On the latest Vital Signs, Nikhil Krishnan and I sit down with Dr. Vivek Garg, the Chief Medical Officer & SVP, Primary Care at Humana as well as the Chief Medical Officer at both CenterWell and Conviva. He previously also held clinical leadership roles at CareMore, Oscar, and One Medical. Our conversation hits on Vivek’s insights around primary care, value-based care, and more: 🏥 Takeaways from building primary care programs Vivek breaks his primary care experiences into two types: 1) consumer tech-enabled services (e.g., One Medical, Oscar) and 2) full-risk care delivery for complex patient populations (e.g., CareMore, CenterWell). From the former, he recognized the importance of consumer simplification of healthcare – does this make sense for people in their everyday lives? And from the latter, he highlights the need for comprehensive, coordinated, team-based, relationship-focused care to manage complexity in the face of fragmentation. Overall, Vivek emphasizes the importance of thinking about the business model first, for which full-risk models are one solution. 🏥 Where is value-based care headed? Vivek believes that the VBC payment models are just the first step to reorienting the healthcare system towards preventive care, comprehensive general management, and outcomes. Going forward, he expects greater focus on rethinking specialty and hospital care, as those are the places where most healthcare spend resides. With this, there remains an unfilled gap for consistent quality metrics for specialty care. 🏥 Approaching partnerships for value-based care At Humana, there’s a strong focus on the evaluation of partnerships, which leads to the identification of improvement opportunities. In thinking about the partner vs. build question, Vivek asks himself, “Is there going to be enough of this thing that I think is needed in healthcare, even with all the partners I’m working with?”. If the answer is no (e.g., senior primary care), then it makes sense to also build internally while still maintaining partnerships. Additionally, Humana aims towards risk-based engagement with all of its partners, enabling it to work with everybody while delivering consistent care across all the patients it serves. Stop Optimizing, Start Reinventing For years, the healthcare industry has focused on optimizing and reengineering traditional office flows and clinical practices. Despite these efforts, doctors and nurses are more burned out, and U.S. physician productivity (patients seen per day) has declined. While EHR implementation contributed to both, the fact is that EHRs are a tool and only one component of practice design. The optimization approach has fallen short because it remained grounded in traditional office flows and healthcare incentives. Efforts often neglected new care models enabled by technology and the most critical healthcare resource – the patient. The sequence diagrams used in EHR workflows and implementation often attempt to digitize old processes rather than leverage technology for drastically better options. Old flows are frequently supported by policies meant to standardize on "best practices" – which, in reality are only current practices. An opportunity was missed when EHR use was mandated, and that opportunity is at risk again as organizations simply adopt or adapt to a new EHR instead of reinventing better healthcare delivery. Consider Amazon’s reinvention and disruption of retail. It didn't focus on improving parking, extending hours of operation, or adjusting staffing. Instead, it challenged the sacred mantra of "location, location, location." Amazon understood that in the digital world "location" isn't a geographic coordinate – it's about accessibility, satisfaction, and value for all. The focus was not on using a faster cash register or improving a single step in the process. The aim was a pragmatic reinvention of retail, utilizing available tools and processes with a plan to leverage data for continuous improvement. If you save a minute creating a note, but the next encounter remains just as long or labor-intensive, did a transformative change occur? What if, instead, we reimagined healthcare using a "Virtual First" approach on the entire encounter process, exploring opportunities to use all modes of care to facilitate the next right decision? Wouldn't this better serve patients and reduce healthcare team burnout? Reimagining allows designing improved efficiency of each encounter while making subsequent encounters increasingly more efficient. We can deliver the most efficient care which includes preventing unnecessary visits. Healthcare delivery should no longer revolve around the "encounter process" (or billable event). It shouldn't be limited by parking lots, waiting rooms, exam room numbers, or in many cases proximity to facilities. A focus on reinvention can improve access and capacity while addressing growing staffing shortages. The templates for this transformation are available. Let there be change. #VirtualFirst

Really exciting news out of the Oracle Healthcare Summit in the US this week. Oracle Healthcare is promising a new #EMR “built from the ground up” to be released in 2025 and embedding #AI across the entire clinical workflow. Seema Verma told the conference that the new platform was not “a refurbished EHR”, saying that “in this day and age, you can’t leverage modern technology by bolting new innovation to something built in the 1990s”. This is spot on and what many in the #digitalhealth community have been saying for some time. We need a #nextgenEMR designed to embed AI across the whole workflow with modern tools for automation, documentation, and tasking that uses conversational AI to navigate and create tasks and documentation, to drive down the #clinicianburnout. 100% agree with her statement: “One of today’s most important and widely used healthcare technologies, the EHR, has not lived up to its promise.'' Are We Listening to the Right Things? For over two decades, US healthcare has grappled with a quality chasm. Electronic Health Records (EHRs) were introduced to address quality by improving data availability, record sharing, and legibility. However, EHRs led to burnout and impacted patient-provider relationships, as the patient's history, needs, and care plan were lost in bloated notes with copy-paste errors. The purpose and value of notes has been lost. The importance for useful and concise notes paired with effective communication and collaboration increases as care moves more virtual and asynchronous. Clinical notes were not intended as transcripts of patient-provider interactions or for billing support. Rather, clinical notes were meant to provide a readable summary of the patient's status, concerns, and care plan while capturing diagnoses, clinical reasoning, and follow-up instructions. As the medical legal record, notes serve as a reference to improve ongoing care's efficiency and effectiveness. To reduce provider’s documentation burden, ambient listening is a current focus. However, data on its ability to do so is limited, nor does its use alone improve access or productivity without process and scheduling changes.  Unfortunately, EHRs digitized paper-based processes and added burdens. Subsequent efforts tried to “optimize” these processes and data collection instead of creating new processes enabled by technology. We should improve clinical notes in the context of creating a better process for the entire healthcare engagement and ongoing patient health journey, not just the time spent by providers creating notes. A healthcare engagement begins when a patient seeks assistance, shares information, and interacts with any staff.  Aggregating and summarizing patient information at the beginning of an engagement can alleviate time spent reviewing data in tabs (~1/3 of provider EHR time). The new process will engage patients, and support team documentation while creating notes that provide a quick reference for ongoing care through concise, legible, easily consumable elements and information. That includes capture of structured data with minimal clicks and NLP use. Using systems thinking design and leveraging AI and ML, the care engagement process can be reimagined and redesigned to deliver better outcomes with greater efficiency for all (instead of trying to repair broken EHR and billing processes that templates, copy-paste, and scribes did not fix). Value can be added throughout the care engagement process, missed care opportunities and medical errors (a key goal of EHR implementation) can be reduced. The goal is transformation not optimization. With a Virtual First mindset you can solve the right problems while achieving measurable efficiency with greater ROI. Let's not live with past digitized errors. Now that our HIT industry has spent nearly 40 years figuring out how to effectively pass data between sites, settings, and institutions of care (and while that journey isn't completely over, we're well on our way), we can finally turn and look farther down the road and consider what's next. In my view, advancing process consistency, agility, and change is that horizon. The BPM community is focused on that challenge, and having recently transitioned into HL7 as HL7 BPM, is squarely trying to take this on. With today's press release, HL7 is truly open-for-business in this space. Thanks to the OMG for nurturing this work along and for HL7 for having made our next home for this effort.
