Healthcare data integration is notoriously expensive and complex. The landscape is dotted with legacy systems, obscure data formats like HL7v2 and FHIR, and diverse, cross-organizational stakeholders, each with their own unique needs and requirements. This complexity isn't just technical; it's foundational, rooted in the very evolution of healthcare IT. Decades-old systems, designed long before the dawn of the internet era, are still in active use, posing significant challenges in terms of interoperability and data standardization. The solution presented by the industry incumbents is more bespoke integration standards - the belief being if there was just a better, more specific standard for data exchange things would be better. Navigating the Complex World of Lab Result Queries with HL7 v2 As GDPR privacy legislation tightens the flow of sensitive data across systems, laboratories can no longer simply send lab results to all systems by default. Combined with the increased commercialization and outsourcing of laboratories—resulting in the loss of access to lab priors—there’s a growing demand for more efficient and standardized v2 Lab Result Queries. Historically, QRY^R02 and ORF^R04 have been commonly used for these queries. However, HL7 v2, while powerful, supports a wide array of variations, often leading to complexity. A potential path forward is adapting the FHIR query mechanism to v2 workflows, leading to the use of QBP^Z89, which allows for a more standardized, limited set of server-side parameters—closely mirroring the parameters used for Observations in FHIR.
