2 aircraft go around for tailwinds and 3 aircraft receive vectors to alternates. 3. Do you hold or divert? Where? You pick Batam as your alternate. As captain, you take the aircraft from the FO and fly south to Batam. 9 minutes later, the FO declares mayday fuel (landing fuel is 7,000/30 minutes). Batam arrival clears you for the VOR 22 approach. Descending through 1,600’ the winds are a tailwind from 14 knots gusting to 30. 5. Do you land or go around? You go missed approach and brief the autoland approach to 04. Short vectors puts you in heavy rain on final. Passing through 668’ you engage the autopilot and receive a NO AUTOLAND EICAS. Moments later, both localizer and glideslope go full scale deflection. 7. What do you do? You go-around. The FO informs arrival, "We must land on runway 04". Tower clears the corridor for shortend approach vectors. As you break out, the aircraft is above glide path. You lower the nose exceeding 1,400 fpm and receive “SINK RATE”. 9. What do you do? At 212’, you go around. The PM tells arrival that you will perform a tear-drop maneuver and land on runway 22. Arrival informs the crew that the wind is 330/15 and clears them to land. The flight lands safely with very little gas onboard. 1. What did you learn about decision making? Why? 2. Would you manage workload differently? How? 3. How would you fly an immediate return to airport? 4. Did CRM impact decision making? How? Our company will build training scenarios that reduce risk to your safety system.
