In this post, Research Analyst Ciannah Correa, MPH, explains how Holly Koehler, VP of Access, and Michele Pirog, Senior Director of Operations, developed a program that is revolutionizing patient care transitions. Rather than leaving discharged patients and caregivers to navigate complex follow-up appointments alone, Northwell Health's program provides structured support to coordinate post-discharge care. This initiative has achieved a remarkable 11% non-attendance rate—far below the industry average of 50%."
