Nikhil Krishnan joked about chat room functionality in the context of value-based care coordination as if it were a pipedream. It was all a dream until a couple of months ago - now it’s real - along with automated value-based care administration. We’ve natively integrated and automated enrollment, authorization, claims, adjudication, repricing & provider reimbursement to remove the bottleneck hindering greater direct-to-employer adoption. Scalable same-day payvider reimbursement is here. This rough demo video shows the functionality in the context of a multi-party surgical bundle ordered and pre-approved (ie codified plan design) by a DPC doc anchoring a level-funded plan. The pre-authorized procedure order triggers automated claims fund withdrawal at predefined direct contract rates stored in the dedicated plan environment in our database. The member chats and shares records with the physician, facility, and anesthesiologist right from the comfort of their personal longitudinal health record in Mishe. The video then cycles through the views from each individual provider’s dashboard where they “redeem” their “voucher” (ie submit their claim) for instant adjudication triggering automated EFT reimbursement. The team’s been HARD at work bringing the value-based surgical bundle administration automation to life. Personally, I've been actively working towards this moment for about 2 years and passively for 10. We're officially a clinically integrated network (CIN) without the admin headaches and look forward to developing unique quality-driven care models with the best clinical advisory board one could ask for. More on this soon. We’re implementing this with surgery centers treating patients from your favorite venture-scale COE companies and look forward to accelerating DTE adoption with the release of our API later this year. 🚀 Upscaling Generative AI as an Enterprise Platform 🥌 In my experience, adopting Generative AI (GenAI) transcends a mere application choice; it represents a strategic enterprise-wide platform decision that demands careful consideration and alignment with organizational goals. As enterprises embark on their GenAI journey, it's vital to treat scaling efforts with the same strategic approach as previous enterprise-wide initiatives, like cloud adoption. To successfully scale GenAI, consider forming a steering committee that brings together cross-functional stakeholders. This committee will oversee strategic planning, resource allocation, and change management, ensuring a holistic approach to adoption. Have you been part of a steering committee for enterprise-wide initiatives? If so, you will relate with this approach. Looking back at cloud adoption a decade ago, we can draw parallels to today's GenAI landscape. Just as organizations needed to re-architect their IT infrastructure and embrace new workflows for cloud integration, scaling GenAI requires a similar mindset shift and strategic planning. We should start by looking at business processes. Engage with stakeholders from various LoB (lines of business)—clinical, IT, and business—to foster cross-functional collaboration and gather diverse input. Just like we had to explain the business benefits of cloud to everyone during cloud adoption, scaling GenAI warrants similar motion. Educate & Communicate. Don't make assumptions. Adopt an agile approach to development, testing, and deployment, allowing for continuous learning and improvement as you scale GenAI applications. There is so much to be done. My preferred execution model is Scaled Agile Framework (SAFe) Program Increment planning. The goal is to align everyone on a shared vision, prioritize features, identify dependencies, and create a detailed plan for the upcoming Program Increment (PI), which is a time box of 8-12 weeks. Lastly, don't forget the importance of data governance, quality and privacy. Address any challenges and comply with regulations to build trust and pave the way for successful adoption. I have witnessed numerous organizations postponing implementing a comprehensive data strategy and robust data governance practices during previous transformation initiatives, such as cloud adoption. Oracle Health announced plans to launch a next-generation EHR platform in 2025, incorporating advanced cloud and AI capabilities. This new EHR will feature Oracle's clinical AI agent, voice-activated navigation, and Health Data Intelligence, which integrates patient data from over 300 sources to enhance clinical workflows and improve patient outcomes. The platform aims to replace traditional EHRs, which often dictate rigid workflows, with a more dynamic system that allows for personalized care plans. Oracle Health emphasizes that the new EHR is a complete reinvention rather than an upgrade of the previous Cerner system. Additionally, Oracle plans to become a Qualified Health Information Network to facilitate standardized data sharing across the healthcare ecosystem. In my years navigating the complex landscape of healthcare AI, from clinical decision support to patient engagement platforms, I've witnessed a stark contrast between boundless potential and cautious reality. The numbers tell a story of hope and hesitation: $370 billion in projected impact, $30 billion invested in AI-focused healthcare companies, yet only 6.8% of organizations planning to implement AI solutions. These aren't just statistics; they represent countless late nights, impassioned debates, and the emotional roller coaster of innovation in a field where lives hang in the balance. I've seen AI's transformative power in clinical trials, where algorithms sifted through data to identify promising drug candidates. I've also felt the frustration of watching brilliant solutions gather dust in marketing, thwarted by regulatory hurdles, entrenched workflows or internal politics. It's bittersweet that while tech startups race ahead, healthcare slogs forward with measured steps, weighed down by its weighty responsibilities. The focus on operational efficiency isn't just a trend—it's a lifeline for an industry drowning in paperwork. I've seen the relief in a nurse's eyes when an AI-powered system freed up hours, allowing focus on patient care. Yet, as we chase measurable ROI, I wonder about the life-saving potential of clinical AI tools left unrealized. The disparity in success rates between AI startups targeting health systems (5% reaching maturity) versus life sciences (10%) or health plans (16%) isn't just a statistic from Flare Capital Partners—it's a testament to the uphill battle. I've stood in meetings with boardrooms, pitching solutions that could revolutionize patient care, only to see decision-makers fixate on the bottom line rather than the horizon of possibility. Despite the obstacles encountered, moments of triumph fuel my passion. I've watched AI-powered tools catch subtle abnormalities in medical images, potentially saving lives through early detection. I've seen patient engagement platforms break down barriers, fostering personalized care at scale once thought impossible. As we stand at the crossroads of AI and healthcare, I'm reminded of why I entered this field: the unwavering belief that technology can alleviate suffering and extend the boundaries of human health. The path forward isn't about chasing the next big investment or flashiest solution. It's about bridging the gap between innovation and implementation, between potential and practice. AI technologies are supercharging the productivity of the U.S. clinical and administrative labor force, a 2T labor segment. LI members explored strategic opportunities with Hemant Taneja and Tanay Tandon who founded/lead Commure, an integrating platform offering a suite of automation services including RCM, ambient clinical copilots, RPM, sensors and operations. Of the $180B spent on RCM and workflows, Commure believes this entire segment should be replaced with $25B-50B in software +ops. One key reason why healthcare technologies like AI, telemedicine, and software haven't structurally lowered costs or broadened access for the entire system after decades of experimentation (excluding pure financial engineering of payer contracts) is the limited reimagining of care delivery itself, with asynchronous telemedicine as a notable exception. In the same way that digitizing physical paper failed to exploit the capabilities of a new computing medium for word processing, hosting physician-patient conversations over video does not transform the actual delivery of care. In telemedicine, many of the inputs (e.g., clinician time) and therefore cost structures of in-person care persist, which is why we often see fee-for-service telemedicine companies struggle to achieve economies of scale (i.e., growing revenues non-linearly while growing costs linearly). Until we deconstruct the components of a medical appointment – from intake and triage, to chart review, history gathering, physical examination, diagnosis, treatment, and follow-up – we will continue to be underwhelmed by the disconnect between the transformative potential of these technologies and real-world patient/system impact. I’m optimistic that the emergence of reliable and performant medical AI agents will urge us to perform this deconstruction and redesign the fundamental building blocks of a “medical appointment.”

> Turning retroactive HEDIS measures into proactive care gaps and increase reimbursements for health systems HEDIS measures are used extensively in healthcare at the end of the year to measure quality of healthcare delivered. 90% of health plans use HEDIS measures. In addition, there are incentives in place where healthcare providers can make (or lose) a lot of money if they meet or miss HEDIS measures. According to a 2021 study by the National Committee for Quality Assurance (NCQA), approximately 80-90% of healthcare dollars in the United States are tied to quality measures, including those from the Healthcare Effectiveness Data and Information Set (HEDIS). While looking at how your health system performed at the end of the year is valuable, what if you could proactively find the patients who will miss the HEDIS measure and address their care gap BEFORE the end of the year? With the b.well partnership with NCQA, now you can! The b.well platform now automatically generates proactive care gaps for each patient using the HEDIS official calculations. Patients get notified automatically to book appointments to come in and resolve those care gaps.
