Last but not least at Becker's Healthcare CEO/CFO conference - terrific panel led by my ECG Management Consultants colleague Steve McMillen, including Andrew Chang from UChicago Medicine, Patrick Cockey from Jefferson Health, and Maura Buxton from CVS Health. Topic: The Digital Patient Experience. Fascinating discussion in which the panelists agreed on many things, but productively disagreed on some topics. These are my favorite panel discussions, where constructive disagreement fosters truly engaging dialogue. Some of my takeaways (not attributed to any individual member of the panel): (1) The term "digital front door" can imply that we want everyone to begin their journey with us on a device or through an app; we really should be talking about making every channel accessible and seamless. (2) Improving patient experience is necessary but not sufficient; any solution should also simplify provider workflow. (3) We have a lot to learn from our existing patients and consumers, but also much to learn from those who choose NOT to engage with us digitally. #patientexperience #leadership #healthcare

Excited to share this new JAMA study with David Cutler, Karen Joynt Maddox, and my McKinsey colleagues which estimates that more than a quarter of the care delivered in the hospital setting today could be shifted to nonhospital sites without compromising clinical outcomes based on the view of 1000+ clinicians. This shifted volume, when extrapolated out across US healthcare system, represents a potential net savings opportunity of $113.8B to $147.7B. For health systems already facing compressing margins due to inflation and labor shortages, this represents another long-term headwind to manage. Risant Health, a new entity formed by Kaiser Permanente in 2023, is expanding its value-based care network with a strategic acquisition of Cone Health, a nonprofit healthcare delivery system in North Carolina.    This move echoes Risant Health’s earlier acquisition of Geisinger in April, underscoring its commitment to growing its value-based care network and expanding access to evidence-based healthcare for all populations. Like Geisinger, Cone Health will keep its decision-making autonomy and maintain its brand. As an organization within the portfolio, Cone Health will receive centralized support from Risant Health to enhance its operations and implement best practices that are established for the network.    Despite the need for value-based care, the industry has seen only a targeted shift away from fee-for-service models. CNIO and Health IT colleagues from across the country gathered at The Health Management Academy CNIO Fall Forum. A great opportunity to reconnect, share and compare experiences about new and innovative approaches to our common challenges. My take-aways from our discussions: -         Informatics: work continues on role clarity and incorporating scope and standards of nursing informatics. Recognizing non-nursing informatics expertise and supporting their certification in areas such as Respiratory Therapy is an ongoing challenge. Informatics is more than the EHR implementations and tasks; innovation, supporting practice, ensuring a team approach and using data to guide daily clinical and administrative work is the future. -         AI Governance is in its infancy: Katie Barr provided a great description of what Advocate is doing-impressive! The continued challenge of clinicians not documenting in real time will impact what clinical insight AI can provide. What can AI do to reduce the administrative burden for nursing? -         Ambient technology: the development for nursing use cases is lagging as developers struggle with nursing workflows resulting in solutions based on a provider’s workflow which don't match a nurse's. More work is needed in this area. -         Documentation burden: standardizing and streamlining are important. AND so are non-IT solutions include challenging the burden that regulatory and compliance requirements have placed on the workforce. To reduce the burden, the requirements must change. Several noted that we can’t use documentation to prove the clinicians ‘did their job’. -         Training: the contention between ensuring competence and reducing non-productive time spent in training continues. AI may help. Some noted that teaching specific documentation should be incorporated into the clinical precepting/education and to stop treating orientation to the EHR as something 'separate' from the clinical work. One colleague commented that there was no class teaching nurses how to document in the fancy trifolds of days gone by… -         Business Continuity: “Change Management is the greatest challenge” Jennifer Jones, MSN, RN noted as she kicked off this lively session. Organizations must prepare to keep the business running and care delivered; this has moved beyond simply planning for a ‘downtime’ of a few hours. How can adjacent hospital systems support each other? How can they quickly pivot to support access to data across systems needed for ongoing patient care in the event of an outage? Cyber and climate events have changed the landscape and require systems to rethink their preparedness and their partnerships. Lots to consider and we all came away richer for time spent together.
