My new Aviation Lessons Unlearned book (first of three) explores aviation safety realistically, using ROOT CAUSE analysis in aircraft accidents, then provides long term solutions. We - the aviation industry - can't put off safety as something politicians will fix; they've already 'fixed it' beyond all repair. We must act now.
