Caught Flat-Footed! Back in 2018, Jon McNeill, former COO at Lyft, shared a pivotal moment from their capital-raising roadshow. Their pitch deck was dialed in—loaded with impressive growth numbers, market share stats, and the kind of forward momentum that gets investors excited. But there was a hitch: when investors started asking about Lyft’s path to profitability, they found themselves flat-footed. The focus had been entirely on growth; they weren’t prepared to answer the hard questions about sustainability and profitability. Interestingly, Uber was facing the same scrutiny. This isn’t just a story about two ride-sharing giants; it’s a wake-up call for anyone trying to make a case. When you’re up in front of people, it’s not enough to be armed with just the positives. You need to anticipate objections and address concerns. Enter Red Teaming. Red teaming, though it has military roots, is the practice of rigorously challenging your own assumptions and strategies before stepping into any high-stakes situation. It’s about asking the tough questions yourself—or having a trusted group of friends, colleagues, or peers do it for you—before your audience does. What are the potential weaknesses in your argument? What concerns might someone raise? How would you respond if someone challenged your numbers or questioned your strategy? The idea isn’t just to find answers but to build a mindset that embraces resistance and leans into tough questions. Here’s how to start red teaming to prepare for your next big moment: 1. Create a “Red Team” Document: Write down the questions you fear the most or haven’t even considered. This should be a living document where you continually add and refine questions as you gain new insights or face new challenges. 2. Seek Outside Perspectives: Don’t do this alone. Ask your friends, colleagues, and industry peers to poke holes in your argument. The tougher the questions, the better prepared you’ll be. 3. Build the Muscle: Just like any skill, getting comfortable with challenging questions takes practice. Treat every resistance as a chance to sharpen your thinking and refine your response. The goal isn’t always to have the perfect answer but to show you’ve thought critically about potential concerns. 4. Be Present in the Moment: When you’re prepared for resistance, you’re not scrambling for answers. You can stay calm, listen to what’s being asked, and respond with confidence—even if that means admitting you don’t know something but demonstrating how you’ll find the answer. If a company like Lyft can get caught flat-footed, anyone can. The real takeaway is this: you don’t need to fear the hard questions. Instead, use them as a tool to build your resilience and readiness. Don’t just prepare for the pitch—prepare for the pushback. That’s where real confidence is built. Get ahead of the curve. Try red teaming and see how your preparation translates into powerful, poised responses when it matters most.

The (Healthcare) Ground Beneath Our Feet… This year it is likely that over $10 billion will be invested in the digital health sector – a robust amount, no doubt, but inconsequential when compared to the amount of capital tied up in healthcare real estate assets, which is estimated to be $1.2 trillion by Jones Lang Lasalle. Of that amount, approximately $790 billion is in the hospital sector while the remaining $490 billion is in medical office assets. It is estimated that there are 48.6k facilities with 3.4 billion square feet of healthcare real estate, 42% of which are owned by REITs and another 33% held by other private investment vehicles. Much of the digital health investment is meant to make the physical healthcare infrastructure more productive: greater operating efficiencies, greater patient throughput, more relevant personalized care models with better outcomes. As the healthcare system is being pushed to be more distributed to lower cost sites of care, accelerated now by the rapid proliferation of automation and AI capabilities, the potential impact on healthcare infrastructure assets will be profound. A new era of deep tech has emerged. First time funds will raise “unheard of” amounts of capital to fuel next gen deep tech startups - producing outsized, superior returns for LP’s compared to the rest. Prudent investors will act on this data and shift investment strategy as LP’s or risk being left behind savvy wealth managers and CIO’s / FO’s who saw this trend begin 4 years ago.

In addition to building our own VC practice at AAF Management Ltd., we’ve invested in 37 VCs since 2017. Here is what, I believe, it takes to build a multi-vintage, early-stage, venture franchise: 𝟭- 𝗦𝗵𝗼𝘄 𝗙𝗼𝗰𝘂𝘀: hyper-focused GPs build longevity and consistency in their businesses. If you pitch LPs saying you will invest in early stage consumer but end up doing late stage consumer that’s a massive red flag. Stay germane to your mandate and strategy. 𝟮- 𝗔𝗿𝘁𝗶𝗰𝘂𝗹𝗮𝘁𝗲 𝗮 𝗨𝗻𝗶𝗾𝘂𝗲 𝗩𝗮𝗹𝘂𝗲 𝗣𝗿𝗼𝗽𝗼𝘀𝗶𝘁𝗶𝗼𝗻: the market is crowded with venture firms, articulate your differentiated value proposition to LPs and founders. Don’t try to be everything to everyone. Focus on 1-2 value propositions to your stakeholders and execute them with excellence and defensibility. 𝟯- 𝗚𝗲𝘁 𝗕𝗲𝘁𝘁𝗲𝗿 𝗮𝘁 𝗦𝘁𝗼𝗿𝘆-𝗧𝗲𝗹𝗹𝗶𝗻𝗴: we tell stories everyday. Raising capital for a fund and building your venture firm requires that same story-telling skillset. Tell consistent narratives to the market to help build your own brand as well as your firm’s brand. 𝟰- 𝗟𝗲𝘃𝗲𝗿𝗮𝗴𝗲 𝘆𝗼𝘂𝗿 𝗦𝘂𝗯𝗷𝗲𝗰𝘁-𝗠𝗮𝘁𝘁𝗲𝗿 𝗘𝘅𝗽𝗲𝗿𝘁𝗶𝘀𝗲: if you possess subject-matter expertise in a certain vertical, domain or industry, you already own a defensible moat. Accentuate that to your advantage when building your firm, raising capital from LPs and winning deals from founders. 𝟱- 𝗠𝗮𝗶𝗻𝘁𝗮𝗶𝗻 𝗮 𝗖𝗼𝗹𝗹𝗮𝗯𝗼𝗿𝗮𝘁𝗶𝘃𝗲 𝗡𝗮𝘁𝘂𝗿𝗲: early stage venture is a highly collaborate asset class. Keep your ego at the door, you will never be able to do everything it takes to build a firm alone. Leverage LPs, partners and founders for deal flow sourcing, diligence and overall industry best practices. 𝟲- 𝗙𝗶𝗻𝗱 𝘆𝗼𝘂𝗿 𝗣𝗿𝗼𝗱𝘂𝗰𝘁-𝗠𝗮𝗿𝗸𝗲𝘁-𝗙𝗶𝘁: in quintessential startup vernacular, ideally find your product-market-fit by the time you launch your second fund. You need product-market-fit before you scale your AUM, grow your team and carry more fiduciary responsibility. 𝟳- 𝗞𝗲𝗲𝗽 𝘆𝗼𝘂𝗿 𝗣𝗮𝘁𝗶𝗲𝗻𝗰𝗲: early stage VC investing is a marathon. Patience will reward you personally, professionally and financially. Build mental, physical and psychological stamina to endure the inherent ebbs and flows of your entrepreneurial journey. 𝟴- 𝗧𝗵𝗶𝗻𝗸 𝗟𝗼𝗻𝗴-𝗧𝗲𝗿𝗺: early stage VC investing is the archetypal long-term financial asset. Think in 10 year increments for your firm’s grand vision, think in 4 years for a given fund’s investment period and think in quarters when reporting material updates on your portfolio to your LPs. 𝟵- 𝗛𝗮𝘃𝗲 𝗙𝘂𝗻: if instant gratification, short feedback loops and a constant need for reaffirmation is what you are looking for in a career then early stage VC investing will not fulfill you. As a past board member, I'm honored to celebrate the outstanding quarterfinalists. As we reflect on the state of digital health in 2024, it's evident that the COVID-19 pandemic has fundamentally transformed healthcare delivery. The rapid adoption of telemedicine during the crisis has become a permanent fixture, with over 60% of physicians continuing to utilize this technology. The sprint towards smarter healthcare, catalyzed by necessity, has shattered preconceptions about digital-first health experiences. Virtual care is no longer a stopgap but an integral part of a seamless, anytime, anywhere health ecosystem. 🌍 We're witnessing the rise of B2B2C models in remote consultations, with companies like Teladoc Health and Amwell partnering with insurance providers to integrate their services into health plans. Public sector collaborations, like eConsult Health 's partnership with the NHS in the #UK, underscore the potential for remote consultations to enhance efficiency and accessibility at a national level. The digital health landscape is evolving rapidly, with advancements in remote consultations, mental health support, at-home diagnostics, and patient monitoring. AI and generative AI transform care delivery, making it more efficient, personalized, and patient-centric. 🤖 Yet, challenges remain. While venture capital funding for digital health has stabilized after the pandemic boom, it remains below its 2021 peak. Sectors like digital therapeutics face reimbursement hurdles, and we've seen major players like Walmart and Optum close telehealth businesses. Looking ahead, we anticipate further consolidation in digital health, driven by acquisitions to create broader ecosystems encompass telemedicine, mental health, and other specialized services. As healthcare leaders, we must leverage these digital advancements to improve access, enhance patient outcomes, and drive efficiency in our healthcare systems. The future of healthcare is digital, and the time to embrace this transformation is now. SCOOP: Healthtech startup Regard has closed on a Series B funding round. The startup is rumored to have raised a Series B round at a $350 million valuation led by top VC firm Oak HC/FT, Business Insider has learned. Regard uses AI to automate clinical tasks like synthesizing patient data for documentation, and can even suggest diagnoses. Function’s platform offers 100+ tests for $499 per year, focusing on prevention and empowerment. The round was led by a16z Bio + Health and includes several celebrity investors. Congrats to Mark Hyman, MD, Pranitha Patil, Jonathan Swerdlin, Mike Nemke, Seth Weisfeld, and Dan Swerdlin. Sensi.AI raised a $31M Series B led by Insight Partners and Zeev Ventures to enhance its 24/7 monitoring technology for seniors and home care. Way to go Romi Gubes and the entire team! Ironspring Ventures raised $100M for their second fund, focusing on “manufacturing, construction, transportation, and energy.” Exciting times for Ty Findley, Adam Bridgman, Peter J. I recently spoke with an executive from one of the largest healthcare companies in the country.   When I asked them what are the highest priority problems they're concentrating on right now, unsurprisingly, 1️⃣ controlling costs around GLP1s came up first. But surprisingly, 2️⃣ the intersection of health and climate change came up second! It is clear that more healthcare leaders are beginning to think about (and respond to!) the deep interdependence of human and environmental health.   🗣 While my climate friends have long emphasized that "climate is a horizontal," and "there's no such thing as a climate company; all companies will have to adapt to climate change", I'm energized to hear more of these conversations beginning in healthcare.   😷 We already know that wildfires and air pollution are driving increases in respiratory and chronic illnesses, that heat waves and natural disasters are putting more lives at risk, and that rising temperatures and shifting rainfall patterns are enabling the spread of infectious diseases.   As we invest in downstream solutions that support patients suffering from respiratory diseases, cardiovascular diseases, anxiety, depression, and general gaps in gaps in health equity, we're also thinking about holistic, upstream factors, like the effects of climate change.   ⚕ 🤝 🌎 How should we evolve our traditional notions of preventive health to address these emerging challenges? ⚕ 🤝 🌎 How can health-first organizations play their part when it comes to the climate crisis? ⚕ 🤝 🌎 What are the innovative ideas you've seen around climate, sustainability, and human health?  💡 This is both a personal area of interest and something I'm hoping we get to explore more at Inflect! Interesting report from PitchBook on first quarter Digital Health VC activity. - $1.1 billion in funding. Key late-stage deals were led by Transcarent ($126M) and Rightway ($108.8M). - The top investment categories were general & primary care ($213.4M) and health benefits navigation ($140.7M). Challenges & Trends: Valuations: Less than 20% of startups reported a priced round in the last 18 months, with an increase in extension/bridge rounds. Digital Therapeutics: Facing significant challenges due to elusive reimbursement and cautious investors. Better Therapeutics is winding down operations, and Akili is being taken private for $34M, a sharp decline from its peak valuation of nearly $450M. Telehealth: Setbacks include the shutdown of Optum's virtual care and Walmart's telehealth services. The pandemic saw over $15B in VC funding since 2020, but some platforms struggled with unsustainable business models and strategic missteps (e.g., Teladoc's costly Livongo acquisition and Amwell's delisting from Nasdaq). Looking Ahead: While digital therapeutics and telehealth face hurdles, there's potential in: Platforms integrating various treatments for conditions like diabetes, metabolic health, and mental health. Specialty telemedicine, B2B platforms, and hybrid-care models, which offer a promising investment outlook. Despite current challenges, strategic investments and a focus on sustainable models can drive the next wave of innovation in digital health. Hot off the pre-print server – an important publication authored by the Atropos Health team exploring the performance of LLMs in answering real-world clinical questions. LLMs excel at summarizing and contextualizing existing literature, but cannot generate de novo medical evidence today. The results of the study highlight performance gains attributable to injecting novel, relevant, and up-to-date medical evidence into LLM-powered clinical decision support systems. Presenting at the Senate as part of the Coalition for Health AI (CHAI) "Day on the Hill" wasn't on my 2024 bingo card, but I'm feeling energized by the bipartisan discussions on healthcare AI. The numbers tell a compelling story: while nearly 2,000 healthcare AI startups have received funding over the last two decades, only 1 in 20 provider-facing companies reach late-stage maturity. Beyond natural market dynamics, two critical barriers are holding back AI's full potential in healthcare: unsustainable payment models and inadequate evaluation infrastructure – only 5% of research studies on LLMs in medicine utilize real patient care data, and studies advancing our understanding of performance beyond medical licensing exam benchmarks remain anemic at best. At CHAI, we're advocating for essential foundations: • Rigorous technical model performance evaluations • Comprehensive bias assessments across diverse patient populations • Thorough pre-deployment simulations and prospective studies • Robust ongoing local monitoring The path forward demands strong reporting infrastructure, advanced healthcare AI benchmarks, and meaningful public-private partnerships to drive real-world AI deployment in healthcare. HealthTech founders have a lot of blind spots. (And that is to be expected and normal) The majority of founders we work with at Adopt-A-Doc are non-clinical. They usually come from a strong technical / commercial background. Think engineers, marketers, developers, PHDs and CEO types. They bring with them a wealth of experience building products and scaling businesses. 𝐁𝐮𝐭 𝐭𝐡𝐢𝐧𝐠𝐬 𝐚𝐫𝐞 𝐝𝐢𝐟𝐟𝐞𝐫𝐞𝐧𝐭 𝐢𝐧 𝐡𝐞𝐚𝐥𝐭𝐡𝐜𝐚𝐫𝐞. More often than not, founders are less well versed with: 1. 𝐂𝐥𝐢𝐧𝐢𝐜𝐚𝐥 𝐄𝐯𝐢𝐝𝐞𝐧𝐜𝐞 𝐆𝐞𝐧𝐞𝐫𝐚𝐭𝐢𝐨𝐧 - How to design a high quality study, how to carry them out and write them up. 2. 𝐑𝐞𝐠𝐮𝐥𝐚𝐭𝐢𝐨𝐧 𝐚𝐧𝐝 𝐜𝐨𝐦𝐩𝐥𝐢𝐚𝐧𝐜𝐞 - Medical device regulation, data protection, cybersecurity and compliance. 3.
