The tightly packed internal components of the insertion tube and light guide tube make it susceptible to crush damage, which can lead to a ‘crushing’ defeat for both the device and your budget. There are many potential ways that crush damage can occur, but the three most common culprits identified in our data include: 1. Collision of the light/video connector and insertion tube: The light/video connector weighs as much as an orthopedic mallet. When the connector collides with the insertion tube it almost always results in some degree of crush damage. This often happens during transportation or in storage. To prevent this, use specialized transportation bins and tip protectors to hold the scope in place and reduce damage. 2. Mishandling during the procedure: Once the light guide connector is inserted into the video processor, it often becomes an afterthought. Unfortunately, accidents happen. Sometimes, the physician or assistant can accidentally back into the connector, causing the tubing to bend sharply. There have also been situations where the patient stretcher has accidentally run over the light guide tube! In both situations, the processor was positioned too close to the physician and patient. Staff should be mindful of these potential issues while setting the room up for the procedure. 3. Bite damage: Bite damage is a “smoking gun” type of damage because it is easy to identify and originates from one source – the patient. To prevent this, bite guards should always be used in procedures where a scope is fed through the patient’s mouth. Even under sedation, a patient’s gag and bite reflex remain active and can cause significant amounts of damage. The good news is all three of these crush damage culprits can be addressed. Unsurprisingly, many of the most common damage types involve some form of leaking and fluid ingression. However, our data demonstrates that while most leaks are found in the distal area, there are still many other areas where leaks can occur. A Surprising Culprit – Control Body Knobs One of those other locations for leaks is the control body knobs. The design of the knobs hides their most vulnerable point, namely the O-rings underneath the knobs, which seals the external knob from the internal angulation wires. These O-rings will wear down over time, but certain practices speed up the wear and tear that will become a leak. For example, hospital staff and technicians often lay the scope knob-side down during transportation, manual cleaning, and/or the High-Level Disinfection (HLD) process. This puts pressure from the weight of the scope on the knobs and can lead to gaps forming in the O-ring, which allows fluid to enter the scope. How Do You Solve the Problem? If your repair reports include frequent repairs of the control knobs, it’s a good indicator that somewhere in the process, a technician or other staff member is resting the scope with the control knobs down. An audit is always the best way to determine where the mistake is happening so education can be delivered to the responsible individual(s). Great case demonstration of #XOScore using low-pressure inflation to crack and dilate calcific stenosis in the left iliac artery. XO Score was used with an 8mm x 40mm off-the-shelf PTA balloon, inflated to nominal pressure, with no energy delivery. No vessel spasm, dissection, or recoil was evident post-treatment and the entire XO Score procedure time was less than 3 minutes. XO Score is indicated to treat both fibrous and calcific lesions in the peripheral vasculature as well as AV dialysis fistulae. It works with a broad range of PTA balloons, offering controlled, concentric dilatation at 1:1 vessel sizing. Unlocking Complex Hernia Repairs: New CPT Codes Reveal Top Hernia Mesh Targets For the first time, CPT codes for anterior abdominal hernia repairs are defined by the total length of the hernia defect. This update provides hernia mesh companies with valuable insights into which physicians handle more complex cases where mesh is often essential. Previously, it was difficult to differentiate between surgeons performing high-volume, low-complexity repairs and those specializing in more intricate procedures. With these new CPT codes, companies can now focus on experts like Dr. John Linn and Dr. Marco Harmaty, who are recognized for their skills in complex hernia repairs. Furthermore, this new code breakout gives further insight into the specialists performing complex hernia repair procedures.
