Someone in a Facebook page I follow asked some general questions about routine equipment maintenance and whether of not it could be ignored… Reposting here…. Hope you give this a good, quick look and share with your staff. Chris Sanford #ohioddsmatch #ddsmtach Just a follow up to the equipment maintenance question….. You should always have backup gaskets for all your autoclaves on hand. They’re very easy for staff to swap out. The parts cost less if you buy them from your supplier than you’ll pay from a service technician…. NOT including the $300 per hour labor. Gaskets are very easy to replace, and only need replaced as needed, not a specific timeframe. If the gasket in a chambered autoclave becomes loose, which happens eventually after a lot of cycles, just pull it out and insert a new one. There’s no need for the practice to have to shut down or create an emergency service call. Depending on the service schedules of your dealer (you’ll call anyone on an emergency), they might not be able to get to you and there’s also no guarantee they’ll have a loaner autoclave, either. I’ve taken more autoclaves in my car to take care of my clients over the years than I care to remember but it’s an extremely valuable service to my great clients. As far as the evacuation systems… it’s critical to use a good non- enzymatic m, non-foaming solution to ensure the foam doesn’t clog the interior of your lines. Also critical is using an atomizing bucket. You don’t have to purchase the actual atomizer bucket from your supplier as long as you remember to NOT completely submerge the have into the bucket. The mixture of air and solution is critical to make sure the solution “swirls” inside the entire inside of the tubing walls. Start form the nearest room to the vacuum. Run the solution for about 30 seconds and make sure you leave the line open for an additional 30 seconds to let the solution pass all the way through the vacuum!!! Work from nearest op to the farthest following the same process to ensure optimal cleaning of the lines. You need to follow this process immediately after surgical cases during the day, but the lines should be cleaned this way every single day to maximize suction performance and to reduce the stress on your expensive the performance of your equipment and optimize it’s lifespan. Same goes for saliva ejector tubing but not as critical as your HVE. Have a tech change the oil and compressor filters periodically, usually once a year. Have a tech swap out all the orings in Handpieces, air water syringes (especially the ones in the front of the syringe- that’s the one after repeated inserting of the AW syringe tip that gets gauged and makes your syringe leak water) Autoclave cleaning should be once a week, last day of the week. Simply running solution like Speedclean is too easy to NOT do every week. Your autoclaves run all day everyday and they’re sensitive to water quality, especially Statims #ddsmatch #ohioddsmatch

Why I don't recommend pricing PRK the same as LASIK or SMILE Yesterday, at the Forefront Refractive Surgery Course, Dan Reinstein shared his approach to pricing refractive surgery procedures. These include ReLEx SMILE, LASIK, and PRK. He charges the same for all three to remove bias from the surgeon and the patient. His argument is based on sound economics. But it made me think about broader pricing principles. Dan's view aligns with Adam Smith’s model, where pricing is tied to the cost plus a margin. This model works well for commoditised products and services. For instance, PRK needs more prep and post-op care, which raises its delivery cost. From this perspective, pricing based on cost and time makes sense. Yet, in the knowledge economy, this model falls short. Adam Smith's view doesn't consider the subjective value that patients place on these procedures. Enter Carl Menger and the subjective theory of value. Menger argued that value isn’t inherent in the service or product or tied to the labour invested. Instead, they perceive value by evaluating the likelihood of achieving their dream outcome over the time it takes and the effort and sacrifice they must make. In refractive surgery, patients don't buy the surgery. They buy the life-changing results that arise AFTER the surgery. Their perception of value varies. A patient might value SMILE or LASIK's fast recovery and convenience over PRK. This is true regardless of the technical complexity involved. This highlights a critical difference between producers and expert marketers. Producers often see price through the lens of cost, while marketers understand that value is subjective. Patients are willing to pay more for procedures they see as better. They do this regardless of the cost. I respect Dan’s view on equal pricing to avoid bias. But, pricing should reflect the value patients place on each procedure. It's not about the cost of delivery but how patients value their outcomes. Avoiding bias is another problem that requires a different solution. One of the more popular implant systems is the LaserLock from BioHorizons, owned by Henry Schein. One thing this implant got right is the connection. After I sold Core-Vent in 2001, along with its patents, to Calcitek, later acquired by Zimmer (today Zimvie), BioHorizons acquired a licence from Zimmer Dental on the Screw-Vent’s connection https://lnkd.in/g7pMpwda BioHorizons switched its external hex implants to the exact same dimensions including the 45 deg. lead-in bevel and 2.5mmD internal hex. It added color coded anodizing to the platform to simplify abutment selection. BioHorizons’ Unique Selling Proposition is its very narrow horizontal grooves at the neck of the implant and also on the necks of some abutments. The claim is that the “laser lines” will improve soft and hard tissue attachment. Extending the blasted surface to the top of the implant and adding laser lines will undoubtedly also improve attachment of biofilm and plaque that has been associated with peri-implantitis.
