I am afraid to learn how this experiment in Florida will turn out. As I am concerned about delivering mothers 🤱 and their babies 👶 . Being a pediatrician and neonatologist for the past 25 years, I have seen and been involved in some heroic resuscitations and emergency stabilizing care of babies after "elective" operative deliveries in full term and near term babies. These delivery room resuscitations require: 1. a multi-disciplinary team 2. a team highly skilled in their craft of coordinating and providing neonatal resuscitation 3. significant technology 4. various medical and hospital services such as blood bank (in case there is acute hypovolemic shock due to blood loss-- which can occur with no warning and only present once a baby has exited the uterus), laboratory, radiology, pharmacy services...to name a few 5. neonatal and maternal specific equipment. 6. neonatal and maternal specific clinical care guidelines. I know how hard this is to do--- to do well-- to create and provide the best neonatal resuscitation services. I helped create a new obstetrical delivery service at a major Children's Hospital's (care for mothers and babies in a children's hospital setting, that needed to prepare to provide maternal [adult] advanced care, which often can be an emergency and require acute and very complicated care). It is not easy and requires adaptation of many hospital systems, processes, and team responses. I have been a medical director of a Neonatal ICU for 17 years and understand what it truly takes to provide the best and most effective neonatal resuscitation AND how frequently hospital systems and processes require adaptation or change to provide this best care. But..... I am sure that Florida legislatures took this into account when passing this law. I am sure that the private equity groups supporting operative deliveries outside of hospitals have considered this AND will provide all the necessary acute services needed to best support these mothers and their babies. There certainly are expanding maternity care deserts--- I am concerned this is a poor solution to this problem. Changing reimbursements models would be a more effective option with much less risk to mothers and babies. Between 5-10% of babies require resuscitation at birth and 1-2 % require heroic resuscitative efforts. This percent increases with operative deliveries without labor. I hope I am wrong. But I am skeptical that this law and decisions were made with a clear understanding of their significance. #UsingWhatWeHaveBetter

Some promising research out of Palm Beach County by Heather Howard, PhD, LICSW and her team. We are steadily learning what aspects of recovery capital promote certain outcomes, such as housing stability and recidivism. From Dr. Howard's article at ATTC: "The study tracked participants' progress through surveys using the Recovery Capital Index (RCI). This tool measures improvements in social and personal well-being over time. Participants received support from peer navigators (individuals with lived/living experience), care coordinators, and housing specialists. The program also provided financial help for rent and other necessities, reducing immediate stress and barriers to recovery. "Participants were recruited from multiple sources, including jails and community reentry programs. Data collection spanned 90 days, with follow-ups at 30-day intervals to monitor personal capital, housing stability, and rearrest rates." Findings: 🏠 Housing Stability: 76% of participants secured stable housing. Access to family support, healthcare, and a sense of purpose were key predictors of housing stability. 👮‍♀‍ Recidivism: Only 14% of participants were rearrested during the 90-day period. Those with better social capital (such as feelings of safety and community support) were less likely to be rearrested. 👥 Personal Capital: Participants who experienced improvements in social capital also showed significant gains in personal well-being.
