Regarding HHA and PCA Training and In-Services in New York State: In addition to the initial training, HHAs and PCAs are required to participate in in-service education programs to enhance their knowledge and skills continuously. These in-service programs ensure that aides stay updated with the latest practices and regulations in home care. 6 clock hours for PCAs, and 12 clock hours for HHAs annually. What are “clock hours?” In-class, in-person training. No on-line training is counted in the State of New York by the NY State Department of Health or the New York State Education Department. Clock-hours requires the RN trainer to have the aides sign in (not electronically), with date, time, subject of in-service and RN signature. Both state agencies are clamping down on in-service matters, including the annual mandates, such as HIPAA, fire/safety/infection control, sexual harassment, etc. If OSHA, DOL or the state requires these to be performed annually, these can be counted as clock-hours. The RN has to sign off on all of this, so don’t get caught short when applying these mandatory standards. HHA/PCA training is also by clock-hours as well. Only during COVID did the state allow for virtual/online training, followed by skills competency in the classroom. New York State also does not allow on-line training either for either course. The agency and the nurse can lose their licenses. Recent on-site surveys have found many deficiencies in these areas, calling for the employee to be re-trained in the classroom. Repeat violators can lose their training approvals. Always finish the in-service with a competency quiz to assure their retained knowledge meets the content of the topic you are providing. This must be graded by the RN, counter-signed and saved in their personnel file. There is no online initial training allowed for PCAs and HHAs either.

"What matters most" This is so important for our older adult patients. As Geriatricians we should prioritise this,by every possible means.Sometimes it's difficult to go past the first hurdle of the caregivers/ family/ people on whom the older patient is dependent on. But still,it should be foremost in our mind to involve the older patient in question in discussions related to their health.Channels of communication should be kept open to understand what matters most to them,what are their goals of treatment, what is their thought process regarding their own health. For 11 consecutive years, reports like this one have found that the prices providers (hospitals, physicians, etc.) charge are the main factor driving Massachusetts’ high health care costs. While we value our hospitals and our health care institutions, it’s worth asking – what are we as taxpayers paying for, and at what cost? Individuals on Medicaid (taxpayer-funded) pay minimal to zero costs (~1/3 of the state’s population) and Medicare (taxpayer-funded) cost share is graduated based on income. But, for people with private health insurance (employer-sponsored health insurance, like Wellpoint members) health care premiums and out-of-pocket costs are taking up a significant –and growing– part of their income- between 20% to 40%. Perhaps it’s time to re-evaluate how taxpayer funds are being used to support health care and work with policymakers to reform reimbursements as means to help people stay healthy and out of hospitals.  In the meantime, below are some steps people can take to start understanding health care costs and stay healthy:   1. Think health, not sick care: eat healthy, exercise regularly, and stay active every day; 2. Take control of your health. Partner with your primary care team and be sure to get recommended preventive care and screenings; 3. Use primary care, urgent care, and community health centers for non-emergent care. These locations are the lower cost alternatives; 4. A study by UMass Chan Medical School and UMass Memorial Health revealed significant financial costs associated with emergency department (ED) boarding, especially for acute stroke patients. While ED boarding times decreased from 182 minutes in 2022 to 110 minutes in 2023, high-volume EDs (over 80,000 patients annually) still face lengthy boarding periods, with an average of 178 minutes. The study, which used a time-driven, activity-based costing approach, found that daily ED boarding costs were nearly double those of inpatient care. For example, medical/surgical boarding costs were $1,856 per day compared to $993 for inpatient care. The gap widened when accounting for travel nurses, with daily boarding costs reaching $2,258. ICU boarding also incurred 5% higher costs than inpatient ICU care. Aside from financial burdens, the study highlighted that boarding patients often receive less equitable nurse care compared to inpatients. This research underscores the urgency for hospitals to address the costly issue of ED boarding.
