Regarding HHA and PCA Training and In-Services in New York State: In addition to the initial training, HHAs and PCAs are required to participate in in-service education programs to enhance their knowledge and skills continuously. These in-service programs ensure that aides stay updated with the latest practices and regulations in home care. 6 clock hours for PCAs, and 12 clock hours for HHAs annually. What are “clock hours?” In-class, in-person training. No on-line training is counted in the State of New York by the NY State Department of Health or the New York State Education Department. Clock-hours requires the RN trainer to have the aides sign in (not electronically), with date, time, subject of in-service and RN signature. Both state agencies are clamping down on in-service matters, including the annual mandates, such as HIPAA, fire/safety/infection control, sexual harassment, etc. If OSHA, DOL or the state requires these to be performed annually, these can be counted as clock-hours. The RN has to sign off on all of this, so don’t get caught short when applying these mandatory standards. HHA/PCA training is also by clock-hours as well. Only during COVID did the state allow for virtual/online training, followed by skills competency in the classroom. New York State also does not allow on-line training either for either course. The agency and the nurse can lose their licenses. Recent on-site surveys have found many deficiencies in these areas, calling for the employee to be re-trained in the classroom. Repeat violators can lose their training approvals. Always finish the in-service with a competency quiz to assure their retained knowledge meets the content of the topic you are providing. This must be graded by the RN, counter-signed and saved in their personnel file. There is no online initial training allowed for PCAs and HHAs either.

Physician assistants still can’t practice without consulting. But some won’t need doctor sign-off. Come Jan. 1, NH's nearly 1,340 physician assistants, the provider many patients see, will no longer have to have a signed “collaboration agreement” with a physician for their entire career. The sponsors of House Bill 1222, which Gov. Chris Sununu signed Friday, said complaints from physician assistants prompted the legislation. I will stick with a physician as my primary care provider, with more in-depth training and knowledge of clinical medicine. What the heck is a trauma registrar, anyway? Once upon a time even my guest asked that question. After starting a career in nursing in 1987, Stefanie Arp became a trauma nurse specialist in 2001. And quickly became tasked with the responsibilities of trauma coordinator, a position she knew very little about but found herself immersed in, whether she wanted to or not. Trauma registry opened up a new world of data abstraction, quality improvement, trauma center designation, high-pressure American College of Surgeons site surveys, and much more, including medical coding and the use of trauma activation codes. And opened Arp’s eyes (and now, mine) to an under the radar profession that plays a huge impact on patient outcomes and organizational quality. Invest in your team! Two weeks ago, I was rounding on my staff and heard about some interest in the education benefits available at Children's Hospital of Philadelphia. We had team members who were at different points in their education journey, but did not know how to take advantage of the excellent benefits at their disposal. I set aside time to lead a lunch session to go over the types of benefits offered and how to apply to them. More importantly, the team walked out of our session with the following information: -How we support and celebrate anyone looking to grow, even if it means leaving our team (preferably while staying internal). -The importance of a psychologically safe dialogue with your leader on your interests, plans, and career goals. Teams need to hear that it is ok to want to grow, and that they can have honest talks with their supervisors about growing to new areas of their organization. I’m proud to have a leadership team that has developed entirely from our frontline staff, as well as celebrated colleagues graduating into new areas of CHOP.
