I like reading mystery novels, so I really enjoyed talking to Stephen J Wallace about his book, Hazardous Lies. I don't often find mystery novels in which the main character is an engineer, but this is one of those exceptions. Contrary to popular belief, i would say this again that Safety is NOT the responsibility of Safety professional. Their job is to install & improve safety system. This is the only way to keep an organization safe & profitable. I have explained this model in detail in my upcoming book, will publish in U.S by mid-August. What do you think about this model? Quite some time ago, I walked up to two fellows operating an AWP. Got into a conversation and began questioning him about his safety procedures..   The first question I asked was "Do you folks have a fall protection plan available for work in an AWP?"  The response I got was "We go to GP for that".   Re-phrasing the question, I asked "Does your company provide you with a fall protection plan before you work in an AWP", and again the answer was "Yes, they send us to GP to get it".  I found his responses odd, especially when I knew that their Fall Protection Plan was available on the wall of their lunch room / safety office    Talking to their safety supervisor, we found that the fellow hadn't fully understood me.  Language barriers were a problem.  Work was about to stop for a monthly safety meeting, so immediately following the safety rep and I met with the employee, discussed his nationality, and then asked him the same questions again using google translator. This time, he was able to answer perfectly.  In our initial conversation he had thought I was asking where they go for training.  I spoke with the fellow a little longer, and the one thing I asked him was if he minded, us using google translator.  In short, I wanted to be sure it wasn't embarrassing for him.  His answer was no... he uses it all the time.   We talked a bit further and I found out that he will often record safety meetings, toolbox meetings, and other directions on google translator to listen to them again in his own language.  From a safety guy perspective, I can see how language barriers can be a hazard; but I think we have to accept that this type of thing is happening. I won't get into what's right or wrong, but going forward, If I should meet someone who I suspect is having a language issue, I am certainly going to utilize google translate as a tool.

Book Review: HOP Beginners Guide to Doing Safety Differently, Volume 3 Controls Save Lives – The Sixth HOP Principle? By Brent Sutton and Jeffery Lyth, Foreword by Todd Conklin. The latest edition of this planned 10-volume series has just been released and shot to #1 on the Amazon sales list! As the title suggests, these are excellent “beginners guides” to HOP, priced at just over $AU10 for 70 pages. Written in everyday language, each volume reintroduces HOP for true beginners, making it easy to pick up any single volume based on your interest. This volume introduces “the 6th HOP Principle,” a concept that resonated with me as it bridged my internal conflict between traditional methods and the HOP approach. The authors discuss two ways to look at risk: through an engineering lens, assessing systems and controls, and through human psychology, understanding behaviors and biases. They argue that HOP helps bridge the gap between these perspectives in a complex and uncertain world. The book introduces the Energy Wheel, a tool I've successfully used at BP for over a decade (aka Sources of Energy). The authors state that in the world of HOP, systems reduce the impact of uncertainty by allowing workers to fail safely or systems to fail gracefully. They emphasize improving the work environment to prevent mistakes from leading to disasters and shifting focus from controlling every error to ensuring errors don’t have catastrophic outcomes. Brent Sutton introduces Critical Controls, focusing on STKY (Stuff That Kills You). A short story by Dr. Todd Conklin concludes with the quote: “We should change golden rules to golden controls.” The book also discusses “Controls Save Lives” as a principle, value, mindset, and practice, followed by a reflective story from Josh Bryant on his HOP journey and the 6th principle. As with previous volumes, the book concludes with an activity for readers to implement in their workplace, similar to BP’s “Safety Leadership Principles in Action” program. It will be interesting to see how our retail fuels industry contractors receive this and where they take it in their organizations after we provide copies at next month’s WPCG Contractor Safety Forum, for which I've engaged both Brent and Josh to speak. #excited

We are not supposed to have favorites like children 🤣 but mechanical integrity hits different. Organizations have worked diligently to address the PSM Rule requirements since its inception, specifically focused on those programs related to safety and management of risk associated with handling hazardous materials covered by the standard. However, many continue to struggle with the Mechanical Integrity, element, and how it relates to an effective Asset Management system that addresses not only safety requirements, but also provides effective asset policy, strategy, objectives and plans to ensure best-in-class management of assets that mitigates risk over the entire life cycle of the assets.

Conklin opens with an honest preface, underscoring that "fixing the work, not the worker" should be the main focus. The core of the book, outlined across five clear chapters, presents principles that redefine how we perceive human error and organizational improvement: Error is Normal: Mistakes are part of human nature; even the best employees err Blame Fixes Nothing: Blaming individuals for mistakes detracts from systemic solutions Learning is Vital: Continuous improvement and adaptation are non-negotiable Context Drives Behavior: Organizational culture and processes heavily influence actions Response to Failure Matters: Constructive responses can transform failures into growth opportunities Todd challenges traditional safety models, emphasizing a shift from punitive measures to a system-focused approach. I’ve seen messages like this from other professions, but it truly resonates within the safety field. Many of my colleagues often resist, push back, shut down, create obstacles, refuse to collaborate, or even show outright disdain for frontline employees, supervisors, and mid-level management. In the safety profession, we have a choice: compliance or coaching. We can either police or persuade. Building relationships with operations and those on the ground doing the work is crucial to helping them succeed in their jobs. We can’t just shut them down and retreat to our offices. I don’t understand why we can’t provide more meaningful support to the workforce, rather than offering unreasonable and often unrealistic solutions. While we may not know everything, it’s essential that we act as servant leaders who facilitate and support rather than obstruct.
