Thinking “Sole Vendor”❓ Vendor consolidation has become an increasingly popular strategy, driven by large orthopedic companies, and employed by both hospitals and private practice orthopedic groups in recent years. ➤ Why the shift? Economics…the orthopedic marketplace is under growing pressure to drive down prices for the implants it provides. The cost of doing business in medicine continues to rise and margins continue to shrink. As a way to combat reductions in average selling price, large orthopedic companies have worked hard to leverage hospitals and private practice groups into agreements that tie lower prices to exclusivity on the purchasing front. The pros and cons: 👇 ➤ Single Vendor: 🔹Pro - Agreement to a single vendor will result in an immediate reduction in the cost of certain implants in a company’s portfolio. This can lead quick savings on often used implants like those related to total hip and total knee arthroplasty. 🔸Con - The financial savings realized in one category of products is often offset by a new commitment to purchase higher priced options in another category and can lead to neutralized savings or even an overall cost increase. This is especially true as new technologies enter the market. 🔹Pro - Consolidation of implant options can streamline operations. When surgical staff see the same products used case after case it’s natural to presume efficiency will improve. 🔸Con - Consolidating vendors ALWAYS requires some portion of the practicing surgeons to switch away from the implant systems they currently use and believe in. Experience suggests that getting all the players to agree on the new implant company of choice is often a deal breaker. ➤ Open Market: 🔹Pro - Competition drives pricing down. When the marketplace is opened up companies will compete for the business. If there is a genuine opportunity, i.e. a real willingness for surgeons to look at new options, there will be a substantial opportunity for price reduction. 🔸Con - More moving parts equals a greater opportunity for things to slip through the cracks. Purchasing groups/practices that struggle to stay on top of things may not realize the savings opportunities available in the open market. 🔹Pro - Access to the open market maximizes options for surgeons…solutions tailored for the specific situation. No single company regardless of size and scope has the best product offering for every surgeon and every situation. 🔸Con - Much like the single vendor option, realizing a cost savings on the open market will often require change, or at a minimum the willingness to genuinely explore different options. Imagine you’re buying a car. It has to have very specific features, the color is non-negotiable, it must come from a specific dealership, and you’re only willing to talk with a particular salesperson. A hundred years ago, the editor of JAMA, Journal of the American Medical Association had a column where he wrote about hucksters and frauds in medicine. This is his piece on The Abrams Box, which is a great example of why medical device manufacturers are held to such high standards (and hold themselves to such high standards) for safety and effectiveness. This was 100 years ago, but you'll hear echoes of The Abrams Box in today's world: Theranos' fraud, Volkswagen's emissions measurement scandal or Boeing's quality woes. Or how about Stimwave, which garnered this horrific press release from the DOJ in 2023: "Former CEO Of Medical Device Company Indicted For Creating And Selling A Fake Medical Component That Was Implanted Into Patients" It's hard enough already to create safe and effective medical devices when we are trying to do our jobs well. So it's especially important that our regulators keep up their great work looking out for the next Abrams Box, and that industry keep up its work of creating cultures of quality, self-policing and collaborating on standards and other policy mechanisms.

Joe's story is a great example of why fully implanted hearing devices matter and should be available to more patients. Joe reports hearing his young son in a whole new way and being able to continue his career as a helicopter mechanic. We look forward to following Joe and sharing more about his individual journey. People say, "the right hearing aid for the patient is the one the patient uses," well what happens with the patient who is not getting enough from their hearing aids, leaves their hearing aids in the drawer, cannot maintain their hearing aids on their own, or finds hearing aids to be problematic for one reason or another? Are they just supposed to suck it up? Spend another few thousand dollars on a new fancy hearing aid with better marketing? Should we just shame them for wanting something different? That might be what other manufacturers want you to do for obvious reasons, but we think the most important thing to do is to meet these people where they are in their hearing loss journey and not try to force them a square peg into a round hole. There are more than 40 million people in the US with significant hearing loss -- no reasonable person should think they all ought to be treated the same way. The design of a fully implanted hearing device allows patients the freedom and safety to use their device 24/7 in more situations than they could with external devices that come off at night or in other situations that are not conducive to external devices typically made (who knows where?) of plastic. Many patients anecdotally report (like Joe) enjoying their hearing experience with our Esteem device more than they did with their hearing aids. Why? You'd have to ask one of our patients (feel free to contact us if you'd like to be connected with one). By the way, Joe was not the first patient to receive the Esteem device in Alabama. We’re going to disrupt every part of the clinical workflow. Really? A start up CEO said that to me once. A different one, Jim Biggins CEO of Access Vascular, Inc. said this, If you do disrupt clinical workflows that disruption needs to be matched or exceeded by the benefit of the innovation. In other words, it’s not enough to just have a proven product benefit. You’re also on the hook for any disruption you cause anywhere else in the workflow. Be careful about disruption.

Orthopedic tray consolidation is the best process to add efficiency and cost savings to a facility! The question becomes who performs and is responsible for the consolidation (facility/rep/OEM). Hint, it should not be the rep independently. If completed how is this set sterily verified in this new configuration and what paperwork is needed to support? Tough questions. We have made it easy with pre-validated/FDA cleared joint systems. Plug and play...no easier way to dramatically increase efficiency, implement real cost savings ($200+/case) and lessen the reliance on OEM set management.

One of the advantages of the newer emerging companies in the digital surgery domain is that there is not a business to “defend” as they attempt to transition and shift owned market share to a likely shift in workflow, care team training, pricing models, and most probably competing with others who also are bringing digital surgery to the markets. Steve Bell and I discussed SS Innovations at the recent LSI event in Portugal and the opportunities for these newer players in the space. The organizations that have the opportunity to look beyond a single market solution, and create a company that brings digital solution platforms first to the market may have a slower uptake but longer term may have competitive benefits. Creating a culture is easier than changing one.

We may be witnessing the early days of the bundling of surgical and interventional platforms of robots. It may also add credence to heading to the ASC’s right now if you are going to be releasing a surgical or interventional robotic platform any time soon. Reminder to not fight a kickboxer in a ring. Drag the fight to a phone booth. The strategy that Intuitive is using that is one of the critical components to the rapid deployment of the Ion system as well as its new Davinci 5 Tower is in plain sight if you are willing to look. It is happening on a few fronts. Intuitive’s sales and clinical teams from soft tissue have paved the way for the Ion system. The bundling potential of digital surgical and interventional robotic platforms with this playbook should be horrifying to the large strategics. Stryker is feeling it on their 1788 towers reorder / upgrade plans. Whereas until the release of the DaVinci 5, there would be a brand affinity as well as technical advantage to the current stack holder incumbent in the OR. Not so fast. Even the best Stryker reps are starting to hear from hospitals, “Instead of ordering five of the 1788’s, we are going to order three, because we can utilize the tower on the incoming DaVinci 5 System”. There is concern. While Johnson & Johnson with their #Monarch System and Noah Medical with #Galaxy are trying to grab market share while Intuitive is using its existing customer base built over time through the Si, Xi, and now Davinci 5 to plow through and grab the early users. That conversation with the squad of Intuitive soft tissue and Ion sales and clinical teams together goes something like: Want to trade out or upgrade your current Intuitive soft-tissue system? Sure thing. Why don’t we trade out the three Xi’s for a few Davinci 5’s and we will throw in two Ion Systems into the deal, no charge. 1-year lease – no problem Fee per use – no problem We will come back around and renew the lease with our next amazing platform. Yes, that’s the sound of a door being slammed shut on competition. When you can dominate on the economic front, technical front, and have a stronghold in soft tissue and then start to use that playbook and a powerful lever arm into other markets, the outcomes are not too difficult to extrapolate. The emerging endoluminal and endovascular markets are even bigger than the current soft tissue robotics market. Intuitive is going there. Currently it will likely be hospital based as the economics and influence are more easily scalable there. As a competitor, how far is too far gone? When do you decide to get into the fight or not into the fight with a massively successful digital surgery / interventional robot so that you can claim excellence to battle against the incumbent? Is there a way to leverage bundling strategies from the other end on your current position with other products you sell into the hospital? How much time do you have?

We're seeing more patients becoming interested in dental implants, which has caused more dentists to expand their treatment options to include them. Here are three major trends I'm noticing starting to gain traction: 1. Guided Surgery Surgical guides improve the precision, accuracy, speed and efficiency of oral surgery while providing a less invasive surgical option for patients. 2. Nanodentistry Nanostructured implants can enhance bone-to-implant contact, improving implant longevity and reducing treatment and healing time. 3. Organic Dental Implants Featuring Stem Cell Technology Although the technology hasn't developed a way to create fully organic dental implants in humans, there has been promising progress in animals.
