Hot take on a lot of early-stage digital health companies out there: You're not a tech company. Most tech-enabled care companies I've seen over the past two years are only tech-enabled in the sense that they use telehealth as their delivery mechanism for care. There are exceptions to this, of course - companies who build computer vision and applied AI tools to better the doctor-patient experience, companies who build tooling around risk scoring and stratification to enable more personalized medicine, companies who build truly novel intakes to better route patients to the appropriate care, etc. These companies have real technology chops and, if they sell it right to the market, can get credit for this from investors. But if you're truly a telehealth provider, you need to realize now that the goal post has shifted dramatically from 2021 and you're going to be valued at maturity at something like 3x EBITDA. That's profit. Not revenue. Throw a revenue multiple out the window. That's not the world we live in if you're a virtual doctor's office. Think of how that impacts venture math. If you're raising a seed round at $15M post-money (or $12M pre-money), it means you'll have to generate something like $150-200M in annual EBITDA to be a fund returner for your seed investors. Maybe even more with dilution, since telehealth businesses are not traditionally capital efficient and will likely need to raise material additional capital to reach that scale. You need to be ridiculously profitable (probably on $800M-$1B in annual revenue since you're likely going to be a 40% gross margin business). Here's the good news: you can set yourself up well (and probably be a contrarian founder relative to your peers), by talking about EBITDA growth and how you can grow into a fund-returning outcome based on realistic mature market comps. This will stand out for the investors you're pitching, even at the early stage. This is how you can beat the malaise of the tech-enabled care market. Focus on the right metrics and the right type of growth. You don't have to have all the answers (no founder does), but you need to make sure the ship is headed in the right direction. A focus on profitability for "tech-enabled care" (*ahem* telehealth) businesses is a good first step.

But when you look at the data a better description would be more of a nibble. Depending on what source you look at the software industry represents between 5-8% of global GDP. They have either been too complex or too specialized for off the shelf software to access in any economically viable way. Someone broke down the estimated costs of a $15 Sweetgreen salad. We need these cost graphics for digital health to help future founders. Especially for tech enabled cos, the greatest cost is labor. Much of the "value created" is in curation and matching, which means the gross margins are reliant on labor arbitrage. Pay someone $X, but get paid $X+Y% per visit. The challenge with healthcare is that you often have to pick one of two business models: 1. specialized, low volume, high cost, low gross margin % 2. generalized, high volume, low cost, high gross margin % I'd argue founders should focus on gross dollars over the lifetime of the customer (like Jeff Bezos said) instead of gross margin %'s, but we'll leave that for another time. The challenge is that many tech enabled digital health cos have high cost of goods sold (labor) and moderately high product, design, and engineering budgets. We must build differentiated solutions for patients, clinicians, and clients to be out incumbents, however, we're realizing that companies are ultimately valued on the discounted value of future cash flows. And the high SG&A costs at most organizations might be inbalanced to the unit economics of the business. I'm not sure the math pencils out for everyone unfortunately. Some VC investors believe that brick and mortar - referring to a physical presence of some kind - should be avoided at all costs. Yet, the track record of clinic and hybrid care businesses in health care indicates otherwise. ==> One Medical (sold for $3.9 billion to Amazon), Oak Street (bought by CVS for $10.6 billion), Lifestance (IPO at $7.5 billion market cap), and PE-backed Premise Health (acquired by OMERS Private Equity for $1.1 billion) If a clinic is indicated as the optimal way to deliver care for that patient population, there are creative ways to build and operate them. Virtual care is no longer a stopgap but an integral part of a seamless, anytime, anywhere health ecosystem. 🌍 We're witnessing the rise of B2B2C models in remote consultations, with companies like Teladoc Health and Amwell partnering with insurance providers to integrate their services into health plans. Public sector collaborations, like eConsult Health 's partnership with the NHS in the #UK, underscore the potential for remote consultations to enhance efficiency and accessibility at a national level. The digital health landscape is evolving rapidly, with advancements in remote consultations, mental health support, at-home diagnostics, and patient monitoring. AI and generative AI transform care delivery, making it more efficient, personalized, and patient-centric. 🤖 Yet, challenges remain. While venture capital funding for digital health has stabilized after the pandemic boom, it remains below its 2021 peak. Sectors like digital therapeutics face reimbursement hurdles, and we've seen major players like Walmart and Optum close telehealth businesses. Looking ahead, we anticipate further consolidation in digital health, driven by acquisitions to create broader ecosystems encompass telemedicine, mental health, and other specialized services. As healthcare leaders, we must leverage these digital advancements to improve access, enhance patient outcomes, and drive efficiency in our healthcare systems. Friends, Q2 was a quarter of progress and learning for us at HawkTower, where we invest in early-stage startups applying breakthrough computing technologies to maximize California's natural resource markets. Our focus is on technology-enabled interventions (e.g., AI-enabled geospatial radars for lithium-ion batteries in the waste stream) for critical, market-level imperatives (e.g., disaster prevention) that can drive rapid commercial adoption (e.g., placed on every recycling truck in the world). Dan Primack had some pointed criticism for #VC yesterday in his Axios Pro Rata newsletter (a daily must read IMO 😤). In short, the model doesn't work if it can't produce exits for LPs. Don't blame public markets (which are at all-time highs) for the lack of liquidity either. 📈 💸 Instead, this is a "liquidity drought of your making" where "...swinging for the fences on every pitch, rather than taking the single or double that's available" is the only way out when you invest at "sky high valuations." 😰 "A whopping 37% of "unicorns" are being held for at least nine years by VC funds, including 13% that are past the 12-year mark." 😳 ⌛ Is he right? As we get prepared for our Angel Conference program, we run workshops to help get things lined up for the right conversations between investors and early pre-seed startups. We have a deadline coming up on August 27th for startups. One of the places that we see companies having a bit of trouble is on the customer discovery side of things. So we have a couple of workshops on that early customer discovery and associated efforts to get traction early before your MVP. As Angel Investors, we need to think with a portfolio mindset instead of investing in one or two companies to "see how it goes". We have a panel of SAC alumni talking about their perspectives on their portfolio of companies. We also know that startups don't practice pitching enough. Orb (#uncorrelated) raised $25M to continue building their pricing and billing platform that lets companies automate a range of different billing modes e.g., usage-based, subscriptions, or a mix, marry analytics and billing, collect and meter product usage data, invoice customers, generate revenue reports, draw on product usage insights to find revenue opportunities, model out alternative pricing plans (thus enabling experimentation), orchestrate changes in real time, schedule future price changes for certain customer cohorts or contract renewals, directly embed draft invoices into product billing portals, surface past-due invoices, and recover failed payments automatically. The most talked about AI x Medical startup Abridge is working to rescue doctors from the never-ending admin grind (which causes burnout and less patient face-time). Abridge's AI listens to patient visits and generates precise medical notes, slashing the clerical workload. Now, doctors can focus on what truly matters: patient care. Major health systems like Yale and Emory are already on board and raving, calling it "life-changing" and "magical." 🔷 A big reason --> Payback of the scribe tool is easy to measure. It supports 14 languages including Haitian Creole, Brazilian Portuguese, and Punjabi. An ongoing criticism of generative AI is that it still has few substantive business applications. This seems to quash that. Question: is a dedicated medical LLM needed to be successful in the space? Or will the huge foundational models (GPT-4o, Google, and Meta) ingest a plethora of medical notes and kill the competition with performance? We're dealing with a fast and furious pace, and today's edge may not be relevant tomorrow. Competitors include Microsoft-owned Nuance Healthcare , Ambience Healthcare, Nabla and Suki. mdhub (YC S24) is a clinical AI assistant for mental health clinicians, helping them run their clinics more efficiently. Mental health issues are a growing concern in America, with approximately 50 million Americans currently grappling with a mental health condition— a number increasing by 1.5 million every year. At the same time, the US is experiencing a massive shortage of 30,000 mental health clinicians in 2024, a situation costing the US health system $300 billion annually due to delayed treatment. Yet, clinicians are spending only 50% of their day with patients, while the rest of their time is consumed by admin-heavy work, delaying access to mental health care even further. With mdhub, mental health clinicians become 10x more efficient by automating tasks such as clinical documentation, drafting patient after-visit reports, pre-charting, and treatment planning. Exciting news for startups! California accounts for about 50% of all venture investments. The not-so-secret reason is that non-competes are illegal in California, so engineers at FAANGs can build competitors without fear. Today, the US Federal Trade Commission declared non-compete agreements illegal nationwide. This means that engineers at big tech companies can now leave and build their own companies without fear of violating a non-competes (legacy agreements for executives are still enforceable - talk to your lawyer), this is a step in the right direction for fostering innovation and competition across the country. each company has its own culture – will each company ultimately have their own set of #AI customized #agents? – congrats to Tabnine for launching their new Code Review Agent – devs and companies can now customize and codify their own #softwaredevelopment rules. As more of the top 1000 #softwareengineering teams participate, it's almost hard to imagine the impact. I love #investing in #extraordinary #founders!

You know I always appreciate Bill Blain’s thoughts; his recent newsletter reminded me of a wonderful New Yorker cartoon, “What Lemming’s Believe.” it is a wonderful piece of art that fits well with Bill’s recent newsletter. I do not always agree with his thinking, but I always appreciate his writing and thinking - a recommended subscription. My personal sense is that Meta and Google are terrified of what OpenAI and Microsoft are doing due to the potential impact on additional revenues (though I feel Meta and Google's reactions will eventually accelerate revenue decline as everyone will ultimately be shouting, which will mean no one will listen) - their response has been a massive investment in LLM related technology, as well as a push to give the models away (in my view) to take the oxygen out of the room for any other aspiring AI startups. Consider the parallels with Zoom. Zoom charged for video, Google’s response was to offer free video for 30 minutes, Microsoft made similiar moves with teams which they bundled in office. Meta is playing the same game with its LLM models. New entrants cannot compete with free; Meta, Google, and others will simply wait and mop up everything after the money is spent; hence, spending is a tad high right now. My personal sense is that Microsoft is very well-positioned, and I’ve never sold my stock. Why? It is a well-run and ecologically diverse company. Google and Meta are not ecologically diverse from a revenue perspective. This means that they have to fight and kill to maintain their ecological/ market niche. Any change in the environment will cause their costs to consume the host. A.K.A BlackBerry versus the iPhone. The difference per unit was a $149 dollar BES license versus no BES license for the iPhone. For a bank, that’s a $500,000 difference in license renewal cost. BlackBerry’s costs killed them, not the iPhone - specifically their Management costs. This reminds me of R. H. Donnelley (RHD), Yellow Pages; more than half a million advertisers paid $3,500 a year for space in the Yellow Pages as recently as 2006, and total circulation reached 80 million copies. The RHD CEO’s presentation to investors in March 2006 was titled “Positioned for Continued Success.” Just three years later, in May 2009, R. H. Donnelley went bankrupt. That, to me, is why Meta and Google are spending money from a gushing fire hydrant - they have one revenue source - advertising. They must kill to maintain that revenue stream; they have none to replace it. To be clear, this neither good nor bad, it simply is. An interesting article below, that demonstrates out how San Diego is punching above its weight.  And how Connect's strategy and execution over time, contintues to be central to that success. The article describes the current state of the market in Seattle.  (And i love Seattle.) It's a market that we tend to track with relative to venture fundings.  They used to kick our butts, we outraised them each of the last three years.  This despite the fact they have 3x as many funds there, and 9x the amount of resident capital there. (per pitchbook) Some key takeaways:  * They continue to compare themselves to SIlicon Valley. Instead, we leverage our proximity. *They whine there's not enough local investors (see note above - they have more than us). We bring over 200 VCs to SD annually! * They say founders are not connected with one another. We bring CEOs together regularly, in a variety of ways - private dinners and through our Springboard program. * They say they need to elevate their image on a national & international stage. Why we created and continue to build Five.Ten.Thirty (aka Inno Day).  * And the last paragraph - they need to concentrate on making their region a great place to live. Our mantra: "It's about Better, not Bigger."
