VC funding in healthcare has been more resilient than in other industries. Here are some key takeaways from recent Carta data: 👉 At the priced seed, Series A, and Series B stages, dilution for health startups has noticeably decreased, while dilution for SaaS startups has stayed flat or increased slightly. 👉 In priced seed rounds, median dilution for healthcare companies dipped to 14.2%, compared to 20% for SaaS.  👉 The frequency of bridge rounds surged in the health sector in Q1, jumping to 44.3% from 36.9% in Q4 2023. This is the first significant increase in bridge rounds in healthcare over SaaS since early 2022. 👉 More founder-friendly terms for health startups may result from enduring VC interest since COVID. Healthcare has remained strong through economic turmoil, enabling higher valuations with smaller round sizes. What are the implications? The healthcare sector might see a rise in new ventures due to favorable investment conditions and lower dilution rates. The surge in bridge rounds suggests that certain sectors within healthcare face unique challenges that require interim financing solutions. 7 RECOMMENDATIONS FOR HARDWARE STARTUPS It’s hard to understate the difficulty of the v1 launch of a consumer hardware company. I see a ton of new HW startups coming out. Here are some counterintuitive recommendations… 1) Target a small market for v1, not a large market. You need a small number of people to love your product and this will earn you the right to build a v2. You have to understand your v1 will not make you a successful company. Your v1 needs to prove that you deserve to exist in the first place. In the case of WHOOP, we started with pro athletes who were willing to put up with our v1’s short battery life and flaky Bluetooth connections because they so desperately needed Sleep and Recovery data. A broad consumer base would have rejected it. 2) Be great at no more than 1-3 things. If you try to be great at more than that, you will build a hardware that is mediocre at a wide range of things. You won’t think it’s going to be mediocre, but it will be. 3) You want a small launch. Not because you want small sales but because you want to have a steady ramp. It’s much better to do 100k of sales steadily over 12 months than in 2 months with a huge drop off. Keep in mind you need to manage your manufacturing partner and that there’s nothing better or more efficient than steady and predictable. 4) CAC is your enemy. Referrals are gold. The magic of referrals is that they signal high NPS and they help you grow steadily. Again everything that helps you have a steady HW manufacturing plan is good. Building a referral program for v1 launch is a good use of your time. 5) Be excellent at Software. Turns out being good at both Hardware and Software is a hard thing and something that your BIG COMPANY competitor is actually not great at (excluding Apple here). For some reason, there’s such a magnifying glass on the Hardware development, that Software is then overlooked by consumer startups. Don’t make that mistake. You can be great at both. 6) You should spend an unreasonable amount of time and money on ONE aspect of your Hardware. Could be the design or a technical breakthrough. But something needs to be SPECIAL. In our case, we designed a modular battery pack that allowed Whoop to be charged without taking it off your body. This made our development timeline much longer, but it allowed us to achieve something that no other wearable has achieved since: 24/7 health data. 7) Finally, have an unrealistic belief in yourself and your team. You are not supposed to succeed and the odds are stacked against you. Building hardware takes confidence and you’re up against big players who will rip you off. We went so far as to leave a message for our competitors on our circuit board (pictured below). In the case of Amazon and others, the prophecy came true. Startmate has invested in 230+ startups, much of it under Michael's leadership, and now has $3.5b in value across its portfolio companies. The GLP-1 space has continued to explode, with the potential for impact expanding to cardiovascular risk, sleep apnea, and liver disease. So who are the emerging players poised to challenge Lilly and Novo? The incoming players to watch are a mix of startups and incumbents. In the startup realm, Viking Therapeutics, Inc., and Altimmune, Inc. have gained significant attention as their GLP-1 candidates demonstrate solid weight-loss outcomes. For San Diego based Viking, weekly doses of VK2735 delivered weight loss of up to 13% compared to placebo in a Phase 2 trial. Perhaps most exciting, the weight loss did not plateau at any time during the 13 week study, suggesting longer duration doses would result in further weight loss. Meanwhile, Altimmune's Pemvidutide (being studied for both obesity and MASH) is on track for an end of phase 2 FDA meeting in Q3 of this year. Notably, Pemvidutide has showed promise in limiting the amount of weight loss derived from muscle, which is a notable problem with the existing GLP-1s. A recent phase 2 trial showed only ~25% of weight loss was from lean mass, more in line with the ratio seen in typical diet & exercise programs. There are also several incumbents working hard to establish their presence. Roche's $2.7B acquisition of California-based Carmot Therapeutics, Inc. Therapeutics in December and Amgen's MariTide are both notable. Roche recently announced data from a Phase 1B study for CT-388 showing placebo-adjusted weight loss of ~19%, with 85% of participants seeing weight loss of more than 10%. Not to be left out, Amgen has recently advanced MariTide into Phase 2 studies, with Phase 1 results published in February showing weight loss of up to 15% in 12 weeks, with some dosage levels only required once a month. To have a credible chance at challenging the existing market leaders, new GLP-1s will have to demonstrate meaningful improvements on the following: 1. Dosage - most approved therapies today (with the exception of Rybelsus) are subcutaneous injections, given weekly. Having an easier form (tablet) or frequency (bi-weekly or even monthly) would be a big advantage. 2. Preservation of lean muscle mass - helping patients derive weight gain from regular adipose tissue instead of muscle mass. 3. Benefits beyond weight loss. Novo's recent FDA label expansion for Wegovy for reducing the risk of serious cardiovascular events provides a blueprint of how to expand the area these medicines can be helpful in. While there are many companies active in the space, it's worth noting just how large a lead Lilly and Novo have in this future $100B+ market. A recent GlobalData report noted that even by 2029, these two giants will have ~80% of the market between them. That said, 20% of a $100B market is still a sizable prize for other innovators, and continued innovation will only help patients.

Great article from PitchBook on why domain expertise amongst VCs matters. TL;DR -- in verticals like healthcare, having investors with high amount of domain expertise can result in up to 40% higher odds of achieving an IPO, 20% higher chance of being acquired and 30% higher chance of an overall successful exit. Should you sell your practice to a VC-Backed MSO Startup? tldr: - new VC-backed startups are forming MSOs to roll up independent practices - The pitch is physicians receive early liquidity from the roll up and are granted ESOP shares that can be used for retirement - Problem is the use of ESOPs with independent practices is not a new concept and has been used with large group practices since the 70's - ESOPs do not grant full ownership and in order for the shares to be valuable for retirement, the practice needs to run the same if not better post roll up - With VC-backed startups being primarily operated by non-physicians with zero clinical or medical practice experience, the likelihood of this happening is extremely low making the entire concept high risk However, there is a viable path for continued ownership + liquidity for independent practices. One of my proudest moments as an investor occurred today as Alaffia Health announced its series A because it shows how the Aperture® Venture Capital vision of multi-level, multi-generational #impactinvesting is succeeding in the marketplace. A lot of healthcare startups fall into the trap of "our performance is solid, especially for healthcare." This is a fallacy. The best companies strive to become the best companies; not the best companies in their vertical. Doing so unlocks more expansive thinking, challenges complacency, and forces teams to tackle more real problems, such as those that structurally hinder progress in an industry (e.g., broken margin structures, perverse incentives, or information asymmetries). Don't aim for excellence in healthcare. Aim for excellence – period.

Some VC investors believe that brick and mortar - referring to a physical presence of some kind - should be avoided at all costs. Yet, the track record of clinic and hybrid care businesses in health care indicates otherwise. ==> One Medical (sold for $3.9 billion to Amazon), Oak Street (bought by CVS for $10.6 billion), Lifestance (IPO at $7.5 billion market cap), and PE-backed Premise Health (acquired by OMERS Private Equity for $1.1 billion) If a clinic is indicated as the optimal way to deliver care for that patient population, there are creative ways to build and operate them.
