EUBREAST-01 (GBG 104) Study: De-Escalating Surgery for Triple-Negative and HER2-Positive Breast Cancer Patients The #EUBREAST01 trial explores the possibility of omitting #sentinel #lymph #node #biopsy in #patients with triple-negative or #HER2-positive breast cancer who have achieved a complete radiological and pathological response in the breast after #neoadjuvant systemic therapy (#NAST). 💡 Study Design: This is a prospective, non-randomized, single-arm surgical trial. Patients who show no signs of residual disease in the breast after therapy may skip #axillary #surgery entirely, including sentinel lymph node #biopsy, helping to reduce the invasiveness of treatment. Supported by the ELSE KRONER-FRESENIUS STIFTUNG and the EUBREAST Network, this study is aimed at transforming surgical approaches for breast cancer patients with high response rates to systemic therapy. Excited to Share Our Last Published Article on the Treatment Outcomes of Urothelial Cancer in Ukraine! Our recent population-based analysis explores the effectiveness of various treatment approaches for urothelial cancer using data from the National Cancer Registry of Ukraine. This comprehensive study covers trends in therapy and survival rates for patients with bladder cancer and upper urinary tract urothelial carcinoma. Key Findings: -Low utilization of neoadjuvant and adjuvant chemotherapy - Minimal surgical complications - Call for improved systemic treatments and a multidisciplinary approach to enhance outcomes for patients in Ukraine. The analysis offers crucial insights into systemic challenges and presents practical, easily implementable recommendations for urologists in Ukraine. 🌟 Publication highlight! 🌟 "Predictors of Early Recurrence of Oesophagogastric Adenocarcinoma After Neoadjuvant FLOT" Despite advancements in treatment, recurrence rates for oesophagogastric adenocarcinoma remain high, particularly following curative surgery. A new study by Abbienaya Dayanamby et al. dives into predictors of early recurrence in patients treated with neoadjuvant FLOT chemotherapy. 🔍 Key Findings: - 27.6% of patients experienced recurrence during follow-up, with 13.8% having early recurrence - Median survival after recurrence is 4.1 months - Independent risk factors for ER include extracapsular spread and ypN3 stage Understanding these predictors enables tailored follow-up strategies, such as more frequent imaging and targeted treatments for high-risk patients. A recent multicenter retrospective study has defined crucial lymph node resection thresholds for pancreatic cancer originating from intraductal papillary mucinous neoplasms (IPMN). The study found that resecting at least 20 lymph nodes can notably enhance survival outcomes in patients, with optimal numbers specified for various surgical procedures: 20 for pancreatoduodenectomy, 23 for distal pancreatectomy, and 25 for total pancreatectomy. These findings, published in Annals of Surgery, could refine surgical guidelines and improve prognostic accuracy in pancreatic cancer treatment. Prior studies often suggested lower thresholds; this data emphasizes a higher benchmark for effective nodal analysis. Gastric cancer (GC) patients with peritoneal metastasis (PM) often face a poor prognosis. While chemoimmunotherapy holds promise in clinical management, its efficacy and predictive biomarkers in conversion therapy for such patients remain uncertain. Recently, a retrospective evaluation of clinical outcomes that enrolled 55 GC patients with PM who underwent chemoimmunotherapy in a conversion therapy setting was conducted by Geneseeq and collaborators and published in the International Journal of Surgery. Baseline PM specimens were collected for genomic and transcriptomic profiling. 📃Scientific paper: Integrated Diagnostics and Theragnostics of Medullary Thyroid Carcinoma and Related Syndromes Abstract: Medullary thyroid cancer (MTC) is a rare neoplasm derived from calcitonin-secreting cells of the thyroid (C cells) for which surgery is the only curative treatment. Successful management of MTC depends on the patient’s condition, tumor stage, genetic background, and the medical team’s experience. This chapter provides an overview of clinical manifestations, diagnosis of MTC and Multiple Endocrine Neoplasia Type 2 (MEN2) and also presents applications of the most up-to-date imaging modalities to MTC with special emphasis on ^18F-FDOPA PET/CT. It aims to guide physicians toward a rationale for the use of imaging prior to prophylactic thyroidectomy, reoperations for persistent/recurrent disease and prior to initial surgery. This chapter also concludes that, in the near future, it is expected that these patients will indeed benefit from newly developed diagnostic and theragnostic approaches. The use of indocyanine green for lateral lymph node dissection in rectal cancer-preliminary data from an emerging procedure: a systematic review of the literature Introduction: Lateral lymph node dissection (LLND) for rectal cancer is still not a widely established technique owing to the existing controversy between Eastern and Western countries and the lack of well-designed studies. The risk of complications and the paucity of long-term oncological results are significant drawbacks for further applying this technique. The use of indocyanine green (ICG) near-infrared (NIR) fluorescence for LLND appears as a promising technique for enhancing postoperative and oncological outcomes. This review aims to evaluate the emerging role of ICG during LLND and present the benefits of its application. Results: In total, 13 studies were found eligible and analyzed for different parameters. LLND + ICG is associated with significantly increased number of harvested lateral lymph nodes (p < 0.05), minor blood loss, decreased operative time, and probably decreased urinary retention postoperatively compared with LLND-alone. Conclusions: The use of ICG fluorescence during LLND is a safe and feasible technique for balancing postoperative outcomes and the number of harvested lymph nodes.
