The Future of the Dental Industry: Chapter 2: The most significant barriers to the future of the dental industry, in terms of providing the best possible care and working conditions for both patients and industry employees, stem from the disinterest and lack of incentive for insurance companies to recognize the growing disparity between inflation and the cost of dentistry. Their unwillingness to meaningfully address these costs for patients and practices is detrimental to the future of dental healthcare. To illustrate this point, I will provide data from reputable sources. The average US inflation rate over the past 50 years has averaged 3.9%, amounting to a 198% increase. During the same period, the average annual maximum paid by insurance for dental care has remained steady at $1,000-$1,500 per patient per year. If we apply the year-by-year inflation rates to $1,250 (the midpoint of the annual maximum average) and increase it each year by the rate of inflation through 2023, the average annual per-patient maximum should be $8,238—a disparity of $6,988. This is where patients are losing out to big insurance companies. Regarding dental providers and practices, according to the U.S. Bureau of Labor Statistics, prices for dental services are 1,023% higher in 2024 compared to 1975 (a $1,122.56 difference in value) when considering the value of $100 in 1975 and what you would need to spend today, with inflation specific to dentistry considered. CPI in 2024/CPI in 1975×1975 USD value=2024 USD value The CPI for dental services was 53.146 in 1975 and 596.597 in 2024: 53.146/596.597​×$100=$1,122.56 According to the U.S. Bureau of Labor Statistics, $100 in 1975 has the same “purchasing power” as $1,122.56 in 2024 (in the CPI category of dental services). It is difficult to find standard reimbursement rates from 1975, but the above clearly indicates a failure by insurance companies to provide fair and reasonable allowable rates by code compared to the cost of providing dentistry. QUESTION: What can we do to hold insurance companies accountable to their patients’ and dental practices’ needs first, as opposed to the shareholders’ wants and expectations?

Investing in advanced dental equipment can be a significant hurdle, especially when machines like CBCT scanners cost up to $50,000. In my practice, I’ve found an efficient solution—outsourcing. By using a mobile company for CBCT scans, I pay just $100 per patient and still provide top-tier care without the upfront expense. This strategy has allowed me to focus on practice growth without financial strain. As dental professionals, it’s crucial to think outside the box to balance patient care and financial management.

The DSO market is expected to grow by nearly 100% between 2018 and 2025. This is unsurprising, as the DSO model streamlines management and operations for dental practices, allowing providers to focus more on delivering quality dental care to patients. One aspect that DSOs should keep in mind while expanding is maintaining consistency across all practices. This consistency is essential for providing the highest quality care to patients and achieving overall organizational growth.

It takes a heck of a lot of work to standardize a dental formulary. Every location has the potential to derail a DSO's margins, too. Each clinic is ordering its own supplies. Invoices from separate vendors are everywhere. DSO managers are scratching their heads because of the inconsistency. Fixation Mechanism: • RV leads often use an active fixation mechanism like a screw-in helix or passive fixation with tines to secure the lead to the myocardium. • CS leads rely more on passive fixation. They have a softer, flexible tip designed to lodge within a coronary vein no screw into the myocardial tissue. • They may also have multiple electrodes along the lead body 4. Insulation and Conductors: • RV Lead: • The insulation and conductors are optimized for placement within the high-pressure, muscular environment of the right ventricle. • CS Lead: • CS leads often have more durable and flexible insulation and conductors to withstand the constant motion and varied pressures within the coronary venous system. 5. Electrode Configuration: • RV Lead: • Typically, RV leads have a single electrode at the tip (unipolar) or a ring and tip electrode (bipolar) for pacing and sensing. • CS leads often have multiple electrodes along their length to allow for different pacing vectors and to optimize LV capture. The dimensions of right ventricular (RV) and coronary sinus (CS) leads can vary depending on manufacturer • Diameter: around 5-7 French (1.7-2.3 mm). • Length: between 50 to 60 cm, depending on the size of the patient and the specific placement within the heart. • Fixation Mechanism: Can be either an active fixation (screw-in helix) or passive fixation (tines). Coronary Sinus (CS) Lead: • Diameter: around 4-6 French (1.3-2.0 mm). • Length: Often longer than RV leads, typically 65 to 85 cm • Electrodes: Often has multiple electrodes (e.g., 2-4) along the lead to allow for different pacing configurations. • Flexibility: Designed to be highly flexible with a pre-shaped curve to navigate the tortuous path of the coronary sinus and veins.
