There appears to be a "Superstar Effect" in healthcare and biomedical AI research right now. A small percentage of publications are truly novel methods contributions that introduce new capabilities or behaviors; whereas a majority of contemporary research publications are focused on implementation science to assess the real-world utility and performance of new methods. This dynamic seems different from prior eras, when the translational infrastructure for implementing novel AI methods was largely unavailable. While methods research often captures more attention and accolades, both types of research are critical – novel methods without implementation science lacks translatability; implementation science without novel methods lacks novelty. If our goal is to invent and deploy transformative AI methods in real-world healthcare and life sciences contexts, we ought to continue encouraging and rewarding stellar implementation science work alongside breakthrough methods development.

Should you sell your practice to a VC-Backed MSO Startup? tldr: - new VC-backed startups are forming MSOs to roll up independent practices - The pitch is physicians receive early liquidity from the roll up and are granted ESOP shares that can be used for retirement - Problem is the use of ESOPs with independent practices is not a new concept and has been used with large group practices since the 70's - ESOPs do not grant full ownership and in order for the shares to be valuable for retirement, the practice needs to run the same if not better post roll up - With VC-backed startups being primarily operated by non-physicians with zero clinical or medical practice experience, the likelihood of this happening is extremely low making the entire concept high risk However, there is a viable path for continued ownership + liquidity for independent practices. Leading US VC General Catalyst just acquired the health system Summa Health for $485 million. Summa Health is a nonprofit #healthcare system with over 30 locations in Ohio (US). This analysis has been made possible thanks to the data platform HealthTech Alpha from Galen Growth | Insights You Can Trust. I wrote down a few thoughts: 1️⃣ 𝐆𝐞𝐧𝐞𝐫𝐚𝐥 𝐂𝐚𝐭𝐚𝐥𝐲𝐬𝐭 𝐢𝐬 𝐩𝐥𝐚𝐧𝐧𝐢𝐧𝐠 𝐭𝐨 𝐛𝐮𝐢𝐥𝐝 𝐚 𝐝𝐢𝐠𝐢𝐭𝐚𝐥-𝐟𝐢𝐫𝐬𝐭 𝐜𝐚𝐫𝐞 𝐝𝐞𝐥𝐢𝐯𝐞𝐫𝐲 𝐦𝐨𝐝𝐞𝐥 𝐥𝐨𝐜𝐚𝐥𝐥𝐲 The health system Summa Health will be used as a testing field for the wide HealthTech portfolio of General Catalyst. General Catalyst, through its subsidiary HATCo, has committed an additional $550 million over the next five years for strategic investments and innovation. I had a similar case in my network of a PE fund that owned both a health system and a portfolio of HealthTech companies. On paper, this was the perfect match. In reality, it failed. 2️⃣ 𝐓𝐡𝐞 "𝐎𝐩𝐞𝐫𝐚𝐭𝐢𝐧𝐠 𝐬𝐲𝐬𝐭𝐞𝐦" 𝐰𝐢𝐥𝐥 𝐛𝐞 𝐚𝐭 𝐭𝐡𝐞 𝐜𝐨𝐫𝐞 𝐨𝐟 𝐭𝐡𝐞 𝐭𝐫𝐚𝐧𝐬𝐟𝐨𝐫𝐦𝐚𝐭𝐢𝐨𝐧 General Catalyst includes Commure in its portfolio. Over the past several years, Commure has integrated data from various systems, including all major EHRs, to become the interoperable operating system used by many leading hospitals across the U.S. One immediate application will be implementing Scribe (based on Commure), an AI-powered tool for automatic clinical documentation co-developed with HCA Healthcare. 3️⃣ 𝐆𝐞𝐧𝐞𝐫𝐚𝐥 𝐂𝐚𝐭𝐚𝐥𝐲𝐬𝐭'𝐬 𝐩𝐨𝐫𝐭𝐟𝐨𝐥𝐢𝐨 𝐡𝐢𝐠𝐡𝐥𝐢𝐠𝐡𝐭: 𝟏 𝐛𝐢𝐠 𝐬𝐮𝐜𝐜𝐞𝐬𝐬 𝐚𝐧𝐝 𝟏 𝐛𝐢𝐠 𝐟𝐚𝐢𝐥𝐮𝐫𝐞 GC invested in Livongo, a telehealth solution for diabetes management, that was acquired by Teladoc for $18 billion (though Teladoc is now valued at only $1.7 billion!). Livongo was the perfect exit before the HealthTech downturn. On the other hand, GC has been a major investor in Olive AI, a BPO and care automation company for hospitals. Despite being valued at over $4 billion in 2021, Olive AI went bankrupt in 2023. Commure is now GC's new bet in care operations, informed by lessons from Olive AI's failure. 4️⃣ 𝐀 𝐕𝐂 𝐨𝐰𝐧𝐢𝐧𝐠 𝐚 𝐡𝐞𝐚𝐥𝐭𝐡 𝐬𝐲𝐬𝐭𝐞𝐦: 𝐰𝐢𝐧𝐧𝐢𝐧𝐠 𝐫𝐞𝐜𝐢𝐩𝐞 𝐨𝐫 𝐝𝐲𝐬𝐭𝐨𝐩𝐢𝐚𝐧 𝐬𝐜𝐞𝐧𝐚𝐫𝐢𝐨? The debate over private investors owning parts of the healthcare system (and potentially prioritizing profits over patient care) is intense (both in the US and Europe). Personally, I am quite excited about this move. And VCs are not PEs: While private equity firms aim to optimize the profitability of an established healthcare asset within five years for a high-multiple resale, venture capitalists focus on increasing the value of their HealthTech portfolio by demonstrating how these solutions bring real-world value. I am not naive though: I know that VCs do not act in the general interest. But we need them to drive innovation in healthcare. #healthcare #healthtech #vc #hospital #transformation

Some VC investors believe that brick and mortar - referring to a physical presence of some kind - should be avoided at all costs. Yet, the track record of clinic and hybrid care businesses in health care indicates otherwise. ==> One Medical (sold for $3.9 billion to Amazon), Oak Street (bought by CVS for $10.6 billion), Lifestance (IPO at $7.5 billion market cap), and PE-backed Premise Health (acquired by OMERS Private Equity for $1.1 billion) If a clinic is indicated as the optimal way to deliver care for that patient population, there are creative ways to build and operate them. This partnership will help you know how food affects your health 🥗 ŌURA (wearable ring that tracks health data) and Dexcom (continuous glucose monitoring device maker) are working together and integrating their data and biosensors so users can adjust behaviors to positively impact their #biometrics and long term health. Dexcom has also invested $75 million into Oura Series D funding round - increasing their valuation to $5 billion. Oura had stated they sold 2.5 million rings earlier this year and is predicting their annual sales to double end of this year. This partnership can help people understand the link between activity 💪, sleep 💤 , nutrition 🍲, and glucose 🍭. "Move fast and break things? Not in healthcare." A hospital CEO raised his hand and said from the crowd. (I was addressing a room full of providers on AI's potential in healthcare.) As a YC alum and multi-time founder, I've seen tech revolutionize industries. But healthcare? The stakes are higher. Lives are on the line. The CEO's concern echoed across the room: "We can't afford Silicon Valley's 'move fast and break things' mentality when millions of lives are at stake." Fair point. But here's where it gets interesting. I asked, "Who here wants to learn more about AI in healthcare?" Every hand shot up. The enthusiasm was palpable. These professionals weren't resistant to change. They were hungry for knowledge. So, I flipped the script: "What if AI could help you move carefully and fix things?" Silence. Then a flood of questions. They asked how AI could: - Automate routine tasks, to free up clinicians to spend more time with patients - Improve diagnostic accuracy - Streamline administrative processes — for doctors, RCM, and patient access teams But the key? Safety and security at every step. I shared our approach: - Rigorous testing protocols (thanks to our CTO's background in self-driving cars) - Always-on human oversight - HIPAA compliance, HITRUST certification, and end-to-end encryption The skepticism turned to intrigue. This interaction showed me that healthcare innovation requires a delicate balance. We need to address people's concerns while showcasing possibilities. The future of healthcare isn't just about new technology. It's about building trust, ensuring safety, and keeping sight of what matters most: the patients we serve.

Latest #PitchBook data for #HealthIT investment shows a preference for later stage investment across healthIT that pre-dates the VC downturn and current interest rate environment. Even though healthIT has been more resilient to current headwinds compared to health care services and PPM plays, perhaps it speaks to a need to scale solutions and points of saturation for new entrants. "...In the lower middle market, we are beginning to see more examples of a phenomenon we have anticipated for a while: PE firms and PE-backed companies acquiring VC-backed startups. Lower-middle-market PE firms perennially complain that VC- backed companies expect too-high multiples, and these firms would generally prefer to invest on a multiple of EBITDA rather than a multiple of revenue..."
