🔍 A common issue I often encounter in the clinic and through numerous phone inquiries is the challenge of patients with very high triglycerides. The therapy is still complex, with many unanswered questions. 📞 Last week, I received a call from a general practitioner about a patient with triglycerides over 1000 mg/dL who couldn't tolerate any fibrates. We had to urgently decide on further treatment steps. Alcohol-associated liver disease (#ALD) is a major global health concern, leading to high rates of liver-related morbidity and mortality. A new Consensus Statement by Mack Mitchell, Laura Nagy & colleagues highlights the urgent need for clinical trials to explore effective treatments that integrate approaches for both ALD and alcohol use disorder (#AUD). The guidelines, published in Nature Reviews Gastroenterology & Hepatology, delve into the complexities of treating ALD alongside AUD, emphasizing the importance of combining methodologies from both fields to enhance the quality and effectiveness of clinical trials. After an extensive literature search and discussion of expert opinion, 28 recommendations for clinical trials of alcohol use and ALD were developed, with recommendations related to definitions of alcohol use, minimum data sets, statistical considerations and more. Canada has one of the highest prevalence of irritable bowel syndrome (IBS) in the world – estimated 18% vs. 11% globally [1]. This statistic highlights the significance of understanding the underlying mechanisms contributing to IBS. Recent research has shed light on a crucial aspect of IBS pathogenesis: the lower activity of glutathione reductase in IBS patients. This enzyme plays a vital role in maintaining the balance of reduced and oxidized glutathione (GSH) in colonic cells. In IBS patients, the lower glutathione reductase activity leads to an accumulation of oxidized GSH, contributing to a heightened oxidative stress load within the colon [2]. The increased oxidative stress in IBS patients has far-reaching consequences. Not only does it impact the colonic cells directly, but it also alters the immune response within the bowel. This dysregulation of the immune system further exacerbates the pathogenesis of IBS. As healthcare professionals, it is crucial to recognize the intricate interplay between lower glutathione reductase activity, increased oxidized GSH levels, and the subsequent oxidative stress and immune dysregulation in IBS patients. By understanding these underlying mechanisms, you can develop more targeted and effective treatment strategies for your patients. 📚 Our latest article, published in Quality of Life Research, investigated the effect of ferric derisomaltose (FDI) and ferric carboxymaltose (FCM) on quality of life in individuals with inflammatory bowel disease (IBD) and iron deficiency anaemia (IDA): https://lnkd.in/eRK-rKiA 📈 The post hoc analysis of the PHOSPHARE-IBD randomized controlled trial showed significant improvements in SF-36v2 scale scores (excluding the Bodily Pain scale) and health state utility values with both FDI and FCM. However, Vitality scores showed a significantly larger improvement with FDI versus FCM (p=0.026) and the overall utility changes were completely explained by the Vitality score improvements. 💡 Patients with the smallest decrease in post-iron phosphate had significantly larger improvements in Vitality scores at each time point (p≤0.05 for all comparisons) and overall (p=0.0006), strongly suggesting that hypophosphataemia brings about fatigue that attenuates improvements in quality of life arising from the correction of anaemia.
