Just back from the really stimulating 2024 Global Symposium on Health Systems Research #HSR2024. Five reflections! 1)     Social science informed approaches are critical for understanding health systems’ context, complexity, and people. The frontiers of this thinking – galvanized by the HSG SHAPES TWG (do join!) and well represented at the symposium – are generating important prescriptions for the how and the what of Health System Strengthening (HSS). A good example is the emerging class of interventions targeting how leadership is enacted and distributed (shout out to the pioneering work of Pr.Dr. Zakaria Belrhiti (PhD), Helen Schneider, and notable others). These insights and evidence are woefully under-socialized across the broader HSS field, yet they need to be infused in the next generation of HSS reforms and investments. 2)     The deep learning partnerships between academics and government practitioners in the Western Cape continues to drive forward the conceptual and empirical frontiers of HSS. H/t to how Oupa S. Motshweneng promoted distributed leadership and psychological safety in his district to ensure more than empty compliance with the National Patient Safety Initiative. Very inspiring! 3)     Strengthening PHC remains a top priority, but this needs to be complexity-informed and comprehensive. I really enjoyed chairing a session on community-based mental health interventions, with excellent and thoughtful presentations from Myanmar (Zay Yar Swe), Liberia and DRC (Dr Maaike Seekles), and South Africa (Andre Janse Van Rensburg). Mental health is a classic example of why a comprehensive approach to PHC is needed – with wickedly complex relationships with the social determinants of health, and the need to mobilize community assets and resources building upon existing structures. 4)     There were too many excellent, thought provoking presentations to acknowledge, but four that are living rent free in my head: Patience Afulani on the Caring for Providers to Improve Patient Experience (CPIPE) intervention – a rare example of a successful, tangible intervention addressing some of the work-climate reasons why providers treat patients disrespectfully (such as stress, burnout, and bias); Dimitri Renmans on the interface between Artificial Intelligence and health systems (and using AI itself to explore this interface); a challenge to how we think about the social determinants of health by Professor Chelsea Watego (identifying the structural drivers of “wrong” behaviour is an inappropriate starting point); and the use of co-production techniques to address illicit behaviors such as absenteeism Dina Balabanova Pamela Ogbozor. 5)     Nagasaki, with its inspirational contribution to the peace movement (and 2024 Nobel Peace Prize) and story of resilience was a great choice of host and delivered HSR2024 brilliantly. The #artificialIntelligence (AI) legal and regulatory landscape is rapidly evolving, as federal and state policy makers work to determine how to appropriately regulate #AI, balancing its risks with the tremendous opportunity it brings. Do you have a plan? Environmental Public Health is uniquely able to provide the first and best proof of value for AI use in public health. The permits, inspections, and related documents are public records, making them great training data for an LLM. Add in all the regulations, research, and training materials every local EH operation has and the ability to keep the public informed is unparalleled in any other field of public health. If we work with the industry, like adding IOT devices to see real time temperatures and hand washing practices, the AI systems could be a food manager alert system between inspections and support a food safety culture. Alternately, building a machine learning tool with image recognition could make onsite sewage system inspections faster and more reliable. This is just scratching the surface. make a plan to participate as this technology evolves. It will be ubiquitous within this decade.

📢 Roundtable in Bishkek: Leveraging Data-Driven Evidence to Inform Diabetes Policy Decisions in #Kyrgyzstan Today, November 5, 2024, our partner the Health Policy Analysis Center hosted an impactful roundtable titled "Leveraging Data-Driven Evidence to Inform Policy Decisions on Diabetes in Kyrgyzstan." This high-level discussion brought together a diverse group of stakeholders to examine critical data on #diabetes and explore ways to embed evidence-based insights into policy frameworks. Taking place just nine days before #WorldDiabetesDay, this event underscored the urgency of aligning research with policy actions to improve health outcomes for people living with diabetes in Kyrgyzstan. Recently I read the terrific "AI Revolution in Medicine" by Carey Goldberg, Isaac Kohane, and Peter Lee. The book attempted to look at GPT-4 honestly to see its capabilities, but also its limitations and failures in its application to healthcare. For me, the verbatim prompts and outputs in a sort of role play from the perspectives of patients, clinicians, and clerical staff was incredibly fascinating and exciting to see.  Yes, of course, machine learning tools are imperfect, what we see now is just the beginning. I agree that no matter what roles AI plays in healthcare, humans will always be necessary to review its output and to maintain its systems. As medical experts, we need to be enthusiastic and thoughtful collaborators with our computer engineering and tech colleagues. We already know what happened when clinicians were underrepresented in the development and implementation of EMR, and we see every day what happens with patients navigating Dr. Google and the endless stream of scientific health misinformation online. Perhaps my favorite quote in the book comes from Dr. Jorge Rodriguez who said “If GPT-4 is as game-changing as it seems, the first question for how to use it should be ‘Who needs the most help in healthcare?’ Ideally technology developers would say ‘This time, we’re going to make sure that marginalized communities are put first’” I truly believe this question should guide our decisions and priorities on this incredible and exciting journey.

In 2015, countries committed to limit global warming to “well below 2°C” as part of the landmark Paris Agreement.
