Afreen Pappa, MD serves as Medical Director of all 10 Innovista Medical Center locations in Dallas and Houston. Deeply committed to the principles of value-based care, Dr. Pappa is passionate about ensuring excellent patient experience and health outcomes across all centers. One of the most important ways we accomplish this is through annual wellness visits for our 65+ population. AWVs are the cornerstone to value-based care, ensuring care gaps are closed and chronic conditions are caught and managed before they become more problematic. At our value-based care symposium in November, Dr. Pappa led an in-depth and informative session for our providers on the ins and outs of AWVs—who is eligible, who can conduct them, what they entail, and more. Thank you, Dr. Pappa, for your commitment to ensuring high-quality care for our patients! #valuebasedcare #AWV

Our incredible COO Mia Willhite, MBA, MHA, JM recently led an inspiring session with our providers on ways to drive organizational success with value-based care. Here are a few of Mia’s transformative insights: 1. Alignment is everything. Connecting individual providers’ work to organization goals is a crucial step. 2. Provider engagement is a precursor to that alignment — and to strong performance in value-based care. 2. Value-based care is so much more than a concept. It’s a strategic approach to patient-centered care requiring specific, implementable strategies and continuous improvement. Mia’s message resonated with our providers: We’re not just delivering health care, we’re creating a journey together — one that patients, providers, payers, and the team at Innovista all take together. Creating alignment between these groups means we're building a collaborative path to value-based care excellence. Thanks, Mia, for helping lead the way! #valuebasedcare

So much of the discourse surrounding value-based care focuses on primary care. And for good reason — primary care significantly impacts overall health outcomes and total cost of care. But the next frontier, according to the Healthcare Transformation Task Force, is specialty care. Their latest report points out that specialty care drives a substantial portion of healthcare spending, making it crucial for achieving VBC goals. We're excited about this new focus. Integrating specialties into VBC models is essential for creating a truly comprehensive, patient-centered healthcare ecosystem. We look forward to supporting and implementing the Task Force's insights — and continuing to push the boundaries of value-based care in both primary and specialty settings. We're inspired by industry leaders like DaVita's Jeff Giullian, MD MBA, who are leveraging value-based models to revolutionize kidney care. Currenly, DaVita has $5 billion at risk in value-based contracts and 80,000 patients benefiting from value-based programs. Their initatives mirror our commitment to: - Reducing care fragmentation - Enhancing the patient experience - Addressing social drivers of health - Balancing the benefits of technology with compassionate, human-centered care Dr. Giullian's emphasis on these points resonate deeply with our mission. At Innovista Health, we're committed to leveraging innovation while never losing sight of the personal, compassionate care people deserve. Major kudos to the team at Innovista Medical Center's Mesquite, TX location! This extraordinary group screened nearly 60 patients for diabetic retinopathy last week, helping close gaps for those at risk. Left undiagnosed and untreated, diabetic retinopathy can lead to blindness. Our teams at Innovista Medical Center give people the resources they need to *prevent* major health issues before they occur — in addition to providing high quality care. That's what value-based care is all about. Thanks, team!

There's new data out on CMS's Oncology Care Model (OCM) evaluation. Here's what you need to know: 1. Impact of two-sided risk: Practices that engaged in two-sided risk achieved net Medicare savings of $299.6 million. These practices not only excelled financially but also met the highest quality thresholds. 2. Quality vs. cost efficiency: While OCM successfully reduced costs, it fell short of improving overall care quality. The model primarily succeeded in lowering payments for higher-risk cancer types by promoting the use of more cost-effective supportive care drugs. 3. Future directions: The findings emphasize the potential for future models, such as the Enhancing Oncology Model (EOM) to build on OCM's lessons and improve qualtiy and cost efficiency in oncology care. At Innovista Health, we're committed to helping you keep tabs on all the ways value-based care transformation is going right — and how we can work together to make it even better.
