We are growing. Earlier this year, we signed a partnership with the United Nations Population Fund (UNFPA) Country Office in the #KyrgyzRepublic to help improve access to quality maternal health care and promote innovative approaches in the field of reproductive health. The project is being implemented within the framework of UN South-South Cooperation, and targets five maternity hospitals– one maternity at the tertiary level (the National Center of Maternal and Child Health based in Bishkek) that will be equipped to provide supportive supervision through tele networking to four maternity hospitals at the secondary level in three provinces, namely Osh, Jalal-Abad, Issyk-Kul and Chui. These hospitals cover a large catchment area bearing a disproportionate burden of maternal and newborn mortality and morbidity. The overall goal of the project is to reduce preventable maternal and neonatal mortality and morbidity in selected regions of the country. Lessons from this pilot project will be used to further scale-up sustainable telemedicine practices. #Lastmile #Impact #Jharkhand #NCDs #NCDs like #diabetes are on the rise. As per the World Health Organization, in India alone, there are approximately 77 million people above the age of 18 years who are suffering from diabetes (type 2) and nearly 25 million who are pre-diabetics– a higher risk of developing diabetes in near future. Given the shortage of #accessible #healthcare in many parts of the country, more than 50% of diabetics are unaware of their status which can lead to various #health complications. Swarno Devi had been living with diabetes for two years. She resides in Serenghatu village in Ranchi, #Jharkhand. She used to buy her medication from a private hospital in Bundu, a small town 20 kms from her village. The medicines were not cheap nor was the travel. In early 2023, she ran out of the medicine and wasn’t able to afford the travel or the medication. As she discovered later, within two weeks of not taking the medicine, her blood sugar levels had spiked dangerously high. She was desperate for help. Her neighbours told her about the #CommunityHealthWorker (#CHO) Sunita Kumari working at the Health and Wellness Centre (#HWC) in the village. Swarno Devi reached out to Sunita and shared about her condition. The CHO immediately arranged a video call with a doctor in Ranchi city. The doctor asked her detailed questions about her condition and after listening to her health complaints, prescribed the necessary medication. Post the call, Swarno Devi didn’t have to go anywhere to buy the #medicine. Sunita promptly provided her with the prescribed medication. Swarno Devi could not believe that it was all so easy. It was a huge relief. The regular follow-up visits to the HWC allowed her to #monitor her condition and receive ongoing #support from the CHO. Today the village does not need to travel anywhere else for various health checkups. They are able to save a great deal of money and time as well as receive #qualityhealthcare services right at their doorstep. #Technology has always had the potential to be used as a tool for greater good and through #telemedicine, it is helping bring life-saving changes at the #last mile.

As per the most recent estimates about India, active healthcare providers density of doctors and nurses/midwives is 5.0 and 6.0 respectively, for every 10,000 persons. This situation is shared by many low-resource economies. Healthcare quality faces significant challenges. This results in brief primary care consultations, averaging just 2.5 minutes, during which only 16% of essential history-taking tasks are completed. Consequently, medical errors are prevalent, with an estimated 5.2 million errors occurring annually in India, though the actual number may be higher. How can AI, specifically Clinical Decision Support Systems (CDSS), enhance healthcare accessibility, quality, and efficiency? Can evidence-based support systems help healthcare providers and patients overcome such challenges? How can careful integration of Large Language Models (LLMs) into telemedicine platforms be of help? Can technologies like GPT-4.0 Turbo actually help increase the diagnostic accuracy, particularly with complex multi-system diseases? What are the challenges that remain? #Ayu is a #CDSS designed by #Intelehealth to assist community health workers (CHWs) and nurses in collecting clinical information based on patient needs and symptoms. It synthesises this information into a clinical summary, generates likely diagnoses, and makes triage decisions at a physician's level. Ayu supports interactions in local Indic languages, facilitating communication between health workers and patients. We will be sharing our learnings about all of this at #GDHF2024, led by Dr. Soumyadipta Acharya, Dr Neha Verma and Dr Shekhar Waikar on the 4th of December in a virtual workshop.
