It's #NationalHospitalWeek which provides an opportunity to highlight the innovative ways that hospitals and health systems are supporting their communities. "A one-year analysis of 21 Kaiser Permanente Northern California Oncology practices presented at the 2023 American Society of Clinical Oncology Annual Meeting found that each of its oncologists respond to an average of 19 secure patient-physician portal messages per day, including weekends (Abstract 1503). The unseen value of these messages was estimated to be more than $11 million, including roughly $3.8 million in avoided out-of-pocket patent copayment costs and $7.7 million in missed billing. Major cancer centers are increasingly billing for some nonurgent patient portal messages, including Johns Hopkins Medicine, Michigan Medicine, Cleveland Clinic, and the Fred Hutchinson Cancer Center. In addressing the ethics of billing, Dr. Nazha said that health systems need to create an approach that, “1) is efficient and equitable, 2) ensures patients receive valuable and timely access to medical services, and 3) protects the well-being of healthcare providers.”" https://lnkd.in/eYgsKy_x

Congratulations to Rhafia Bucoy, DBA, MSN, RN, RN-BC, Regional Clinical Practice Consultant, Perioperative Services, NCAL Kaiser Permanente, recipient of The DAISY Foundation DAISY Nurse Leader Award.   Rhafia Bucoy is NCAL Perioperative Services’ clinical practice and financial performance consultant. She serves as an OR nurse informaticist and analyst, and she is definitely everyone’s “go-to person” for everything Perioperative data, clinical staffing, OpTime build, and all other things. Rhafia is the nurse in the background who silently gets things done, from report build to data mining, analysis, and presentations. She is the clinical liaison to the national teams for KPHC, national product council, and revenue cycle. She is responsible for the clinical testing and auditing of electronic med record documentation.   Despite the overwhelming demand for reports and other competing priorities, Rhafia manages them well and always willingly helps others. She has collaborated with revenue cycle in closing out unposted surgical logs and ensuring accurate implant documentation. She has also recently agreed to close out records related to bedside procedure recovery to ensure accurate capture of workload and patient throughout in the post operative space.   Rhafia serves as the clinical lead and resource for all of NCAL’s surgical system administrators as she lends her expert consulting hat in the OpTime domain from case creation to case closure and posting. Rhafia partnered with supply chain on the implementation of Case Pick App in 19 NCAL facilities, 11 of which went live in 2023. This application helps accurate accounting of supplies used in each surgical case leading to cost valuation and eventual supply standardization. With a few more sites going live in 2024, procedure card maintenance is key and Rhafia recognized this gap in some locations. With this information, she took the initiative to reinvigorate and develop training modules on procedure card updates. She partnered with the national training team and launched the first facilitator-led training in January of 2024.   Another important work that Rhafia is leading is the implementation of the Preop and PACU bay assignment to capture both medical and surgical workload in the perioperative spaces to inform staffing decisions. Her collaboration with the workforce and finance teams is critical in this important work.   Rhafia is a self-directed and a silent nurse leader in the Perioperative space. Her contributions to KP NCAL nursing can easily go unnoticed but definitely worthy of a recognition. Join Us for a Cancer Registry Listening Session    ASTP/ONC and NCI invites you to participate in an upcoming listening session focused on the Cancer Registry use case and USCDI+ Cancer Registry data element list published on July 23rd. This session will provide an opportunity to discuss the importance and ease of collection of the data elements, the potential burden of sharing them electronically, and the impact on the future flow of data as we gather feedback from stakeholders like you.    Date:  August 29, 2024  Time:  2 – 3pm ET  Location: Online   Cancer Registry Overview    The scope of this use case includes the Surveillance, Epidemiology, and End Results (SEER) program, the Centers for Disease Control and Prevention/National Program of Cancer Registries (CDC/NPCR) state cancer registries, and the cancer registry standards organization, North American Association of Central Cancer Registries (NAACCR). Together, these entities play a critical role in understanding the burden of cancer in the US related to cancer incidence, first course of treatment patterns, mortality, and survival.    Currently, there are delays in obtaining data from these sources, with available data often up to 3 years behind the current year. The objective of this use case is to enhance the efficiency and timeliness of the collection of cancer registry data by identifying standards (e.g., FHIR, mCODE, etc.) to efficiently extract and/or collect cancer registry data directly from EHRs and pathology labs at a sufficient level of granularity. This data must be interoperable with the cancer clinical, public health, and research communities. The USCDI+ Cancer use case will define the data standards and models needed for identifying and extracting required data, as well as support current data sharing and linkage approaches for cancer registry data via SEER and CDC/NPCR.    With the advent of real-time reporting, capturing a minimum dataset through a FHIR query as early as possible is crucial. This dataset must meet certain quality standards to make the information usable.    This listening session will cover the cancer registry data flow and the points of exchange of data, which are critical for understanding and optimizing the interoperability and timeliness of data sharing. #Hospitals and health systems are facing mounting #AdministrativeCosts due to the growing #AdministrativeBurden of #PriorAuthorization and #Denials by commercial health insurers. If you don't believe me, the American Hospital Association reported that hospitals’ labor costs alone increased by more than $42.5 billion between 2021 and 2023. That's nearly $6M per hospital in additional costs. Additionally, the overall administrative costs in the U.S. healthcare system are estimated to be around $496 billion annually! This burden makes us the most wasteful healthcare system in the world. These rising administrative burdens are unsustainable. Period.

This was a thought provoking article that I hope sparks more conversation. There were a lot of points, but these three points I walked away with coincide with conversations I have had regarding hospital-based nursing. 1. While not explicit, how we keep the nursing workforce prepared beyond orientation? 2. We should focus more on harm avoidance instead of mostly harm caused when quantifying nursing outcomes. 3. To deliver excellent care, nurses need a supportive practice environment. What are your thoughts?
