While watching the video below created by Unity Care NW's fabulous Planning & Communication team, I couldn't help but think about how integral the #340BProgram is to providing high quality care to ALL patients of Unity Care NW, regardless of their ability to pay. The 340B Program was created by Congress in 1992 to enable Covered Entities, such as Unity Care NW, to "stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services." First and foremost, Unity Care NW uses the 340B Program to ensure patients have access to needed medication. At the end of the day, we rest assured that patients were not turned away from the pharmacy counter because they were unable to afford their prescription. Beyond the pharmacy, #340B enables Unity Care NW to provide high quality, comprehensive services to ensure our patients have the opportunity to live their healthiest lives. The 340B Program is the safety net for the safety net. 340B Program savings "fill the gaps" of uncompensated or undercompensated care. Simply put, health centers would be hamstrung to do what we do if we did not have 340B. The 340B Program is so much more than a drug discount program. It's a lifeline for health centers and the patients we serve. #ValueCHCs #HealthCareForEveryone #FQHC #340B https://lnkd.in/gR3vw527

As a part of Aledade's recognition for National Palliative Care and Hospice Month, Dr. Josh Lowentritt, senior medical director, recently spoke with Hospice News about the mutual benefits palliative care physicians and other clinicians can have in value-based care arrangements. "Thinking about total cost of care, palliative care clinicians ideally would want to be in a value-based care model to understand and get data on where excess suffering and spending may be occurring and how they can impact it," said Lowentritt. Aledade's Comprehensive Advance Care Planning (CACP) program leads to higher quality care, better experience and increased satisfaction for both patients and their families. Clinicians achieve a better experience, as well, with 92% of physician practices reporting that Aledade’s CACP program provided a beneficial service to patients. As we reflect on the meeting co-chaired by THOR Network Foundation and The Allo Hope Foundation, a few things are clear. ✨ This meeting (and the extensive preparation that went in to it) provide an ideal model for establishing clinical care guidelines, with patients at the center of the discussion, and all stakeholders present at the table. ✨ Systematic reviews and original research are critical tools for understanding the breadth and strength of available evidence and filling gaps to establish evidence-based clinical care guidelines. ✨ These groups were able to bring about incredible progress in establishing guidelines that will benefit patients receiving blood transfusions, and those with alloimmunization and HDFN. Much of the credit goes to Phil Spinella, Molly Sherwood, Bethany Weathersby, and Katie Shanahan for elevating these issues and bringing all the pieces together. It was incredible to see your dedication and expertise throughout the discussion. We are humbled to have been a part of it, and we look forward to seeing this model deployed in other areas of medicine and across our other practice areas. #PatientCentered #ClinicalCareGuidelines #SystematicReviews #ClinicalStudies #TransfusionMedicine #HDFN

𝐌𝐚𝐤𝐞 𝐄𝐯𝐞𝐫𝐲 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐈𝐧𝐭𝐞𝐫𝐚𝐜𝐭𝐢𝐨𝐧 𝐂𝐨𝐮𝐧𝐭! 𝐋𝐢𝐬𝐭𝐞𝐧. 𝐎𝐛𝐬𝐞𝐫𝐯𝐞. 𝐃𝐨𝐜𝐮𝐦𝐞𝐧𝐭, 𝐚𝐧𝐝 𝐖𝐢𝐧 𝐟𝐨𝐫 𝐘𝐨𝐮𝐫 𝐏𝐚𝐭𝐢𝐞𝐧𝐭𝐬! (How a Nurse’s Documentation Helped Overturn a Denial) When Mr. Jones was denied another round of home infusion IVIg for Myasthenia Gravis, I was asked to handle the appeal. I initially struggled to find strong progress notes to support our case. But then I looked at the nurse’s home infusion notes, which usually include data like heart rate and medication doses, pre-medications, and other quantitative information. To my surprise, this nurse had also recorded valuable observations about Mr. Jones’s life. She noted that Mr. Jones, who cared for his sick wife and had three grandchildren at home, was the only caregiver in the family. She also mentioned his love for gardening, which his condition limited but was greatly supported by the IVIg treatments. These personal details were crucial in our appeal, and thanks to this insightful documentation, Mr. Jones’s treatment was approved. If your healthcare organization is dealing with issues like denied authorizations, operational inefficiencies, or gaps in patient care, and is experiencing patient dissatisfaction, let’s chat about how we can tackle these challenges together. POV: When you practiced Family Medicine for 20 years in a hospital-owned insurance-based system . . . and then celebrate 1 YEAR of unrushed, accessible and more personalized care for your patients in the DPC model. Practicing medicine in a way that is in line with your values, mission and Oath. We recently sent our rootsMD members a survey about their patient experience in the Direct Primary Care model. I encouraged Dr. Kuhl to read a few comments from her patients aloud . . . and the feelings bubbled over. DPC is saving primary care physicians from the moral injury of volume driven and overpriced corporate medicine. Our incentives are directly in line with those of our patients. And there is space for relationship-based care. DPC is allowing patients to receive the time, care and compassion in a physician-led model they deserve. #directprimarycare #DPC #healthcare #smallbusiness #mission #values

With the ever-increasing options in wound care provider groups and wound supply companies, SNF owners and operators are navigating the challenge of selecting partners that align with their organizational, regulatory, and clinical requirements, all while prioritizing patients' wound healing. Collaboration between these groups is vital for organizational success, cost-effective treatment decisions, and, most importantly, patient well-being. Remember, partnering with a wound provider group that doesn't collaborate with your wound supplier, and vice versa, can hinder progress. As my good friend Aaron Andersen, CWCMS wisely puts it, “everyone must play nice in the sandbox” Ultimately, the patient should always come first! #woundcare #Partnerships #costavoidance #Longtermcare #Patientadvocacy #Nursinghomes
