On Overdose Awareness Day, we reflect on the alarming rise in overdose deaths and the public health challenges that we face in combating this crisis. AOAAM WANTS YOU TO BE AWARE: A summary report has just been released of the December 11, 2023, Listening Session hosted by the Substance Abuse and Mental Health Services Administration (SAMHSA) in partnership with the U.S. Food and Drug Administration (FDA) and the National Institute on Drug Abuse (NIDA). The session's purpose was to discuss the medical need, emerging data, and barriers to accessing higher doses of buprenorphine in the context of high-potency synthetic opioid (HPSO) exposure. The meeting focused on gathering quantitative and qualitative data on the need for, effectiveness of, and safety of “high dose” buprenorphine (defined as > 24 mg of buprenorphine per day) in the treatment of opioid use disorders (OUD) to inform federal guidelines and policies. This document summarizes the recommendations and opinions of the participants and does not reflect an official position, policy, or set of recommendations from any federal or state agency. More than 300 individuals registered for the virtual listening session, with over 200 attending the Zoom meeting. Participants represented a wide range of disciplines related to OUD services, including providers, academics, researchers, and those with lived experience. Kelly Hooper's article from last summer in POLITICO is still resonating as it highlights a crucial issue for public health, national security, and economic prosperity: the opioid crisis. Rahul Gupta, Director of the White House Office of National Drug Control Policy, emphasized:   "Without policy action, 165,000 lives could be lost a year to the overdose crisis by 2025."   There is a growing demand for safer pain treatments. Here’s something to reflect on: Are we thinking about exit strategies when we start opioids? Exit strategies can take many forms—patient counseling, active monitoring, starting non-opioids, or trialing a taper. Anything that opens the door for potential opioid reduction can serve as an exit strategy. Starting an opioid without considering these strategies is essentially starting chronic opioid therapy.
