A funny thing happens when we stub a toe or wake up in the middle of the night with stomach pain…   Our brains no longer tell us the polite thing to say. If we're lucky, we catch ourselves 2 letters in to a 4 letter word …   Our brains no longer give us reasonable choices or causes for the pain. "I'm dying right here." "The pain is never going to stop." "I'm sure I have massive tumors throughout my whole torso. It's over."   Or maybe that third helping of chili cheese-covered nachos wasn't such a stellar idea…   The same thing happens when we are in emotional or psychological pain. Pain is pain to our brains.   So, when we are stressed out… We don't do our best thinking.   When our teams are concerned about their job security… They can't do their work with normal precision.   When there's more change than anyone can handle… We resist. We fall into old habits. We pout.   It's like trying to have a rational conversation with an overtired 5-year-old. Good luck.   But that doesn't mean we give up or give in. We give grace.   1.// Remind the stressed out person (even … especially … if it's you) That stress is normal in your situation. Whatever the situation is. And that stressed brains aren't clear-minded brains. That's normal too.   2.// Take a deep breath. Or seven.   3.// Identify what needs to be done right now. What is the top priority for the next 15 minutes?   4.// Time to breathe again. Believe it or not, prioritizing takes a lot of mental energy. And when you're stressed or in pain, you don't have much.   Keep repeating the cycle of: --> breathing --> working / making progress --> giving yourself grace --> breathing again   Is the work going to take longer? Absolutely.   But forcing yourself through won't do you any favors. Give yourself the grace to recognize that some days… You've stubbed your psychological toe.   It hurts.   And it will get better.     You do hard work. You do it well. Keep it up! Being physically tired and emotionally drained can bring out the worst in anyone; we're human. As a caregiver, it's easy to forget that. Resist the urge to keep doing more, more, more. Things will only get worse. Start making changes. Start determining your “no’s”. It is hard at first but a necessary part of caregiving. Saying “yes” to other things and people you need is OK. It may take some getting used to it if you haven’t done it in a while, but enjoying your life while you are a caregiver is OK. Please do it for you and them. You’ll be surprised how much your mood changes. Things that normally upset you will not bother you as much, and you will find your time with your loved one more enjoyable. I betrayed my sadness by hiding it. My eyes, the windows to my soul, revealed the storm within. They sparkled with fake joy, but deep down, they held unspoken sorrow. One day, a dear friend of mine saw me through my act. We were in a cozy café ☕, enjoying the smell of coffee and pastries 🥐. I laughed at his jokes, but my friend knew me too well. "Aren't you tired of pretending?" he asked softly, looking right at me. I tried to laugh again, but it sounded empty. He gently held my hand. "It's okay to feel sad. It's okay to be vulnerable." My friend then shared his own story. He had hidden his sadness behind a happy mask for years, afraid of burdening others with his pain. He started to heal only when he let himself grieve and be honest about his feelings. "Sadness and happiness are two sides of the same coin," he said. "You can't fully appreciate one without the other. By hiding your sadness, you're also dulling your joy." His words touched me deeply. I realized that by pretending to be happy all the time, I was hurting myself. It's okay to have moments of sorrow, to feel pain, because it makes us stronger and more resilient. From that day on, I began to embrace my emotions, both the highs and the lows. I learned it's okay to cry 😢, to seek support from friends, and to be true to myself. By acknowledging my sorrow, I found deeper, more authentic happiness. Now, my eyes tell a different story. They still sparkle ✨, but with genuine joy, seasoned with the wisdom of facing and embracing sorrow. My friend's story taught me that true happiness comes from being honest with ourselves and others, having the courage to show vulnerability, and facing life's challenges head-on. Tips to Open Up and Share Emotions: 🌸 Find a Trusted Person: Choose someone you trust and feel safe with. 🌸 Start Small: Begin by sharing small, less vulnerable parts of your story. 🌸 Use "I" Statements: Focus on expressing your own feelings and experiences. 🌸 Be Honest: Speak truthfully about your emotions. 🌸Listen: Be open to the other person's thoughts and feelings as well. 🌸Practice Self-Compassion: Be kind to yourself and allow yourself to feel vulnerable. 🌸Take Your Time: Share at your own pace, without rushing. 🌸Seek Professional Help: If needed, talk to a therapist for a safe space and guidance. Why is there always that ONE person who has to be a PROBLEM?     You know who they are.   You cracked a knowing smile when you read the question. (And how does the adage go… "if you don’t know anyone like that, it's probably you"?)     Just kidding.     But… we know that change brings resistance. We grieve what we are losing.   Even if it's a terribly outdated EMR, we don't want the pain of changing to a new one. The devil you know is WAY easier to handle than the one you haven't met yet.     Everyone is going to respond to every change a little differently. Expect that.   It's not a problem. It just is.     𝐏𝐫𝐞𝐩𝐚𝐫𝐞 𝐰𝐢𝐭𝐡 𝐟𝐚𝐜𝐭𝐬. Know what is changing.   And what isn't.   𝐔𝐧𝐝𝐞𝐫𝐬𝐭𝐚𝐧𝐝 𝐭𝐡𝐞 𝐢𝐦𝐩𝐚𝐜𝐭. Know the pros and cons.   From their point of view.   (Far too often I hear leaders talk about the pros/cons for the system, the patient, the community… everyone but the physician or team member's POV)     𝐀𝐥𝐥𝐨𝐰 𝐞𝐯𝐞𝐫𝐲𝐨𝐧𝐞 𝐭𝐨 𝐡𝐚𝐯𝐞 𝐭𝐡𝐞𝐢𝐫 𝐟𝐞𝐞𝐥𝐢𝐧𝐠𝐬. Even you. Even them.   𝙔𝙤𝙪 𝙙𝙤𝙣'𝙩 𝙝𝙖𝙫𝙚 𝙩𝙤 𝙨𝙤𝙡𝙫𝙚 𝙛𝙚𝙚𝙡𝙞𝙣𝙜𝙨.     I know. That's the hard one. We're so good at solving everything else.     But I'll say it again… 𝙔𝙤𝙪 𝙙𝙤𝙣'𝙩 𝙝𝙖𝙫𝙚 𝙩𝙤 𝙨𝙤𝙡𝙫𝙚 𝙛𝙚𝙚𝙡𝙞𝙣𝙜𝙨.   Allow space for the feelings to happen. Grant grace for the feelings that are big. Practice and teach the regulation of feelings. And know that when they happen, they are normal.   No matter how much you practice.   Change is hard. Does your client have trouble sleeping? If so, have them try journaling for five minutes before bed. Many of my clients have had success with this method when using a timer and focusing on writing out what’s on their mind. Journaling should allow your clients to sleep easier because all their thoughts are written down and they may not be thinking about their stressors so much. Wouldn't it be great if DOING the RIGHT thing always led to the RESULT we WANT?     It's one of the biggest fallacies that our brains pick up when we're little.   Maybe it's because fairy tales always have happy endings… Maybe it's because when we ate our broccoli we got dessert…   Our brains tend to believe that when we do the RIGHT thing, the OUTCOME should MATCH our expectations.     Good things should happen to good people. Bad things should happen to bad people. Wouldn't that be easier?     ✔️ I can write a great proposal… and not get the work with that client.   ✔️ We can submit a realistic, streamlined budget… and be required to cut FTE.   ✔️ You can put together a fantastic presentation… and be told that your boss will be presenting it.   ✔️ We can work really hard… and watch the promotion go to someone else.     Each of these is disheartening. And they are all just work examples.   A diagnosis. A divorce. A friendship ended. A fire.   There are plenty of other "bad" things that happen to "good" people in the rest of life too.     Breaking this connection in our minds takes intention. Good things happen to good people. Bad things happen to good people. Good things happen to well, I don't really believe in bad people, but you follow the logic.     Things happen. That's all.     It never means that the effort wasn't worth it.   Healthcare is hard. It seems particularly hard right now.     But you are doing a great job. Everything you say or do makes perfect sense… In a given context.   Every good choice you've ever made. Every bad habit you've ever developed.   If everything you say and do and think makes sense … What about everyone else?   The same is true for them.   The challenge is that we often don't share the same context.   None of us have lived the same life, Had the same experiences, Or even had the same perspective on the same experience.   I often work with teams who are struggling to find better workflows Or to retain team members Or to create a better patient experience.   We look at the process and think, "there has to be a better way." There probably is. There's at least a different way.   There is always a path from where we are To where we want to be.   When we slow down and review the processes we've created Or the habits we've developed, We need to remember to be kind and generous.   Kind and generous toward the people who created them. Kind and generous to our younger selves.   They were doing the best they could in that moment. In that context, the decision was a good one.   Examining the context with kindness and generosity Allows us the opportunity to decide whether The context has changed.   Do we have new tools? Do we have new information? Do we have different motivations and goals? Do we have different expectations?   If something in the context has changed, can we make small Or significant Changes to the process, the habit, or even our perspective?   Remembering that it all makes perfect sense to someone Is a critical leadership skill. It affords us the opportunity to be curious, kind, and generous… To find common ground as we approach new challenges.   You are doing good things. You are doing hard work. Keep going. For those who don’t know who I am or what I stand for—let me make it clear. I am an advocate for an overhaul of the renal care system, a system poisoned by greed, profit-driven policies, and the empty promises of corporations that claim to be "patient-centered" but, in reality, prioritize their bottom line. I will not be silenced, and I will not sit idly by while this industry continues to exploit patients and care partners like myself. If you are here and you haven’t been listening, I can assure you that you will hear me soon enough. I have dedicated my advocacy to pushing for a healthcare model that puts prevention—not profits—at the forefront of chronic kidney disease management. Too many people are needlessly suffering, too many are falling into renal failure, and too many lives are being lost because of a system that’s been warped by greed. I will call out those responsible for this dysfunction, whether they’re individuals or corporations, and I will continue to be a voice for the voiceless. If you are claiming to put patients first, show us the receipts. If your true priority is not prevention, then you are part of the problem. LDOs (Fresenius Medical Care and DaVita Kidney Care) and other companies have contaminated the renal care space for too long. Their harmful policies, detached from true patient care, have set the stage for a deeply flawed, profit-hungry machine. These companies—both new and old—need to be held accountable. The Hippocratic Oath of "first, do no harm" has long been overshadowed by the need to appease shareholders and insurance companies. The industry is built on efficiency metrics and herding patients through dialysis clinics like cattle, plucking vulnerable people out of rehab centers to fill a quota, all while claiming to be "empowering" patients. I will not stand silent while mercenary authors churn out articles, supposedly “exposing” the problems of the industry, only to line their own pockets with the proceeds. Their profit-driven narratives do nothing to solve the core issue—they simply perpetuate it. I am calling for a renaissance in renal care. I speak for patients, care partners, and families who have been silenced by an industry that puts profit over prevention. And I will continue to speak with no fear of retaliation. It’s time we stop hiding like porcupines, quilled and defensive, afraid of public image or financial loss. Instead, we need to come out swinging like badgers—fierce, unafraid, and determined. We need a system that prioritizes prevention, education, and awareness over treatment for profit. If you are in this industry and are not actively fighting for that, then you are complicit in the ongoing harm. I don’t care if I offend anyone. This is my life. This is the life of hundreds of thousands of people suffering under a broken system. I will not stop until this industry recognizes that prevention, not profit, must be the priority.

People tell me they’ve had severe pain for 30+ years…. That it seems unimaginable to them that they can find their recovery. I get it. And I think the culture of how we’ve treated pain is to blame.  When we’ve treated pain from a mostly biomedical standpoint, we are relying on quick fixes, finding the correct diagnosis, looking at tissues as the issue, thinking that if we can find the problem that we can fix the problem.  So, this is what people continue on in search of. Except we are looking in all the wrong places. Correction… we aren’t looking in ALL of the places. Pain isn’t only physical. Pain is physical, psychological, and social. So when someone tells me “I’ve had severe pain for years. Thinking I can recovery from it seems fantastical”. My response…⬇️ I have helped women with decades-long complex pain experiences get back to their lives. It would be fantastical if it were a quick fix, but it isn’t. And in fact there is no reliable quick fix for chronic pain, otherwise it wouldn’t be a growing global issue. The biomedical approach hasn’t quick fixed pain. The women who benefit from my unique methodology come in with willingness, openness, and are ready for the hard work of change and to approach pain through a different lens. Change isn’t happening overnight, and it requires hard work and showing up daily with imperfect action to move toward their unique recovery. Then, and only then, will they know it “worked”. Recovery and something “working” looks different and means different things for everyone.
