IRA Implementation Is in Trouble: We Need Straight Talk, Not PR from CMS Check out my latest Substack. In a STAT article, Dr. Meena Seshamani, the Director of the Center for Medicare, offered an uncompromising defense of the #InflationReductionAct and its implementation by the Centers for Medicare & Medicaid Services and U.S. Department of Health and Human Services (HHS). Published on the eve of the election, the article read, to me, like #IRA cheerleading. Though I celebrate her public service in the ultra-marathon that has been #IRA implementation under a (ridiculously) aggressive time table, I felt compelled to draft this open letter response. My bottom line? The views expressed in Director Seshamani’s article, a perfect reflection of CMS’s position, shows us the agency doesn't appreciate the challenges it faces and is disinterested in honest, candid dialogue about its extensive implementation failures. Consider the role of complexity not from the top down or the designer perspective, but from the bottom up - the patient and delivery team member perspective. You might see the increasing challenges of More to do (micromanagement, digitalization, regulation, certification) fewer team members (lower ratios) and lesser delivery team members (replacements of experienced team members by less experienced, made much worse by massive expansions of annual graduates as in NP PA and DO) ... to share the increasing complexity in many dimensions (patient, practice, community, relationships, environments, insurance quality poor made worse) ... and despite less time ... and despite disruptions of personal and professional lives ... that prevent best on on one innovation with each patient ... thus shaping moral injury, burnout, turnover, higher turnover costs, and even worse finances in practices and hospitals

New study in #HSR from et al Kaitlin Fertaly PhD assess factors impacting obstetric transport and referral processes for pregnant patients experiencing an emergency in a rural state without a perinatal regionalized system of care. WHAT IS KNOWN Perinatal regionalization coordinates regional facilities for risk-appropriate obstetric care. However, such systems are often undefined or do not exist in many US regions, especially rural areas. WHAT THIS STUDY ADDS Approximately 37% of surveyed facilities faced difficulties coordinating urgent transport for pregnant patients. Qualitative data indicated this may be due to the state's decentralized transport system. CONCLUSION A decentralized transport system in a rural state can exacerbate existing challenges faced by providers arranging transport for pregnant patients during an obstetric emergency. Primary Care should be Funded as a Common Good The recent publication in the NEJM by Dr Mandy Cohen, Director of the CDC, describes the underused and much needed synergistic power of integrating Primary healthcare and Public Health. This synergy is underpinned by the reality that Primary Care and Public Health are the “Twins Separated at Birth” with both having the common DNA that allows them to be recognized as “Common Goods”, that both benefit society as a whole.  Public Health has long been accepted and funded as a Common Good but Primary Care recognition has lagged behind until recently.  The CDC Director makes the important point that both Public Health and Primary Care need to be sufficiently funded if they are to serve the “Common Good” adequately.  Public Health has been underfunded by states and the federal government. Primary Care has been underfunded by insurers (Commercial and Federal). The clear path to increasing Public Health funding is through government budgeting. The path to increasing Primary Care funding is more complicated and must pass through the hands of many insurers and intermediaries with diverse priorities (including profit and power) that may or may not be directed at using Primary Care to bring better health to people and communities. The 2021 NASEM report “Implementing High Quality Primary Care” suggests that Primary Care is a common good and should be provided with government funding in the same manner as public health. The report offers a governmental policy framework to support, fund, define required elements and hold primary care accountable. As we navigate this framework there are a number of important issues to address, including but not limited to: What does funding primary care as a common good mean? How should the “common good” be funded; capitation, or an enhancement to current funding models? Is moving forward to fund PC as a public good a state-by-state decision process, federal decision or hybrid? Must states and/or the federal government establish non-exhausting funding mechanisms? Must funding be linked to new or existing tax revenues and how will sufficiency of current and future funding be established? Will the current insurance providers remain as intermediaries and how will the Public Good funds be paid? How will primary care accountability be established and enforced? How will PH/PC integration be assured? Physician Group Practices Accrued Large Bonuses Under Medicare’s Bundled Payment Model, 2018–20 In the May issue of Health Affairs, Sukruth Shashikumar of Brigham and Women’s Hospital and his team of coauthors examine the financial experiences of physician groups participating in Medicare’s Bundled Payments for Care Improvement Advanced Model (BPCI-A). BPCI-A aims to reward participants for reductions in spending and improved care quality. Using physician group data from performance periods between 2018-20, the researchers find that these practices “accrued large financial bonuses under the model, amounting to $421 million in incentive payments.” The researchers also conclude that higher target prices were associated with higher bonus payments. The leadership of the College of Medicine, University of Lagos, under the guidance of Professor David A. Oke, demonstrates a strong commitment to the institute's success. During my recent visit to discuss the International Longevity Summit Africa, Professor Oke and his team, including the deputy Provost, college secretary, and librarian, expressed genuine interest in the initiative. Following my presentation on longevity, they appeared enthusiastic about the prospect. I also mentioned our affiliation with Claretian University of Nigeria. During our conversation, the provost even began proposing potential courses and programs. Afrolongevity is fully prepared to collaborate with the esteemed scholars in the continent to combat biological aging and its associated conditions. We will soon announce the availability of various programs, and I assure you that all longevity programs will soon be open for study in all countries in Africa. But, in Nigeria, through collaborations with the College of Medicine, UNILAG, and Claretian University of Nigeria. Professor David A Oke, the Provost of the College of Medicine, University of Lagos, is an innovative thinker, demonstrating a willingness to expand the college's scope. He also expressed an interest in blockchain technology in healthcare, an area that our technology department at TAFFD's is currently addressing. It's gonna be massive. At the global stage, whenever the longevity community stresses on putting Africa on the global map (a la longevity), I always respond that, Africa is the cradle. All we need is to do our homework. CDC issues Health Alert Network (HAN) Health Advisory to notify healthcare providers, public health authorities and the public of mpox vaccination recommendations for travelers to areas of clade I mpox outbreaks in Central and Eastern Africa. Clinicians should also counsel all patients on risk reduction strategies. The session was moderated by Dr Grace Miheso, Chief of Party at the Johns Hopkins Center for Communication Programs, and featured insights from several experts: Patricia Nansamba Wamala (Senior Advisor, Sexual and Reproductive Health, World Health Organization Kenya) shared how the Implementing Best Practices (IBP) Network promotes knowledge-sharing and best practices in sexual and reproductive health. The network fosters collaboration to prioritize research aimed at improving sexual and reproductive health outcomes. Dr. Richard Kabanda, PhD, MPH, MBA, FAIPH (Chair, Africa Continental Community Health Technical Working Group, Africa CDC) emphasized the need for harmonized resource mobilization and partner coordination to ensure government accountability in sustaining community health. Regeru Njoroge Regeru (Medical Affairs Social & Implementation Scientist, Johnson & Johnson) highlighted the importance of data and training for community health promoters. He advocated for tailored consumer analytics and diverse messaging to achieve effective engagement. Willice Onyango (Executive Director, The Youth Cafe) stressed the need for inclusive health systems that actively engage youth, empowering this largest demographic to contribute meaningfully to health solutions. Josephine Alidri (Deputy Project Director, REACH Project, IRC) explained how the REACH Project ensures access to maternal healthcare, particularly in conflict areas, by collaborating with community leaders and community health promoters to secure life-saving vaccinations.
