Nursing Productivity and Nursing Workload are two different metrics. One measures the cost of care (labor hours) divided by charges (bed/birth/etc.), which is primarily a financial indicator. The other measure is the amount of hours needed (labor hours) divided by the all the activities needed to safely care for patients (workload), which is a care delivery metric. In hospitals today, we commonly use the Nursing Productivity metric to determine how many nurses we are allocated in our budgets. However, productivity metrics and the Units of Services (UOS) we use are only snapshots of patient activity....it misses the majority of the workload of the RN. Thus, this metric leads to the major rub between the frontlines and finance. From my perspective, the main issue is we're using the wrong metric to measure care. We should be challenging the 40+ year old standard of using Nursing Productivity metrics and shift to use Nursing Workload metrics to determine the nursing levels necessary to care for patients. Don't get me wrong, Nursing Productivity metrics have their place in healthcare, as we need them to assess the cost of care compared to revenue. At the same time, I do believe it is time to we add care delivery metrics (Nursing Workload) to measure the operational staffing needs of clinical units. At the Rochester Regional Health Leadership Summit, Carmine D’Angelo and I presented a poster update on the ICU Nursing Shift Note. Last fall, we gave a presentation on the background, development & implementation of the standardized note template. Recently, we surveyed our CTICU providers where the majority agreed or strongly agreed that the note was beneficial, informed patient care, and contributed to other initiatives such as reduced device utilization. Looking at the data since implementation of the standardized note, we did see that CVC and Foley utilization rates have consistently trended downward in the CTICU. This project highlights the innovative use of existing resources to meet regulatory requirements for nurses while improving communication between disciplines, in a way that decreases workload for both providers and nurses while improving patient care. #EvolveCareGrowthForDelivery #RelentlessFocusOnQualityAndSafety #UnleashThePowerOfOurPeople #StrengthenOurCommunities

Louder for the people in the back!! 🙌 Birth is FAR MORE than a nursing productivity measure. The amount of sweat, tears, and love a labor and delivery nurse pours into their patient is truly remarkable. When I worked on the floor as a labor and delivery nurse, I came home exhausted, sweaty, sore, and covered in bodily fluids that weren't mine more times than I can count. But I also came home happy knowing I gave the best patient-centered care that I could. Patient-centered nursing care can be the difference between someone having a vaginal delivery over a cesarean section. It can be the difference in an infant successfully breastfeeding vs. switching to formula. It can be the difference of life vs. death.
