Exciting times at 4C Digital Health! Wilmington, Delaware - 4C Digital Health (4C), the preeminent organization delivering independent financial oversight and control solutions for self-funded employee health plans, recently announced the strategic expansion of its services into the Commonwealth of Kentucky. In late 2023, 4C successfully competed for an initial award issued by Kentucky’s Department of Employee Insurance, and Kentucky has since extended the initial contract to allow 4C to continue its important work on behalf of the Commonwealth, its employees, and its taxpayers. The Kentucky Employee Health Plan (KEHP) serves more than 288,000 members, which amounts to 1 in 16 Kentuckians. In addition, KEHP spends over $1.8 billion dollars in total healthcare benefits on behalf of its members per year and covers over 8.2 million claims per year. KEHP objectives include “improving employee health and well-being” and “providing state-of-the-art benefits while maintaining reasonable premiums.” Recognizing its important fiduciary responsibilities to its employees and its state health plan, the Commonwealth of Kentucky enacted Senate Bill 42 (2023) introduced by Senator Stephen Meredith for the purpose of ensuring that critical health plan dollars are being spent properly on behalf of state employees. The legislation requires that 4C analyze 100% of medical claims submitted for payment to KEHP; identify erroneous overpayments; and identify inappropriate or erroneous fees imposed by KEHP’s third-party administrator. Yes, CVS/caremark is aiming to save the patient money. Yes, big corporate PBM is projecting an image to care about the patient. However, reimbursing pharmacies below the cost of the medication is robbing the pharmacy. If the pharmacy is unable to fill the script and the patient is forced to go elsewhere, highly likely to a place that has minimal knowledge the patient, this is not caring about the patient. This different pharmacy is highly likely a place where the patient doesn’t know the staff, and where the patient is often uncomfortable sharing important details. This is not caring about the patient. This could potentially endanger the patient. Also, and this is key, the claims data on claims subjected to programs like the Cost Saver and MedsYourWay programs, automatically re-routes the claim, often without the patient’s knowledge and/or full comprehension of what is happening behind the scenes. Then, there is the scenario where the patient no longer has reasonable access to a pharmacy because the excessive below cost reimbursements forces the pharmacy to close its doors. This is not caring about the patient.

I was looking at a RFP the other day for a Digital Health solution. There was not a single question about the company track record, clinical evidence or processes & playbook. The whole RFP looked like this: → Do you have feature X? → Do you have feature X? → Do you have feature X? → Do you have feature X? → Do you have feature X? So yes the RFP was almost certainly baked for another vendor. And maybe they do care about the other elements, but just really didn’t care to ask since it’s baked. But then I spoke to another health system recently that was evaluating several vendors and I asked them: 𝘞𝘩𝘢𝘵 𝘳𝘦𝘢𝘭𝘭𝘺 𝘮𝘢𝘵𝘵𝘦𝘳𝘴 𝘵𝘰 𝘺𝘰𝘶? Their answer: 𝘑𝘶𝘴𝘵 𝘵𝘩𝘦 𝘧𝘦𝘢𝘵𝘶𝘳𝘦𝘴 & 𝘧𝘶𝘯𝘤𝘵𝘪𝘰𝘯𝘢𝘭𝘪𝘵𝘺. I asked the same question: 𝘞𝘩𝘢𝘵 𝘳𝘦𝘢𝘭𝘭𝘺 𝘮𝘢𝘵𝘵𝘦𝘳𝘴 𝘵𝘰 𝘺𝘰𝘶? Their answer: 𝘛𝘩𝘪𝘴 𝘵𝘪𝘮𝘦 𝘸𝘦 𝘸𝘢𝘯𝘵 𝘢 𝘳𝘦𝘢𝘭 𝘷𝘦𝘯𝘥𝘰𝘳 𝘱𝘢𝘳𝘵𝘯𝘦𝘳 𝘸𝘩𝘰 𝘢𝘤𝘵𝘶𝘢𝘭𝘭𝘺 𝘥𝘦𝘭𝘪𝘷𝘦𝘳𝘴 𝘰𝘯 𝘸𝘩𝘢𝘵 𝘵𝘩𝘦𝘺 𝘴𝘢𝘺 𝘵𝘩𝘦𝘪𝘳 𝘱𝘳𝘰𝘥𝘶𝘤𝘵 𝘤𝘢𝘯 𝘥𝘰 𝘢𝘯𝘥 𝘸𝘩𝘢𝘵 𝘵𝘩𝘦𝘺 𝘸𝘪𝘭𝘭 𝘥𝘰. Sometimes experience is the biggest teacher… although in healthcare, the challenge is we really don’t have time to waste. Probably every health system leader reading this will tell me their organization is in the latter example of this story, not the former. But I’m telling you, if you actually unpeel the layers… the former story of focusing on just the Tech and not the People and Process… is happening way more than you think in your organizations. And when you’re wondering why that piece of Tech you invested in doesn’t work… it might very well be that you focused on the wrong things in the initial vendor selection process. Inaccurate fee schedules are costing providers millions of dollars every year.
