It is our second poll..... The question was; are you required to have a National Provider Identification number (NPI) to submit claims to any type of insurance payer. The answer is YES, you do! Also keep in mind that if you are billing under a group name, you must have a valid NPI number for the group as well as each rendering provider within the group. Both the group NPI and the rendering provider's NPI must be properly listed on the claim.
When working with insurance payers there are requirements you must meet in order to ensure payment on your claims. What is required when it comes to obtaining a NPI number? Do you know? (Check back in one week for the answer)

Medical notes being done correctly are crucial! Do you know what is required in your medical notes? Are there any differences in information that must be contained in your medical notes if you do not take insurance? Does your medical notes need to be different for each payer type if you do accept insurance? These are all great questions to ask as a medical provider whether you work with insurance payers or you do not. Having accurate, complete medical notes ensures that treatment is progressing as outlined and agreed to in your treatment plan for the patient/client (you should have a treatment plan too!). It also ensures that in the event you should receive a request for medical notes to prove medical necessity from an insurance payer, then responding is as easy as 1-2-3. What is crucial information that must be present in your medical notes; 1) patient's name and date of birth 2) reason for the session/treatment 3) related diagnosis being dealt with 3) notation of any progress made in treatment 4) time and length of the session/treatment 5) any tools used during treatment to assist with treatment 6) any one else present during treatment (i.e. spouse, parent, etc.) 6) treating provider's name 7) provider's signature 8) date of signature Keep in mind that initial assessments require much more details due to past history and other information that needs to be gathered during the assessment that then allows for creation of the treatment plan. Medical notes should be uniform across ALL payers. Medical notes also are the same regardless if you accept insurance or not. Making sure that you have all of the required information in your medical notes will ensure that you are also meeting the requirements under your contract with payers too. Be ready to produce copies of signed and dated medical notes when requested by the payer to prove MEDICAL NECESSITY. Failure to have accurate and through medical notes will result in claims being reversed and payments recouped by payers. ATTN: Practice Owners and Managers Medicare and Medicaid re-validation......have you completed yours yet? After becoming an enrolled/contracted Medicare or Medicaid provider regards what state you practice in, you will be required to become re-credentialed every 3-5 years. If you are practicing and thereby contracted under a group name and Tax ID number, remember that your re-validation will involve two applications, one for the group and one for each rendering provider within the group. Failure to complete one of the two REQUIRED applications could result in involuntary dissenrollment which will lead to declining payment on claims.
