Walmart's thesis is fairly simple: Buy and transport products/services as cheap as you can at scale and sell them as cheap as you can to transfer most that value to the customer, so the customer can buy more, so Walmart makes a reasonable profit at scale. This thesis can gets derailed in the healthcare ecosystem in many different ways: - Pricing power is by definition unavailable - Reducing cost of operations in the areas Walmart has stores is pretty hard for medical services - Transferring value to the consumer with lower service price is meaningless for most medical services given the consumer doesn't pay and recognize the value. - Walmart's edge in retail is demanding their suppliers to give them better pricing at scale. This edge doesn't exist in healthcare services. All of these is why "Healthcare is hard!" and Walmart won't be the last big enterprise that fails in building for this industry.

Creatine has other benefits outside what most think of - muscle growth. Creatinine can play a beneficial role in injuries and immobilization. Injuries and being taken out of the game are mentally and physically hard, but being able to help reduce muscle loss is huge! If you are in a cast or immobilized, you’ll lose muscle. The longer you are inactive, the more you lose strength and size. How can you maintain muscle to the greatest extent possible if you can't lift weights or use those muscles? Eat enough protein (and calories). And use creatine. In one study, creatine helped maintain muscle mass, strength, and endurance to a greater extent than a placebo in subjects with casts on their arm. Why? Early stages of immobilization (1-5 days) show increases in muscle protein breakdown that contribute to overall atrophy. During this period, muscle strength, insulin sensitivity, and basal metabolic rate all decline. Nutrition also declines during this time due to appetite changes, limited access to healthy options, and a general lack of knowledge about preventing muscle loss. Creatine can help with muscle hypertrophy even in the absence of activity. In one study, creatine increased muscle size by 10-15% over 12 weeks in the muscle not going through rehab. For instance, if you are only doing rehab on your injured right leg, your non-injured left leg (even if you don’t do anything) will benefit from taking creatine. If you are in a cast, on crutches, or rehabbing, it makes sense to try creatine. Have you ever used creatine when having an injury? Regulatory Update with Office of Pharmaceutical Quality (OPQ) Reorganization, #CDER, FDA, over 5,000 manufacturing facilities worldwide, providing guidance and quality assurance, pharmaceutical quality, regulatory decision. FDA is announcing a webinar on the reorganization of the Center for Drug Evaluation and Research’s (#CDER) Office of Pharmaceutical Quality (#OPQ). This reorganization was implemented in January and does not result in any direct or immediate changes for external groups, including drug manufacturers. This reorganization enables OPQ to evolve and adapt to the ever-changing global healthcare landscape and: - Streamline processes and systems for enhanced efficiency, eliminating operational redundancies and bottlenecks. - Embrace new technologies and tools as the key to staying ahead in a dynamic environment. - Actively pursue innovation to make a meaningful difference in patients' lives. Adopt a culture of continuous improvement, in which OPQ is consistently seeking better and more efficient ways of working and advancing the field. Stay away from individual PVA’s (primary vendor agreements), join a group. There are other services provided in addition to the PVA discounts. Individual deal negotiations (sub 20m annually) hold barely any leverage unfortunately. Blended WAC discounts are common in individual deals but they’re not in GPO PVAs for a reason. This spreads out different WAC discounts over thousands of drugs so it’s a constant guessing game if you’re getting 0 or WAC -6. Which creates random branded drugs with much larger losses.
