M13 is thrilled to lead the Series A in RadiantGraph which is focused on personalization in healthcare. As we continue to invest in early stage companies it is abundantly clear that the biggest indicator of success is the founder and his or her ability to execute on their vision of the future. In this case, I don't think I have seen much better founder-market fit than we have with founder and CEO Anmol Madan. We believe in his vision: in order to have the best patient outcomes in healthcare we need increased personalization and AI is the breakthrough to get us there. Anmol started one of the pioneering digital health companies Ginger and then was Chief Data Scientist at Livongo and Teledoc. He has done it all: early stage, late stage, public company. And he deeply understands the nuances of the healthcare market from all angles which is critical for success. While there are many players building healthcare-specific AI capabilities, RadiantGraph is focused on outcomes: The platform is already processing personalization models for more than 3.5 million people, despite launching just last year. Some VC investors believe that brick and mortar - referring to a physical presence of some kind - should be avoided at all costs. Yet, the track record of clinic and hybrid care businesses in health care indicates otherwise. ==> One Medical (sold for $3.9 billion to Amazon), Oak Street (bought by CVS for $10.6 billion), Lifestance (IPO at $7.5 billion market cap), and PE-backed Premise Health (acquired by OMERS Private Equity for $1.1 billion) If a clinic is indicated as the optimal way to deliver care for that patient population, there are creative ways to build and operate them. This week Mikal Ventures brings you a 4-week breakdown of the non-US capital market with an emphasis on the Mikal verticals. The 4-week period ending November 25 saw just over 1,300 new companies added, bringing the total to 112,141 companies included in the non-US Health Tech, Med Tech, AI Tech Fin Tech and Ed Tech verticals. Hot take on a lot of early-stage digital health companies out there: You're not a tech company. Most tech-enabled care companies I've seen over the past two years are only tech-enabled in the sense that they use telehealth as their delivery mechanism for care. There are exceptions to this, of course - companies who build computer vision and applied AI tools to better the doctor-patient experience, companies who build tooling around risk scoring and stratification to enable more personalized medicine, companies who build truly novel intakes to better route patients to the appropriate care, etc. These companies have real technology chops and, if they sell it right to the market, can get credit for this from investors. But if you're truly a telehealth provider, you need to realize now that the goal post has shifted dramatically from 2021 and you're going to be valued at maturity at something like 3x EBITDA. That's profit. Not revenue. Throw a revenue multiple out the window. That's not the world we live in if you're a virtual doctor's office. Think of how that impacts venture math. If you're raising a seed round at $15M post-money (or $12M pre-money), it means you'll have to generate something like $150-200M in annual EBITDA to be a fund returner for your seed investors. Maybe even more with dilution, since telehealth businesses are not traditionally capital efficient and will likely need to raise material additional capital to reach that scale. You need to be ridiculously profitable (probably on $800M-$1B in annual revenue since you're likely going to be a 40% gross margin business). Here's the good news: you can set yourself up well (and probably be a contrarian founder relative to your peers), by talking about EBITDA growth and how you can grow into a fund-returning outcome based on realistic mature market comps. This will stand out for the investors you're pitching, even at the early stage. This is how you can beat the malaise of the tech-enabled care market. Focus on the right metrics and the right type of growth. You don't have to have all the answers (no founder does), but you need to make sure the ship is headed in the right direction. A focus on profitability for "tech-enabled care" (*ahem* telehealth) businesses is a good first step.

Hot off the pre-print server – an important publication authored by the Atropos Health team exploring the performance of LLMs in answering real-world clinical questions. LLMs excel at summarizing and contextualizing existing literature, but cannot generate de novo medical evidence today. The results of the study highlight performance gains attributable to injecting novel, relevant, and up-to-date medical evidence into LLM-powered clinical decision support systems. Now you can sequence your every cell one by one. Expect that for protein folding (AlphaFold was that moonshot) and other simulation uses, a full "twin" model of you, cell by cell -- Google has no angle here -- worse chips, worse cloud, worse models, worse phones / edge devices -- Specialty silicon for unique use-cases, lower-power, lower-memory are surely coming fast - like back in the day for mobile phones when Qualcomm, Broadcomm, Infineon, Mediatek, etc duked it out.

After the first deep dive discussion w/ founders I often hear a comment "I wish I had heard about this alternative [to all-in must-become-a-unicorn] way to build a startup earlier as I have wasted years in pursuing the wrong thing". Our mission is that no founder would ever need to say that again as it is such a waste. Us preaching the gospel 1:1 just doesn't scale, hence we use mass media to multiply our efforts - that's how Gorillacast came to be. The 1st episode is out, featuring yours truly. "How can you know what food you like unless you taste everything" said a chef. How can founders make informed choices about the best strategy for them to follow unless they know of all alternatives? This might not be of interest to you but pls spread the word so that the founders who should hear it can find it. I have a dream!

And my second investment at defy is in 🐶 zeroETL puppies 🐶... I mean PuppyGraph, which is all the beauty of a graph database, none of the data migration or continuous upkeep of a graph database. Congrats to Eniac Ventures seeded Nirvana on their big A! 🚀 "Nirvana, a startup aiming to give health care providers and consumers the price of treatment before an appointment, raised $24.2 million in Series A funding, it tells Axios exclusively. Why it matters: The raise comes amid a push for greater transparency in health care bills, with one new government rule put into effect in 2021 and more being considered today. What they're saying: "In some weird behavioral way, people were used to the fact that you would sit in a waiting room for six hours and get the bill six weeks later," says Nirvana CPO Kelvin K Chan. "Looking back, that's incredibly strange and archaic, but I think that mentality is starting to shift." How it works: Embedded into health care providers' websites, the software reduces the chances that an insurance claim will be denied after the fact. It does so by using the patient's name, date of birth, and ZIP code to verify a consumer's coverage, Nirvana says. Traditionally, providers required the insurance ID card, which the company's executives say is susceptible to human error. Combining that with information from the provider about the consumer's condition, Nirvana offers an estimate of the appointment cost at the time of booking. We are excited to have announced our Series A lead investment in Abett. Founded by Mike Hanlon, the company provides a data layer that enables any self-insured employer in the US to receive a regular and consistent feed of their claims data so they can actually understand healthcare costs, the efficacy of their employee benefits, and how to better serve their employees (to whom they have a fiduciary responsibility). While this sounds like table stakes, it's far from the norm. After payroll, healthcare is the biggest cost consideration for American employers. A majority of Americans receive employer-based health insurance. A majority of those Americans receive insurance from a fully or partially self-funded plan (i.e., the employer is taking risk instead of the insurance company). Abett brings transparency and alignment to a thorny data problem to empower benefits leaders & CFOs. I'm excited to announce that Mindset Care, the platform designed to simplify the application process for Social Security Disability benefits for individuals with mental illnesses and their caregivers, has raised $13 million. Science led the latest funding round and has backed the company since it launched from Science Inc.’s startup studio. We are thrilled to have Wellington Management (Van Jones, Jackson Cummings), Tubbs Ventures, Enable Ventures (Regina "Gina" Kline, Margaret Knowles), XYZ, and LivEdge Capital on board to enable Mindset to meet the growing demand for disability assistance. 17 million Americans live with a serious mental illness and qualify for disability benefits. Yet, it is estimated that about 65% of initial disability applications are denied, often due to errors or incomplete information​. This financing will enable Mindset Care to expand into partnerships with community and large-scale organizations, including local mental health nonprofits, government agencies, and homeless shelters.
