Ketamine provides near-instant relief from treatment-resistant depression for nearly half of the study participants. These patients suffered from crippling depression as most suffered from severe suicidal ideation. It's so effective that ketamine infusion clinics are starting to open across the U.S. and other countries. Here's what you need to know. \ Ketamine Infusions for TRD: Ketamine is a medication used in low doses to rapidly alleviate symptoms of treatment-resistant depression (TRD). A recent study found that 3 ketamine infusions over 11 days resulted in TRD remission for 52% of people (linked below). For those who can afford it, a new route of ketamine delivery was recently developed... \ Ketamine Nasal Spray: A new nasal spray form of ketamine, called esketamine sold under the name Spravato, was recently approved by the FDA for human use. \ Disproportionate Responses to Ketamine: Researchers are identifying blood biomarkers that might differentiate ketamine responders from nonresponders. These studies might also help reveal ketamine's antidepressant mechanism of action. \ Potential Mechanisms of Action: Ketamine has been shown to reduce inflammation (1), which is associated with the future onset of depression (2). It's also a potent NMDA receptor inhibitor, shown to increase synaptic connections and block the cytotoxic effects of quinolinic acid, a neurotoxic metabolite in the kynurenine pathway (3). Its anti-depressive properties are also proposed to be caused by targeting: 1. The α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor (4) 2. metabotropic glutamate receptor (mGluR) (5) 3. opioidergic receptors (6) The rapid relief is likely attributed to a convergence of many pathways. We envision a world where the biological drivers of treatment-resistant depression are widely understood promptly diagnosed and effectively treated, including in ketamine nonresponders!
