From one hand, so called guidelines state TA is class III. On the other hand, this paper clearly shows the difference between a successful versus failed TA during PCI. Each patient is different. Do not make please dogmatic statements especially with the guidelines. I have been practicing for 30 years whenever there is a large thrombus and espcially if it is fresh, we always aspirate, give IC adenosine prior to aspiration using the distal port of the export etc. catheter and use 2b 3 a inhibitors beside heparin during the PCI and for 2b 3a agents overnight post PCI IV infusion. Kerp ACT between 200-250 sec dıiuring PCI when using unfractionated heparin and integrilin together. Amazing results the next day if not immediately post PCI. Time to update the so called guidelines. Can someone talk to academic mafias to get this done please?

Kyle Williams, Stephen Rudin MD, Daniel Bednarek, Ammad Baig, Adnan Siddiqui, MD, PhD, Ciprian Ionita, and our team conducted a recent study that introduces a novel approach to neurovascular diagnostics that integrates artificial intelligence with physics modeling using Physics-informed Neural Networks (PINNs). This method leverages patient-specific vascular models, providing a significant improvement over traditional computational fluid dynamics (CFD). 🧠 Study Insights: ◾ Efficiency and Accuracy: Our PINNs method calculates high-resolution velocity and pressure fields in blood vessels without the manual data processing required by conventional CFD, thereby enhancing diagnostic efficiency and accuracy. ◾ Application: Successfully applied to cases such as aneurysms and carotid bifurcations, this technique supports more precise and personalized treatment planning for patients with neurovascular pathologies. By combining AI with detailed physical models, our approach streamlines the diagnostic process and enhances the accuracy of neurovascular assessments. This innovation paves the way for more advanced and patient-specific therapeutic strategies.
