CMS has set a goal of having 100 percent of traditional Medicare beneficiaries and the vast majority of Medicaid beneficiaries in an accountable care relationship with their providers by 2030. Case management is a key strategy in helping achieve this successful alignment and is also a key strategy in value-based care (VBC) to improve patient experience, quality of care, and healthcare equity.
