CM is a type of behavioral therapy in which individuals are ‘reinforced’, or rewarded, for evidence of positive behavioral change. These interventions have been widely tested and evaluated in the context of substance misuse and treatment attendance, and they most often involve provision of monetary-based reinforcers for submission of drug-negative urine specimens. Peer support is becoming a fundamental part of mental health services in states like Massachusetts. Young adult peer mentors or certified peer specialists provide young patients a chance to build connections with someone who has shared a similar experience. Whether people are struggling with depression, anxiety, or BPD, it's overwhelming to feel alone. Despite the benefits of clinical work, mental health care settings realize that there is a real benefit to allowing patients to experience non-traditional methods of care. Dr. Ken Duckworth of the National Alliance on Mental Health comments: "There’s no big effort to create more psychiatrists, psychologists, and social workers. Behavior is comparable to disease in driving distress, illness, and dysfunction in primary care patients. Behavioral clinicians have solutions, but few are employed in the setting. We need to rectify this and prioritize behavior change in primary care. Countless patients need behavioral interventions, and clinicians today are making an impact in brief (20-minute) visits. Dysfunctional behavior exacerbates chronic diseases like diabetes and depression. For example, patients often fail to take helpful medications for many reasons. Dysfunctional behavior ranges in complexity and magnitude, impacting physical and emotional health. Behavior merits attention as a unique clinical domain, not just for contributing to many disease processes. Traditional solutions are inadequate.  More behavioral training will not help PCPs succeed, and models like Collaborative Care cannot scale to meet the need. If behavior is as fundamental as disease in determining health and illness, we must reconfigure primary care and make behavior specialists routinely available. Over time, behavioral clinicians will be viewed as co-leaders of primary care teams, not simply adjunctive staff. Their accountability for the ubiquitous domain of behavior makes them essential clinical partners to PCPs. Each domain’s importance is undiminished by the other. The Pennsylvania Department of Human Services, Office of Mental Health and Substance Abuse Services released this past April 2nd, its revised Restraint and Seclusion policy for its six state hospitals and two forensic centers. This release updated the 2009 state policy that prohibits the use of mechanical restraint, seclusion and exclusion. Physical restraint use is now limited to no more than three minutes in duration and prohibits any type of floor control, (physical restraint in the supine or prone position). PRN orders for psych indications, by 2005 policy, are no longer used in the hospital system and are unaffected by this change. While the hospitals and forensic centers ended, on their own, the use of mechanical retraint and seclusion back in 2015, this signifcant and momentous change represents the first time, that I know of, where any large system of care in North America has issued policy ending the use of these harmful and traumatizing procedures. Beyond Clinical Walls: New Approaches in Mental Health Care Mental health services often fail marginalized communities, highlighting the need for inclusive care models. Peer support and community health worker programs are effective but underutilized in clinical settings. Combining community wisdom and evidence-based practices can enhance mental health support and preventive care. Reflecting on her journey, a child psychiatrist shared her personal and professional experiences, emphasizing the importance of community in mental health care. She recognized early signs of anxiety and depression in herself, which were managed through therapy and medication. However, the traditional medical model, with its long-term therapy requirements and medication focus, often falls short. To bridge these gaps, she advocates for integrating community-based programs into mental health services. Peer-to-peer support, credible messengers, and community health workers have proven effective but need wider implementation.
